Abstract
Ortodontide posterior dislerin intrüzyonu oldukça zor gerçeklestirilmektedir. Mikroimplantlarınortodontide ankraj amaçlı kullanılmasıyla, geleneksel mekaniklere göreçevre dokulardaki yan etkileri oldukça az olan ve hasta kooperasyonu gerektirmeyenintrüzyon mekanikleri uygulanmaya baslanmıstır . Fakat bu mekaniklerin etkileri ileilgili yeterli çalısma bulunmamaktadır. Çalısmamızda mikro-implantlardan ankraj alandört farklı üst çene posterior intrüzyon mekaniginin, uygulandıkları dislerde meydanagetirdikleri etkilerin sonlu elemanlar analizi ile incelenmesi amaçlanmıstır. Birincimodelde, dis segmentlerine iki vestibülde, iki de palatinalde olmak üzere dört mikroimplanttankuvvet uygulanmıstır. İkinci modelde dis segmentlerine vestibülde ikimikro-implanttan kuvvet uygulanmıs olup momentlerin dengelenmesi için ikitranspalatinal ark olusturulmustur. Üçüncü modelde vestibül ve palatinalde birer mikroimplanttankuvvet uygulanmıstır. Dördüncü modelde vestibülde tek mikro-implanttankuvvet uygulanmıs olup momentlerin dengelenmesi için tek transpalatinal arkolusturulmustur. Analiz sonuçlarına göre en homojen gerilme dagılımı ve en dengeliintrüzyon birinci modelde elde edilmistir. En yüksek gerilme degerleri ve vestibüleryönde en fazla devrilme dördüncü modelde meydana gelmistir. Transpalatinal arkiçeren modellerde, digerlerine göre gerilme degerleri daha yüksek çıkmıs olup,vestibüler yönde devrilme daha fazla meydana gelmistir. Dis segmentlerine anterior veposteriordan aynı anda kuvvet uygulanan intrüzyon sistemlerinde, kuvvetlerin tekbölgeden uygulandıgı sistemlere göre anteroposterior yönde daha dengeli bir intrüzyonelde edilmistir. The execution of intruding posterior teeth is indeed quite a difficult issue. After thebeginning of the application of micro-implants as anchorages in orthodontic treatments,the need for cooperation of the patient has been eliminated, and side effects implied onthe surrounding tissues in the process in comparison to the traditional mechanics havereduced to such a considerable degree that it is being used ever since. In our study, it isintended to evaluate the effects of four different maxillary posterior intrusion mechanicswith micro-implant anchorage on the applied teeth, by using finite element analysis. Inthe first model intrusive forces were applied on the posterior dental segment from fourmicro-implants which were placed in the vestibular and palatal side of the alveolarbone, two on each side. In the second model the forces were applied from two microimplantsplaced in the vestibule alveolar bone, and in order to balance the momentsproduced, two transpalatal arches were constructed. In the third model two microimplantswere placed in the vestibular and palatal alveolar bone to apply forces. In thefourth model intrusive forces were applied by using a single micro-implant on each sideof the vestibule alveolar bone and just one transpalatal arch was constructed. The mostuniform stress distribution and the most balanced intrusion have occurred on the firstmodel. The highest stresses and the most vestibular tipping movement were found in theforth model. At the models that contain transpalatal arches, vestibular tippingmovements were more prominent, and the stresses were higher as well. The mechanicswith simultaneous intrusive forces applied from anterior and posterior of the dentalsegment, lead to a more uniform intrusion in anteroposterior direction.
