Abstract
Giriş: Hematolojik malignite tanısı konulmasıyla birlikte, hem hastalık hem de tedavi komplikasyonları hasta çocuk-ergenin yaşam kalitesini etkilemektedir. Tanı alan çocuğun ebeveynlerinde anksiyete, psot travmatik sendrom ve depresyon görülebilir. Bu süreçte çocuk-ergen ve ebeveynlerinin hastalığa adaptasyonu önemlidir.Amaç: Bu çalışmada Hematolojik malignitesi olan çocuk-ergen ve ebeveynlerin hastalık süresince; gerek tanı alma, gerek hastanede yatış süresi ile birlikte yaşam kalitelerindeki değişim ve ebeveynlerin depresyona eğilimleri araştırılmak amaçlanmıştır.Gereç ve Yöntem: Araştırma İstanbul' da bulunan bir Eğitim ve Araştırma Hastanesinde Çocuk Hematoloji servisinde mayıs 2019- temmuz 2019 tarihleri arasında Tanımlayıcı ve ilişki arayıcı olarak gerçekleştirilmiştir. Çalışma gönüllü kişiler üzerinde yapılmıştır. Verilerin değerlendirilmesinde yüzde, ortalama, standart sapma, independent samples test analiz yöntemleri kullanılmıştır. p< 0.05 değeri istatistiksel olarak anlamlı kabul edilmiştir. Araştırmanın evreni hematoloji servisine son bir yılda yatışı yapılan, hematolojik malignite tanılı 144 kadar çocuk-ergen ve ebeveynleridir. Araştırmanın örneklemi ise, standart örneklem büyüklüğü tablosuna göre 92 hesaplanmıştır. Ancak çalışmamız tedavi süreci tamamlayan hastaların taburcu olması nedeni ile 80 çocuk-ergen ve ebeveynden oluşmuştur. Veriler Kişisel Bilgi Formu, Beck Depresyon Envanteri (BDE), Çocuklar İçin Yaşam Kalitesi Ölçeği (ÇİYKO) ile toplanmıştır. Veriler Shapiro-Wilk testi, Kolmogorov Smirnov testi, ANOVA testi ve pearson korelasyon analizi ile değerlendirilmiştir.Bulgular : Çalışmamıza katılan hastaların % 57,5' i erkek, % 40' ı okula gitmiyor, 2-18 yaş aralığındaki 40 çocuk-ergen hastaın yaş ortalaması 8,17± 5,04, % 42,5' i AML hastasıdır. Çalışmamıza katılan ebeveynlerin % 92,5' i anne, 35' i ilköğretim mezunu, % 80' i işsiz ve ev hanımı, % 37,5' inin gelir durumu orta düzeydir. Çocuk-ergen hastanın yaşam kalitesi ve ebeevynlerin beck depresyon toplam puanı arasında istatistiksel olarak analamlı farklılık bulunmuştur (p<0.01).Sonuç : Çocuk-ergen hastanın yaşam kalitesi puanları ile ebeveynlerinin beck depreyon puanları arasında negatif yönde anlamlı ilişki bulunmuş olup; çocuk-ergenin yaşam kalitesi puanları düştükçe, ebeveynlerin depresyon puanları yükselmektedir. Introduction: In the following period of diagnosis of the hematologic malignancy, both disease and treatment complications affect the living area of sick child-adolescent. Anxiety, post-traumatic syndrome and depression may occur in the biological parents of those who diagnosed.In this period the adaptation of sick child-adolescent and their parents to the disease is very important.Aim: In this study, it was aimed to investigate the changes in the quality of life during both diagnosis and hospitalization period of both sick child-adolescents and their parents and to evaluate the tendency of parents to depressionMaterial Methods: The study was held in Göztepe Research and Training Hospital located in Istanbul with volunteers between dates May 2019 -July 2019. Percentage, mean, standard deviation, independent samples test analysis methods were used to evaluate the data. p <0.05 was considered statistically significant. The population of the study consisted of 144 children and adolescents and their parents who were hospitalized in the hematology service in the last year and diagnosed with hematologic malignancy. The sample of the study was calculated 92 according to the standard sample size table. However, our study consisted of 80 children-adolescents and parents because the patients who completed the treatment had been discharged before the questionare. Data were collected using Personal Information Form, Beck Depression Inventory (BDI), Quality of Life for Children Scale (QoL-C). Data were evaluated by Shapiro-Wilk test, Kolmogorov Smirnov test, ANOVA test and pearson correlation analysis.Results: %57.5 of the patients who participated in our study were male and %40 were not going to school. The mean age of 40 child-adolescents between the ages of 2-18 was 8.17 ± 5.04, and %42.5 were AML patients. %92.5 of the parents who participated in our study were mothers, 35 were primary school graduates, %80 were unemployed and housewives, and %37.5 had middle income. A statistically significant difference was found between the quality of life of the child-adolescent patient and the beck depression score of the parents (p <0.01).Conclusion: There was a negative correlation between quality of life scores of child-adolescent patients and Beck Depression scores of parents. As the quality of life scores of the child-adolescent decrease, the depression scores of the parents increase.
