Abstract
Amacımız sleeve gastrektomi (SG) uygulanan obez hastalarda ve kontrolgrubunda operasyon öncesi, postop birinci gün ve oral beslenme sonrası lipoprotein altgrup analizlerini, kolesterol ester transfer proteini (CETP) ve aktivitesini, lesitinkolesterol açil transferaz (LCAT) seviyelerini ve aktivitesini, apolipoprotein A-I (apoA-I), apolipoprotein B (apo B) ve paraoksonaz (PON) seviyelerini tespit etmek, eldeedilen bilgiler ışığında SGnin erken dönemde revers kolesterol transportuna olanetkisini incelemekti.Serum CETP, LCAT, apo A-I ve apo B seviyeleri ölçümü ELİSA yöntemiyle,LDL ve HDL alt grup analizleri, Lipoprint sistemi kullanılarak disk poliakrilamid jelelektroforez yöntemi ile yapıldı. Paraoksonaz seviyeleri manuel olarak, CETP veLCAT aktiviteleri ise florometrik metodla ölçüldü. Total kolesterol (TK), glukoz,HDL ve trigliserit enzimatik kolorimetrik metodla, insülin immünassay metodla, LDLFriedewald formülüyle, insülin direnci HOMA-IR ile belirlendi.Hasta grubunda HDL-1 ve HDL-büyük alt gruplarında postop birinci gündepreop döneme göre anlamlı bir artış HDL-8, HDL- 9, HDL-10 ve HDL-küçük altgruplarında, TK, LDL ve CETP düzeylerinde postop birinci günde preop döneme göreanlamlı bir azalma görülmüştür. Trigliserit ve VLDL düzeyleri oral beslenme sonrasıpostop birinci güne göre anlamlı şekilde artış göstermiştir. Hasta grubunda oralbeslenme sonrasında preop döneme göre PON, insülin ve HOMA-IR seviyelerindeanlamlı bir azalma görülmüştür. Hasta grubunda oral beslenme sonrası hem preophem de postop birinci güne göre HDL düzeylerinde anlamlı azalma ve apo A-Iseviyelerinde anlamlı artış görülmüştür.Sleeve gastrektomi sonrası erken dönemde CETP seviyelerindeki azalmayabağlı olarak anti-aterojenik özellikteki HDL alt grupları artmış, aterojenik özelliktekiHDL alt grupları ise azalmıştır. Ayrıca SG sonrası, hem insülin direncindekiiyileşmeye hem de HDL klirensindeki artışa bağlı olarak apo A-I seviyelerindeyükselme görülmüştür. The aim of this study was to evaluate the lipoprotein subfractions, cholesterylester transfer protein (LCAT) and activity, lecithin cholesterol acyltransferase (LCAT)and activity, apolipoprotein A-I (apo A-I), apolipoprotein B (apo B) and paraoxonaselevels in obes patients underwent sleeve gastrectomy (SG) and in control grouppreoperatively, at 1st day after surgery and after oral feeding, and to investigate theeffect of these parameters in the revers cholesterol transport.Serum levels of CETP, LCAT apo A-I and apo B were determined by ELISA.CETP and LCAT activity were measured by fluorometric method. The activity ofPON was assessed from the rate of enzymatic hydrolysis of phenyl acetate to phenolformation. HDL and LDL subfractions analysis were done by continuous discpolyacrylamide gel electrophoresis. Total cholesterol (TC), glucose, HDL andtriglyceride (TG) were assessed by enzymatic colometric method. Levels of insulinwas evaluated by immunoassay method, LDL by Friedewald formula while inulinresistance was determined by HOMA-IR.Levels of HDL-1 and HDL-large subfractions were significantly higher, whileHDL-8, HDL-9, HDL-10 and HDL-small subfraction levels, TC, LDL and CETPwere significantly lower at 1st day after surgery compared to preoperative levels inpatient group. A significant increase was observed in TG and VLDL levels after oralfeeding compared to 1st day after surgery. There was a significant decrease in PON,insulin and HOMA-IR levels after oral feeding compared to preoperative levels inpatient group. HDL levels were significantly decreased while apo A-I levels weresignificantly increased after oral feeding compared to both preoperative and 1st dayafter surgery in patients underwent SG.The anti-atherogenic HDL subfractions HDL-1 and HDL-large were increasedand atherogenic HDL subfractions HDL-8, HDL-9, HDL-10 and HDL-small weredecreased due to lower levels at 1st day after surgery compared to preoperativemeasurements. Additonally, apo A-I levels were increased due to insulin resistanceimprovement clearance and increased HDL catabolism after SG surgery.
