Hızlandırılmış asidometrik yöntemle Staphylococcus aureus suşlarının beta-laktamaz aktivitesinin tayini

Abstract
14- ÖZET İstanbul Üniversitesi İstanbul Tıp Fakültesi Mikrobiyoloji Anabilim Dalı Laboratu varı ' na gelen kan, idrar, cerahat, balgam, boğaz salgısı gibi muayene maddele rinden izole edilen 100 stafilokok susunun saflığı ve S. aureus olup olmadığı incelenmiştir. Bunun için katı besi- yerine yayılan kültürlerden saf kültür alınarak, Gram boya sı ile boyandıktan sonra, tür tanısı için tavşan plazmasına ekilmiş, kanlı jeloza çizilmiş, Löfflere ve mannitli pep- tonlu suya ekilmiştir. Suşların tümü plazmayı pıhtılaştır- mış kanlı jelozda hemoliz yapmış, Löffler besiyerinde altın sarısı pigment oluşturmuş, mannitten asit meydana getirmiş tir. S. aureus olduğu belirlenen 100 susun beta-laktamaz aktivitesi hızlandırılmış asidömetrik yöntem ile tayin edilmiş ve disk yöntemi ile penisilin G, ampisilin, metisilin, azidosilin, mezlosilin ve karbeni si 1 ine duyarlı lıkları denenmiştir. 100 sustan 50'sinde beta-laktamaz aktivitesi tespit edilmiş, aynı zamanda bu suşlarda beta-laktamaz ile harap olan penisilin G ve ampisiline %100 oranında dirençli bu lunmuştur. Bu 50 susun 5623' ü mezlosiline, %21'i azidosiline, %19'u metisiline, %17'si karbeni si 1 ine direnç göstermi ştir. Enzim aktivitesi göstermeyen 50 su ş penisilin G'ye %90 ( 45 ' i ), ampisiline %68 (34'i) dirençli bulunmuştur. 7 ' si mezlosiline, 6'sı azidosiline, 18'i metisiline, 4'ü karbeni si 1 ine direnç göstermiştir. Enzim aktivitesi gösteren ve göstermeyen 100 susun 95 ' i penisilin G'ye, 84'ü ampisiline, 30'u mezlosiline, 27 ' si azidosiline, 37'si revtisiline, 2 1 ' i karbeni si 1 ine dirençli olarak tespit edilmiştir. -15- SUMMARY From the blood, urine, pus, sputum, fftiroat secretion and other similar specimens which came to Istanbul Medical School's main laboratory Istanbul Faculty; of Medicine 100 staphylococ strains were isolated and it was investigated whether the strans were pure or whether S. aureus was present or not. For this reason a pure culture was taken from the culture wich had been spread on an agar medium, after its Gram-stainned preparation was made. For the species diagmosis rabbit plasma was inoculated and bloody jeloz was streaked, Löffler and mannit pepton medium were inoculated, as well. All of the strain coagulated were rabbit plazma, on the bloody jeloz hemoliz was produced, on the Löffler medium a golden yellow pigment, was produced and acid was produced from mannit. In 100 cultures, the presence of S. aureus was determined, with the Rapit Aidometric methods, lTLctamase activty determination was made and with the disk method sensibility to penicillin G, ampcillin, mezlocillin, azidocillin, methicillin, and to carbenici 1 1 in, was tested. From 100 cultures, 50 of them were found to have p- lactamase activity, at the same ruins the same cultures which had been desolated by penicillin G and ampicillin were found to have 100% porportional resistance. These cultures also showed 23% resistance mezlocillin, 21% to azidocillin, ! ]9% to methicillin, and 17% to carbenici 1 1 in. Of the 50 cultures which did not show enzyme activity, 90% (45'i) were resistant to penicilin G and 68% (34'u) were resistant to ampcillin while 7% were resistant16- to mezlocillin, 6% to azidoci 1 1 in, 18% to methicillin and 4% to carbenici İlin. Of the 100 cultures, whether they did show enzyme activity or not, 95% were resistant to penicillin G, 84% to ampic illin, 30% to mezlocillin, 27% to azidocillin, 37% to methicillin, and 21% to carbenic i 1 1 in.
