Abstract
Amaç: İntraoperatif hipoksemiye katkıda bulunan pulmoner şanttaki artışın en önemli nedeni atelektazidir. Genel anestezi sırasında çok yaygın olarak ortaya çıkan atelektazi, postoperatif pulmoner komlikasyonların başında gelir. Klinik bulgu vermeden kendini sınırlayabildiği gibi diğer ciddi postoperatif pulmoner komplikasyonlara da zemin hazırlayabilir. Anestezi uygulamalarının çeşitliliği atelektazi oran ve alanlarında değişikliklere neden olabilir. Kolay uygulanabilen, hızlı ve radyasyon içermeyen akciğer ultrasonografisinin, atelektazi varlığını göstermedeki güvenilirliği ortaya konulmuştur. Çalışmamızda rejyonel ve genel anestezi ile yapılan alt ekstremite ortopedik cerrahi sonrası gelişen atelektaziyi transtorasik akciğer ultrasonografisi kullanılarak saptamayı ve postopereatif atelektazi oluşumu üzerine uygulanan anestezi yönteminin etkisini araştırmayı hedefledik. Gereç ve Yöntem: Ortopedi ve Travmatoloji Kliniği'nde planlı alt ekstremite cerrahisi endikasyonu konulan, yazılı ve sözlü bilgilendirilmiş onamları alınmış, yaşları 18 ve daha büyük olan, ASA I-III gruplarından 60 hasta çalışmaya dahil edildi. Çalışmaya alınan hastalar 2 gruba ayrıldı. Grup I (n=30) olgulara rejyonel anestezi ve Grup II (n=30) olgulara genel anestezi uygulandı. Hastaların hepsi ameliyattan hemen önce ve ameliyat sonrası ilk 2 saat içinde; solunum fonksiyon testi (SFT), arteriyel kan gazı örnekleri ve transtorasik akciğer ultrasonografisi ile değerlendirildi. Akciğer USG protokolüne göre toraks anterior duvar 4, sağ ve sol yan duvarlar 4 ve posterior duvar 4 olmak üzere toplam 12 bölge tarandı, görüntüler kaydedildi. Perioperatif anestezi yönetimine kör olan iki deneyimli uzman tarafından USG görüntüleri değerlendirildi. Modifiye akciğer USG skorlama sistemine göre her bölge atelektazi derecesiyle uyumlu olarak 0-3 arasında puanlandı. Her hasta için toplam skor hesaplandı. Sonuçlar %95'lik güven aralığında, anlamlılık p<0.05 düzeyinde değerlendirildi. Bulgular: Demografik veriler ve ameliyat süreleri açısından gruplar arasında istatistiksel olarak anlamlı fark yoktu. Genel anestezi uygulanan grupta postoperatif akciğer USG skorunda preoperatif skora göre istatistiksel olarak anlamlı artış oldu (p<0,0001). İki grupta da postoperatif SFT parametrelerinden FEV1 ve FEF 25-75, preoperatif değerlere göre anlamlı azaldı. Genel anestezi uygulanan grupta postoperatif PaCO2 artarken, PaO2 azaldı. Sonuç: Genel anestezi, rejyonel anestezi ile karşılaştırıldığında alt ekstremite ortopedik cerrahisi geçiren hastalarda atelektazi insidansını belirgin olarak artırmaktadır. Bunun sonucu olarak solunum fonksiyon parametreleri ve arteryel kan gazı değerleri olumsuz etkilenmektedir. Gelişen bu atelektazi akciğer USG ile erken dönemde tespit edilebilir. Bu hastalarda rejyonel anestezi yönteminin kullanılması atelektazi riskini azaltabilir. Objective: Pulmonary shunting of blood, the principal contributor to intraoperative hypoxemia, is principally caused by atelectasis. Atelectasis is one of the most prevalent postoperative pulmonary complications of general anesthesia. It can limit itself without causing clinical evidence, but also it might be an important culpit in the development of other serius postoperative pulmonary complications. The type of anesthesia can be a significant factor for incidance of atelectasis and the regions it is seen. Lung ultrasonography, which is noninvasive, easy to use and devoid of the risks associated with repeated exposure to ionizing radiotions has proved a valuable tool for diagnosis of atelectasis. The aim of this prospective observational study was to detect the development of postoperative atelectasis using transthoracic lung ultrasonography in patients having orthopedic surgery of lower extremity either regional or general anesthesia. Material and Method: In this prospective observational pilot study, 60 consecutive patients, aged 18 and older, with ASA I-III, who underwent informed written and verbal consent, scheduled for orthopedic surgery of lower extremity were enrolled in this study. The cases included our study were divided into 2 groups. Patients in Group I (n = 30) were performed spinal anesthesia and patients in Group II (n = 30) were performed general anesthesia. Patients were investigated with arterial blood gases, spirometry and lung ultrasonography before anasthesia and in 2 hours after extubation. For each ultrasonographic examination, 12 pulmonary quadrants were imaged. Each of the 12 quadrants was assigned a score of 0 to 3 according to modified lung USG score. A summed score for each patient was calculated by adding up the 12 individual quadrant scores. Images were analyzed by two independent anesthetist who were blinded to the type of anesthesia. The results were evaluated in a confidence interval of 95% with a significance level of p <0.05. Findings: There was no statistically significant difference between groups in terms of demographic data. There was a statistically significant increase in postoperative lung USG score according to preoperative values in general anesthesia group (p<0,0001). Forced expired volume in first second (FEV1) and forced expiratory flow 25-75 (FEF 25-75) were significantly decreased in both groups compared to preoperative levels. General anesthesia caused increase in PaCO2 and decrease in PaO2 values. Conclusion: General anesthesia increases the incidence of postoperative atelectasis when compared to regional anesthesia in patients having orthopedic surgery of lower extremity. As a result, respiratory function parameters and arterial blood gas values are adversely affected. Early recognition of atelectasis with lung USG and appropriate treatment may reduce the risk of developing postoperative pulmonary complications. Preference for regional anesthesia in this group of patients may reduce the risk of atelectasis.
