Laparoskopik intraperitoneal onlay mesh tekniği ile opere edilen kasık fıtığı hastalarının yaşam kalitesi ve postoperatif komplikasyonlar yönünden değerlendirilmesi

Abstract
Amaç: Laparoskopik intraperitoneal onlay mesh (IPOM) tekniğinin günümüz kasık fıtığı onarımındaki yerini; opere edilen hastaların postoperatif bulgularını, ağrı skorlarını ve yaşam kalitesi skorlarını inceleyerek değerlendirmektir.Gereç ve Yöntem: Marmara Üniversitesi Genel Cerrahi Anabilim Dalı'nda (MUGCAD) 1 Ocak 2015-31 Ağustos 2018 tarihleri arasında IPOM uygulanan hastaların vizuel analog skala (VAS) değerleri (preoperatif; postoperatif 6cı, 12ci, 24ncü saat; 1nci, 6ncı, 12nci ay), kısa form 36 (SF36) skorları (preoperatif; postoperatif 1nci, 6ncı, 12nci ay) ve operasyon sonrasındaki takip bulguları retrospektif olarak incelenmiştir.Bulgular: Çalışmaya dahil edilen 56 (54 erkek, 2 kadın) hastanın preoperatif VAS değerleri postoperatif 12nci saat itibari ile azalmıştır (p<0,05). VAS medyan değeri preopatif 5 iken bir yıl sonunda 0'a gerilemiştir (p<0,05). Tüm SF36 alt gruplarında preoperatif değerlere göre ilk ay sonuçlarından itibaren yükseliş gözlenmiştir (p<0,05). Bir yıllık takip sonunda 4 (%7,1) hastada nüks gelişmiştir. Nüks gelişimi ile peroperatif bulgulardan herhangi biri arasında anlamlı ilişki saptanmamıştır (p>0,05). Nüks dışında uzun dönem komplikasyon izlenmemiştir.Sonuç: Laparoskopik IPOM tekniği kasık fıtığı hastalarında ağrı ve yaşam kalitesine olumlu etkileri ile diğer tekniklerden geride değildir. Literatüre göre diğer onarımlara kıyasla nüks oranları yüksek saptanmış; ancak operasyon süresi, hastanede kalış, operasyon için gerekli minimum tecrübe gibi veriler açısından IPOM daha etkili bir yöntemdir.Anahtar Kelimeler: Kasık Fıtığı, Ağrı, Yaşam Kalitesi, Nüks, IPOM Aim: To find where is the laparoscopic intraperitoneal onlay mesh (IPOM) technique among present-day inguinal hernia repairs, according to the patient's postoperative outcomes, pain scores and quality of life scores.Material & Method: Visual analogue scales (VAS) scores (preoperative; postoperative 6th, 12th, 24th hour; 1st, 6th, 12th month), short form 36 (SF36) scores (preoperative; postoperative 1st, 6th, 12th month) and after operation results of patients had undergone IPOM between 1 Jan 2015-31 Aug 2018 in Marmara University General Surgery Department (MUGCAD) were examined retrospectively.Results: Preoperative VAS scores of 56 (54 men, 2 women) patients, that had been included in the study, decreased as start from the postoperative 12th hour (p<0,05). While median of preoperative VAS had been 5, the median of VAS went down to 0 after a year from surgery (p<0,05). In all subgroups of SF36 scores, an increase was observed as start from the postoperative 1st month in comparison with preoperative ones (p<0,05). End of the following for a year, recurrence was evolved in 4 (%7,1) patients. There was no significant relationship can be found between evolving recurrence and per-operative data (p>0,05). Except for recurrence, there was no other long-term complication.Conclusion: With positive effects on pain and quality of life of inguinal hernia patients, laparoscopic IPOM is non-inferior in comparison with the others. According to the literature in comparison with other repairs, it's recurrence rates were detected high but in terms of data like operation periods, staying hospital, the minimum experience that requires for the operator IPOM were more effective.Key Words: Inguinal Hernia, Pain, Quality of Life, Recurrence, IPOM
