Abstract
Bu çalışmada, dirençli ağrıları olan ve lomber disk hernisi (LDH) tanısı almış erkek hastaların baba-oğul ilişkisinin projektif yöntemler aracılığıyla psikanalitik açıdan araştırılması hedeflenmiştir. LDH, kadın ve erkeklerde eşit olarak görülmekteyken erkeklerin kadınlara göre daha sık cerrahi müdahaleyi tercih ettiği bilinmektedir. Bu sebepten ötürü çalışmada erkek hastalar dirençli ağrıları kapsamında mercek altına alınmıştır. 27-49 yaş aralığında olan gönüllü on dört LDH tanısı almış katılımcı ile çalışma yürütülmüştür. Değerlendirmede LDH hastalarına klinik öngörüşme yapılmıştır; ardından projektif testlerden olan Rorschach Testi ve Tematik Algı Testi uygulanmıştır. Son olarak ise Visual Analog Skalası(VAS) ve Oswestry Özürlülük Ölçeği verilmiştir. Uygulanan projektif testler, Fransız Okulu'na özgü içerik analizi ile değerlendirilmiş ve psikanalitik kuram bağlamında yorumlanmıştır. Araştırmanın sonuçlarına göre ise; dirençli ağrıları olan LDH hastalarının libidinal ve agresif dürtülerini eklemleme güçlüğü barındırdığı dikkat çekmiştir. Bunun ise erken dönem nesne ilişki kalitesinin yanı sıra ödipal dönem öncesi ve ödipal sonrası baba-oğul ilişkinin niteliğinin dürtü organizasyonunda ve aynı cins ebeveyn ile özdeşim ile ilgili yetersizlik barındırdığı bulgulanmıştır. Bu bağlamda dirençli ağrının dürtüsel aktivitenin bedensel dışavurumuna olabilecğeini akla getirmiştir. Ayrıca LDH tanısı almış katılımcıların ağrı ile ilişkisinin bu hastalığın seyri ve tedavisi açısından önemli olduğu; sonrasında ortaya çıkabilecek bedensel belirtilere ilişkin yatkınlığını belirlenmesi yönünden önemli olduğu düşünülmüştür.Anahtar Kelimeler: Lomber Disk Hernisi , Ağrı, Projektif Testler, Baba-Oğul İlişkisi In this study, it was aimed to investigate the father-son relationship of male patients with resistant pain and lomber disc herniation (LDH) by means of projective methods. LDH is equally seen in men and women, whereas, it is known that men prefer surgery more often than women. Therefore, in this study, male patients were examined in this study within the scope of their experiences of resistant pain. The study was conducted with 14 volunteer patients that aged between 27-49 years and had received LDH diagnosis before. In the overall evaluation, a clinical intake with LDH patients was followed by the administration of Rorschach Test, Thematic Apperception Test (TAT). The participants also completed the Visual Analog Scale (VAS) to assess subjective pain along with the Oswestry Disability Questionnaire that assessed daily functioning. The results were evaluated by the content analysis of the French School and interpreted based on psychoanalytic theory.According to the results, it was determined that LDH patients with resistant pain have difficulty in articulating the libidinal and aggressive instincts. Hence, it is thought that the nature of early object relations, as well as the nature of pre-oedipal and post-oedipal father-son relationship has caused deficiencies in the courses of instinct organization and identification with the same sex parent. Resistant pain is thought to be the somatic expression of instinctual activity. In addition, LDH participants' experience of pain was found to be important in terms of the prognosis and treatment of this disease, also in terms of determining the predisposition to the somatic symptoms that may arise afterwards.Keywords: Lomber Disc Herniation, Pain, Projective Tests, Parent-Son Relationship
