Abstract
Bu çalışmanın amacı üç boyutlu sonlu elemanlar stres analizi yöntemini kullanarak, atrofik posterior maksiller bölgede sinüs elevasyonu yapılamadığı durumlarda farklı dizaynlardaki implant yerleşimlerinin çiğneme kuvvetleri altında karşılaştırmak ve gruplar arası farkın anlamlığını değerlendirmektir. Bu amaçla seçilen hastanın posterior maksillası Konik Işınlı Bilgisayarlı Tomografi (ILUMA Orthocad, CBCT, Imtec Imaging, Oklahoma, USA) görüntülerinden elde edilen kesitler ile Rhinoceros 4.0 (Robert McNeel & Associates USA) yazılımına alındı ve burada rekonstrükte edilip modele edildi. Oluşturulan üç boyutlu model üzerine seçilen implant modeli (AstraTech Dental Implant System; Mölndal, Sweden) entegre edilmiş ve teknik olanakların elverdiği ölçüde gerçek yaşamdaki çiğneme kuvvetlerine yakın kuvvetler altında stres analizi yapılmıştır. Atrofik posterior maksillada, 2 implant üstü distal kantileverli, 2 veya 3 implant üstü konvansyonel veya 2 implant üstü 2 üyeli (kısa ark) sabit köprü protetik restorasyonlar oluşturularak Von Misses Stresi, Gerilme Stresi, Sıkışma Stresi ve Yer Değiştirme değerleri incelenmiştir. Posterior maksillanın rezorpsiyon paterninden dolayı, implantlar çoğu zaman açılı pozisyonda yerleştirilmek zorunda kalınmaktadır. Bu nedenle modellerde, açılı yerleştirilen implant üstü açılı postların ve düz yerleştirilen implant üstü düz postların etkisi de araştırılmıştır. En yüksek riske sahip model, açılı yerleştirilen implant üstü açılı postlu kantileverli model bulunurken, en düşük riskli model, kısa ark oluşturularak düz yerleştirilen implant üstü düz postlu 2 üyeli sabit köprü protetik restorasyonlu model bulunmuştur. Since osseointegration was described by Branemark, dental implantology became a subsequent demand in dentistry. The implant supported prosthesis has become the popular and demanding option for the edentulous and partially edentulous patient. However, a prerequisite for the successful implant treatment is sufficient bone volume. So, variations in maxillary bone volume, degree of pneumatization of maxillary sinus are primary concern in placing dental implants in the atrophic posterior maxilla. The insufficient height of bone between the maxillary crest and maxillary sinus, the highly pneumatization of maxillary sinus leads difficulties in placing implants. Regarding these kind of variations in maxillary bone volume, different techniques were described in the literature to increase the primary stability of implant and bone volume. But these techniques requires up to 6 months of extra healing time, additional cost and additional surgical operations.The purpose of this study was to analyze the stresses around implants placed in different position combinations in the atrophic maxilla without sinus lifting operation performed and compare the stresses between groups.For this reason a model of posterior maxilla was obtained from a patient?s Orthocad Conic Beam Computer Tomography sections (ILUMA Orthocad, CBCT, Imtec Imaging, Oklahoma, USA) which were then transferred to the software Rhinoceros 4.0 (Robert McNeel & Associates USA) reconstructed and modeled. With the same program Astra implants, loading conditions and chewing forces were modeled and stress analysis performed.
