Abstract
Amaç= Araştırma preterm bebek annelerine uygulanan progresif kas gevşeme egzersizlerinin (PKGE) annenin emzirme öz-yeterlilik ve depresyon düzeyine, bebeğin vücut ağırlığına, anne sütü alma ve emzirilme durumuna etkisini araştırmak amacıyla yapıldı. Materyal-Methot= Randomize kontrollü deneysel tasarımda yapılan araştırmanın örneklemini 20/11/2018- 30/06/2019 tarihleri arasında bir üniversite hastanesinin Yenidoğan Yoğun Bakım Ünitesi'nde yatan, 32-36 gestasyon haftaları arasında olan, 70 preterm bebek ve anneleri oluşturdu. Veri toplamada tanıtıcı bilgi formu, Emzirme Öz-yeterlilik Ölçeği (EÖYÖ), Edinburgh Doğum Sonrası Depresyon Ölçeği (EDSDÖ) ve hasta takip formu kullanıldı. Deney grubundaki annelere Progresif Kas Gevşeme Egzersizi (PKGE) uygulandı, öğretildi ve PKGE uygulama yönergesi verildi. Anneler 2 hafta süreyle sabah-akşam yönerge doğrultusunda PKGE yaptı. Her iki gruptaki annelerin EÖYÖ puanı, EDSDÖ puanı; bebeklerin vücut ağırlığı başlangıç, 1. hafta ve 2. hafta izleminde değerlendirildi.Verilerin normal dağılıma uygunluğu Kolmogorov Smirnov testi ile incelendi. Çalışmada nicel veriler için ortalama ± standart sapma (medyan), nitel veriler için frekans (yüzde), gruplararası testlerde Bağımsız Örnekler t Testi, Kikare Testi, Tekrarlı Ölçümler Varyans Analizi Test İstatistiği ile inceleme yapıldı. Bulgular= Araştırma sonucunda deney grubundaki annelerin 2. hafta izlemindeki EÖYÖ puan ortalamalarının daha yüksek olduğu (p=0,024), her iki grubun EDSDÖ puanlarında anlamlı azalma olduğu (p<0,05) saptandı. Grupların başlangıç ile 2. hafta izlemindeki EDSDÖ puan farkları karşılaştırıldığında deney grubunun puanında daha fazla azalma olduğu bulundu (p=0,001). Gruplardaki bebeklerin gestasyon haftalarına göre hem başlangıçtaki hem de 2. hafta izlemindeki vücut ağırlığı ortalamaları arasında anlamlı bir fark olmadığı saptandı (p>0,05). Her iki gruptaki bebeklerin sadece anne sütü alma ve emzirilme oranlarının arttığı bulundu.Sonuç=Preterm bebeklerin annelerinin emzirme öz-yeterliliğini arttırmak ve depresyon düzeyini azaltmak amacıyla progresif kas gevşeme egzersizinden yararlanılabilir. PKGE'nin bebeğin vücut ağırlığına etkisini değerlendirmek için anne ve bebeğin daha uzun süreli izlenmesi önerilir. Anahtar kelimeler= Preterm, anne, progresif kas gevşeme, emzirme öz-yeterliliği, depresyon Objective: The aim of this study was to investigate the effect of Progressive Muscle Relaxation Exercises (PMRE) applied to preterm infant mothers on breastfeeding self-efficacy and depression level, body weight of the baby, breastfeeding and breastfeeding status.Materials and Methods: The sample of the study, which was conducted in a randomized controlled experimental design, consisted of 70 preterm babies and their mothers between the ages of 32-36 weeks, lying in the Neonatal Intensive Care Unit of a university hospital between 20/11/2018 and 30/06/2019. Data were collected by using the descriptive information form, the Breastfeeding Self-efficacy Scale (BSES), Edinburgh Postnatal Depression Scale (EPDS) and the patient follow-up form. PMRE was applied to the mothers in the experimental group and they were instructed and instructed to apply . Mothers did PMRE in line with the morning-evening instruction for 2 weeks. The mothers in both groups had an BSES score and an EPDS score; infants' body weight was evaluated at baseline, 1st week and 2nd week follow-up. Kolmogorov Smirnov test was used to evaluate the normal distribution of the data. In the study, mean ± standard deviation (median) for quantitative data, frequency (percentage) for qualitative data, independent samples t test, chi-square test, repeated measures analysis of variance test statistics were used.Results = As a result of the study, it was found that the mean scores of BSES at the 2-week follow-up of the mothers in the experimental group were higher (p = 0.024) and there was a significant decrease in the EPDS scores of both groups (p <0.05). When the scores of the groups were compared at baseline and at the 2-week follow-up, it was found that there was a greater decrease in the scores of the experimental group (p = 0.001). There was no significant difference between the mean weight of the babies in both groups at the beginning and at 2 weeks follow-up (p> 0.05). Only breastfeeding and breastfeeding rates of the babies in both groups were found to be increased.Conclusion = PMRE exercise can be used to increase the breastfeeding self-efficacy of mothers of preterm infants and to decrease the level of depression. Long-term follow-up of the mother and the infant is recommended to assess the effect of PMRE on the baby's body weight.Key words = Preterm, mother, progressive muscle relaxation, breastfeeding self-efficacy, depression
