Abstract
Yürütücü işlevleri gerçekleştiren beyin bölgesi olan prefrontal korteksin majordepresif bozuklukla (MDB) ilişkili bölgelerden biri olduğu da çeşitli çalışmalardagösterilmiştir. MDB'de yürütücü işlevleri değerlendiren çalışmaların sonuçları çelişkilidir. Buçalışmada MDB hastalarında yürütücü işlevlerin değerlendirilmesi ve ilk epizod MDBhastaları ile tekrarlayıcı depresif epizodları olan hastaların yürütücü işlevlerininkarşılaştırılması amaçlanmıştır.DSM-IV tanı ölçütlerine göre MDB tanısı konulan 33 kişi ve 33 sağlıklı gönüllüçalışmaya alındı. Tüm katılımcılara ilk oturumda DSM-IV Eksen I Bozuklukları İçinYapılandırılmış Klinik Görüşme (SCID-I), Durumluk-Sürekli Kaygı Envanteri (DSKE), KısaAkıl Muayenesi (KAM), Beck Depresyon Envanteri (BDE) uygulandı. İkinci oturumdahastalara nöropsikolojik değerlendirme kapsamında Wisconsin Kart Eşleme Testi (WKET),Sözel Akıcılık Testi, Stroop Testi, Bas-Basma Testi ve Sürekli Performans Testi (SPT)yapıldı.Çalışmada majör depresyon hastaları ile sağlıklı kontroller arasında tümnöropsikolojik testlerde farklılık bulundu. Testlerin tümünde majör depresyon hastalarınınkontrol grubundan daha başarısız oldukları tespit edildi. Tekrarlayıcı depresyon hastaları ileilk epizod depresyon hastaları arasında yaş, cinsiyet, eğitim düzeyi, depresyonun şiddeti veanksiyete düzeyi açısından fark bulunmazken, tekrarlayıcı depresyonu olan hastalar WKETtestinin perseverasyon eğilimini gösteren tüm alt puanlarında ve Sözel Akıcılık Testindeölçülen toplam kelime sayısında daha kötü sonuçlar ortaya koydular. İki grup arasındayürütücü işlevlerdeki farkın depresyonun tekrarlayıcı olma özelliği ile ilişkili olduğusonucuna varıldı.Bu çalışmada, tekrarlayıcı depresif epizodları olan hastaların ilk epizod depresyonhastalarından yürütücü işlevler açısından farklılık gösterdiği bulunmuştur. Prefrontal cortex is considered to be the brain region where executive functions areperformed. There are contraversial findings in the studies that investigated executivefunctions in patients diagnosed with major depressive disorder (MDD). In this study, weaimed to investigate the executive functions in patients diagnosed with MDD compared tonormal control subjects and the differences between first episode depression and recurrentdepression in terms of executive functions.Patients with MDD (n:33) based on DSM-IV criteria and healthy control subjects(n:33) were taken into the study. In the first session, Structured Clinical Interview for DSMIVAxis I Disorders (SCID-I), State-Trait Anxiety Inventory (STAI), Mini-Mental Test, BeckDepression Inventory (BDI) were applied. In the second session, neuropsychological tests,including Wisconsin Card Sorting Test (WCST), Verbal Fluency Test (VFT), Stroop Test,Go-No Go Test, and Continuous Performance Test (CPT) were performed.We found statistically significant differences in the scores of all neuropsychologicaltests between MDD patients and control subjects. In all tests, patients showed worseperformance than controls. While there were no significant differences in terms of age,gender, education level, severity of depression, and level of anxiety between first episode andrecurrent depression patients, the patients with recurrent depression episodes revealed worseperformance in all subtests of WCST that are related to perseveration, and in the total wordcount in VFT. The differences between two groups of patients in terms of executive functionsare considered to be related to the recurrence of depression.In this study, we found that patients with recurrent MDD episodes have significantexecutive function differences compared to the first episode MDD patients. These differencesappeared in the WCST and VFT which are related to the fronto-striatal circuit functions.Key words: First episode depression, recurrent depression, executive functions,perseveration, Wisconsin Card Sorting Test, Verbal Fluency Test
