Abstract
Amaç: Bu çalışmanın amacı, gebelikte hipertansif bozukluğu olan hastalarda, sezaryen sonrası tromboemboli profilaksisi yapılıp yapılmamasının tromboembolik olay oluşması üzerine etkisini karşılaştırmaktır.Materyal ve Metod: İstanbulda üçüncü basamak bir merkeze, 2012-2018 yılları arasında başvurmuş gebelikte hipertansiyon tanısı olan ve sezaryene alınmış 386 hasta dahil edilmiştir. Hastalar gebeliğin hipertansif bozukluğu çeşidine göre ve tromboemboli profilaksisi yapılıp yapılmamasına göre gruplara ayrılıp, tromboembolik olay ve diğer gebelik sonuçları açısından karşılaştırılmıştır.Bulgular:Gruplar arası sonuçlar karşılaştırıldığında 210 tromboprofilaksi yapılmamış hastada 11 tromboembolik olay (%5) izlenirken, tromboprofilaksi yapılmış 176 hastada sadece 2 hastada (%1) tromboembolik olay saptanmıştır (p<0.05).Sonuç: Gebelikte tromboemboliye yatkınlık artmıştır. Gebeliği ek olarak hipertansiyonun eşlik etmesi bu ihtimali daha da artırmaktadır. Çalışmamızda gebeliğe eşlik eden hipertansif bozukluklarda tromboemboli için yapılan profilaksinin önemi vurgulanmıştır.Anahtar kelimeler: Gebeliğin hipertansif bozuklukları, Tromboemboli, Profilaksi Background: The aim of this study is to compare the effect of thromboembolic prophylaxis on thromboembolic event occurrence in patients with the diagnosis of hypertensive disorders of pregnancy who underwent cesarean section. Materials and Methods: Three hundred and eighty six patients who admitted to a thertiary center in Istanbul with the diagnosis of hypertensive disorder of pregnancy and underwent cesarean section were included in the study. The patients were divided into groups according to the type of hypertensive disorder of pregnancy and whether or not thromboembolism prophylaxis was performed.Results: When the results were compared between groups. Among 210 patients who had thromboembolism prophylaxis, 11 patients had thromboembolic events (%5). In 176 patients who didn't have prophylaxis, only 2 patients (%1) had thromboembolic event (p<0.05).Conclusion: There is an increased tendency to thrombormbolism in pregnancy. Accompanying hypertensive disorders increases preexisting thromboembolic event risk. In the present study we emphasized the importance of thromboembolism prophylaxis in patients with hypertensive disorders of pregnancy.Key words: Hypertensive disorders of pregnancy, Thromboembolism, Anticoagulants, Prophylaxis
