Abstract
Giriş: İdrarda ya da kanda, keton varlığının gösterilmesi, Emesis Gravidarum ve Hiperemesis Gravidarum tanı ve tedavisinin yönlendirilmesinde acil servis hekiminin klinik kararını etkileyebilir.Amaç: Bu çalışmadaki amacımız acil serviste Hiperemesis Gravidarum tanısını koymada parmak ucu kan keton düzeyinin tanısal değerliliğini belirlemektir.Gereç ve Yöntem: Çalışmamız 01.01.2008 ile 31.12.2012 tarihleri arasında Akdeniz Üniversitesi Tıp Fakültesi Hastanesi Acil Servisine bulantı kusma şikayetleri ile başvuran gebelerin dosyaları geçmişe dönük olarak arşivde taranarak ve verileri derlenerek hazırlandı. Bu taranan hastalardan hem parmak ucu kan ketonu, hem de idrar ketonuna bakılmış olan hastalar çalışmaya dahil edilmiştir.Sonuçlar: Çalışmaya alınan 102 hamile hastanın, yaş ortalaması 27,6±5,1 idi. Hamilelik sürelerinin ortalaması ise 63,1±2 gün idi. Hasların 70 (%68,6)'i Emesis Gravidarum 32 (%31,4)'si ise Hipermezis Gravidarum tanısı konarak hastaneye yatırılmıştır. Parmak ucu kan keton değeri 0,8 mmol/L ve üzerinde olduğunda Hipermesis Gravidarum tanısı koymadaki sensitivitesi 40,6 (23,7 – 59,4) spesifisitesi 98,5 (92,3 – 100), +LR 28,4 (3,9 – 208,1) ve –LR 0,6 (0,5 – 0,8) p<0,0001 olarak saptandı.Tartışma: Bulantı ve kusma nedeniyle acil servise gelen hastalarda parmak ucu kan keton ölçümün Hiperemesis Gravidarum tanısını koymadaki tanısal değerliliği oldukça yükseltir. Bu hastalarda idrar keton ölçümü yerine parmak ucu kan keton ölçümü kullanılabilir Introduction: Demonstrating the existence of ketones in urine or blood can effect the decision of the emergency medicine physician in guidance of diagnosis and treatment of emesis and hyperemesis gravidarum.The Goal: Our purpose in this study is to determine the value of fingertip blood ketone level in diagnosis of hyperemesis gravidarum in emergency service.Material and Method: Pregnant patients who were admitted to the Akdeniz university emergency department between 01.01.2008 and 31.12.2012 with a complaint of nausea and vomiting were retrospectively detected from the hospital database. Patients who were measured a bedside fingertip ketone plus urine ketone analysis were included to the study.Results: the mean age of the 102 pregnant patients who admitted to the study was 27,6±5,1. Mean pregnancy time was 63,1±2 days. 70 (68.6%) of the patients had emesis and 32 (31.4%) were diagnosed as hyperemesis gravidarum and then admitted to the hospital. When Fingertip ketone level was 0.8 mmol/L or more, sensitivity was 40.6 (23.7- 59.4), specifity was 98.5 (92.3- 100), + LR 28.4 (3.9- 208.1) and – LR 0.6 (0.5- 0.8) p<0.0001, in diagnosis of hyperemesis gravidarum.Discussion: The value of fingertip blood ketone measurement for the diagnosis of hyperemesis gravidarum in pregnant patients, who applied to the emergency service with nausea and vomiting, is considerably high. Fingertip ketone measurements can be used instead of urine ketone in these patients
