Deneysel olarak oluşturulan kırıklara bifosfonat ilaç kullanımının etkilerinin histopatolojik olarak incelenmesi

Abstract
Üçüncü jenerasyon nitrojen içerikli bifosfonatlardan olan zoledronik asitin endokondral kemikleşen uzun kemiklerdeki kırık iyileşmesine etkilerini inceleyen çalışmalar bulunmasına karşın, daha çok intramembranöz kemikleşen bir kemik olan mandibulanın kırık iyileşmesine etkilerini inceleyen bir çalışma bulunmamaktadır. Bu çalışmanın amacı, sıçan mandibulasında oluşturulan kırık modelinde, zoledronik asitin, tedavi süresini kısaltmaya yönelik etkilerini histopatolojik ve radyolojik yöntemlerle incelemektir.Çalışmamızda kırk adet yetişkin erkek Sprague Dawley cinsi sıçan kullanılmıştır. Tüm deneklerin sağ mandibulalarında molar dişler arasından geçen dikey bir kırık hattı oluşturulmuş ve kırık, dört delikli bir mikroplak ve dört mikrovida yardımıyla internal olarak tespit edilmiştir. Denekler her biri on hayvan içeren dört gruba ayrılmıştır. İki deney grubuna post-operatif birinci hafta sonunda intravenöz olarak 0.1mg/kg dozda zoledronik asit uygulanmıştır. İki kontrol grubuna ise sadece serum fizyolojik uygulanmıştır. Kontrol ve deney gruplarının birer grubu altıncı haftada, birer grubu ise sekizinci haftada sakrifiye edilmiştir. Sakrifikasyon sonrası elde edilen mandibulalardan bilgisayarlı tomografi verileri ile kırık iyileşmesi gözlenen bölgedeki kemiğin hacimsel yoğunluk ölçümleri yapılmıştır. Mandibulaların daha sonra histopatolojik incelemeleri yapılmıştır. Çalışmamızın histopatolojik ve radyolojik incelemeleri sonucunda, post-operatif birinci haftada tek doz intravenöz zoledronik asit uygulamasının, sıçan mandibula kırığı iyileşmesi üzerine altıncı haftada olumlu etkilerinin olduğu saptanmıştır.Anahtar Kelimeler: sıçanlar, mandibula, kırıklar, mikroplak, zoledronik asit Zoledronic acid is one of the third generation nitrogen-containing bisphosphonates. While it has been previously studied in the fracture healing of endochondral ossification of long bones, it has never been studied in the mandible, which predominantly ossifies by intramembranous ossification. The aim of this study is to evaluate the effects of zoledronic acid on shortening the healing period of a rat mandible fracture model, by means of histopathological and radiological parameters.In our study, forty mature male Sprague Dawley rats were used. In the right mandible of all the animals, a vertical fracture line, passing in between the molar teeth, was prepared. The fracture was internally stabilized using a four hole microplate and four microscrews. All the animals were divided into four groups, each containing ten animals. At the end of the post-operative first week, a single dose of 0.1mg/kg zoledronic acid was given intravenously to the two experimental groups. The two control groups were just given saline solution instead. One experimental and one control group were sacrificed on the sixth post-operative weeks. The remaining one experimental and one control group were sacrificed on the eighth week. After sacrification, mean volumetric densities were calculated from the fracture healing area of the harvested mandibles, using the computerized tomography data. The mandibles were then evaluated histopathologically. Using the histopathological and radiological evaluations of our study, it was concluded that a single dose of intravenous zoledronic acid, applied on the post-operative first week, has positive effects on the rat mandible fracture healing on the sixth week.Key words: rats, mandible, fractures, microplate, zoledronic acid
