Abstract
Obsesif Kompulsif Bozuklukta (OKB) frontal lob ve bazal ganglia disfonksiyonuyla ilişkili nöropsikolojik işlev bozuklukları görülebileceğine dair kanıtlar mevcuttur. OKB?de yapılmış çalışmalara rağmen henüz tutarlılık yoktur. Panik bozuklukta (PB) nöropsikolojik işlevlerle ilgili araştırmalar sayıca azsa da, bazılarında bilişsel işlevlerde bozulmalar gösterilmiştir. PB?ta yürütücü işlevlerde bozulma olduğuna dair bir araştırma henüz yayımlanmamıştır. PB ve OKB anksiyete ve kaçınma davranışlarının olması ile benzer özellikler gösterirler. Bunun yanında, OKB?de hastalığa özgül belirtiler de mevcuttur. Bu çalışmada, üç grupta (OKB, PB ve sağlıklı kontrol grupları) yürütücü işlevleri karşılaştırarak, PB?ta sağlıklı kontrollere göre bir bozulma olmadığı ve OKB?ye özgül bir bozulma olduğu hipotezimizi araştırmayı planladık. Araştırmamıza DSM-IV-TR?ye göre tanı konmuş 17 OKB, 15 PB, 26 sağlıklı kontrol alınmıştır. Hastalarda ilaç kullanımı, tıbbi hastalık, Eksen-1 eştanıları, alkol-madde kullanım bozuklukları varsa, zeka geriliği saptandıysa dışlanmışlardır. Sağlıklı kontrol grubu hastalarla yaş, cinsiyet, eğitim açısından eşleştirilmiştir. Katılımcılar SCID-I, Hamilton Depresyon Derecelendirme Ölçeği, Porteus Labirentleri Zeka Testi?yle değerlendirilmiş ve OKB hastalarına Yale-Brown Obsesyon Kompulsiyon Ölçeği uygulanmıştır. Her üç gruba İz Sürme Testi, Sözel Akıcılık Testleri (Kontrollü Oral Kelime Çağırışım Testi: K, A, S ve Kategorik İsimlendirme Testleri), Stroop Testi, Wiskonsin Kart Eşleme Testi uygulanmıştır. Araştırmamızın sonuçlarına bakıldığında, üç grup arasında yürütücü işlevlerde istatistiksel olarak anlamlı farklılık bulunmamıştır. Bu sonuçlar PB açısından literatürle uyumludur. OKB grubunun sonuçları ise, hasta grubumuzun eştanılarının olmaması, depresif belirtilerinin düşük düzeyde olması ve hastaların değerlendirme sırasında ilaç kullanmamaları ile ilişkili olabilir. Çalışmamızın en önemli kısıtlılığı örneklem grubumuzun küçüklüğüdür.ANAHTAR SÖZCÜKLER: Obsesif kompulsif bozukluk, panik bozukluk, yürütücü işlevler, Wisconsin Kart Eşleme Testi, Stroop Testi Many studies provide evidence about the fact that obsessive compulsive disorder (OCD) may present with impairments in neuropsychological functions, related to frontal lobe and basal ganglia dysfunctions. However, these findings seem to be controversial. Only a small number of research about neuropsychological functions in panic disorder (PD) being carried out, cognitive functions were shown to be disturbed in some of these studies. Up to date, no published research is available on executive functions being impaired in PD. PD and OCD have similiar features as they both present with anxiety and avoidance behaviours; beside OCD is also characterized with some other disease-specific symptoms. In this study, by comparing executive functions in three groups (OCD, PD, healthy controls), we aimed to investigate the hypothesis that these functions are impaired in OCD whereas PD does not differ from healthy controls.Diagnosed according to DSM-IV-TR, 17 OCD, 15 PD patients and 26 healthy controls were included in our study. Exclusion criteria were: drug use, medical illness, comorbid Axis-1 disorders, substance use disorders and mental retardation. Healthy controls and patients were matched in terms of age, gender and education level. All participants were evaluated by means of SCID-I, Hamilton Depression Rating Scale, Porteus Maze Intelligence Test and Yale-Brown Obsessive-Compulsive Scale was additionally used for OCD patients. In each of the three groups, Trail Making Test, Verbal Fluency Test (semantic and lexical - K, A, S), Stroop Test and Wisconsin Card Sorting Test were also applied. According to the findings of our study, there was no statistically significant difference among the three groups in terms of executive functions. Although results concerning PD patients are consistent with the literature, findings of the OCD group may be related to factors such as the absence of comorbidities, low levels of depressive symptoms and not using drugs during the evaluation.
