Kronik Myeloid Lösemi (KML)` li olgularda konvansiyonel sitogenetik ve FISH yöntemiyle Ph kromozomu ve varyant translokasyonların tespiti

Abstract
Bu çalışmada, Kronik Miyeloid Lösemi (KML)'li olgularda %80 sıklıkla rastlanan Philadelphia kromozomunun varlığının ve %5-10 sıklıkla rastlanan varyant Phildelphia translokasyonlarının konvansiyonel sitogenetik ve FISH tekniği ile araştırılması hedeflendi.Çalışmada yaş ortalaması 51 ? 12,55 (29-76) olan ve İmatinib Mesilat (Gleevec) tedavisi altındaki varyant translokasyonlu 7, sitogenetik olarak Philadelphia kromozomu (Ph1) pozitif 12 , negatif 6 olmak üzere toplam 25 olgu değerlendirildi.Çalışmaya alınan varyant translokasyonlu olguların translokasyon mekanizmasını açıklamak ve kromozom anomalisini daha detaylı incelemek amacı ile translokasyona katılan kromozomlara özgün tüm kromozom boyama probları (wcp) ve kozmid problar kullanılarak FISH çalışmaları yapıldı.Çalışmalar sonucunda;1-konvansiyonel sitogenetik çalışmasında Ph1 pozitif olarak saptanan olgular FISH çalışmasında da pozitif ,2-konvansiyonel sitogenetik çalışmasında Ph1 negatif bulunan 6 olgudan 4'ü FISH çalışmasında Ph1 pozitif bulundu,3-varyant translokasyonlu olgularda ise yapılan FISH çalışmaları sayesinde translokasyon mekanizmaları ayrıntılı olarak açıklanabildi.KML'li olgular için Ph1 kromozomunun varlığının tedavi seçiminde ve prognozun tayininde en önemli kriter olması nedeni ile özellikle sitogenetik olarak Ph1 negatif saptanan olgularda mutlaka FISH tekniği ile sonuçların onaylanması gerektiği kararına varıldı.Varyant translokasyonlu olgularda ise translokasyon mekanizmasını açıklayabilmek ve sitogenetik olarak görülemeyen ancak prognostik açıdan önemli olabilen, translokasyona katılmış diğer kromozomların varlığını araştırabilmek için FISH tekniği ve konvansiyonel sitogenetiğin bir arada kullanılması gerektiği sonucuna ulaşıldı. In this study, we aimed to investigate the presence of the Philadelphia chromosome encountered in 80% of the Chronic Myeloid Leukemia cases, and the Philedelphia translocations seen in 5-10% of the cases via conventional cytogenetic and FISH technologies.25 patients with mean age of 51±12,55 (29-76), treated with Imatinib Mesylate (Gleevec), with translocations (7), with cytogenetically positive Philedelphia chromosome (12) and controls (6) were included to this study.Wcp and cosmid probes specific to the chromosomes participating to the translocations were used for FISH studies to interpret the mechanism of translocation and to analyze in detail the chromosome anomaly in cases with variant translocations.The obtained results are:1. The cases determined as Ph1 positive with conventional cytogenetic studies were confirmed by FISH studies.2. 4 of the 6 cases determined as Ph1 negative with conventional cytogenetic studies were found Ph1 positive by FISH studies.3. In cases with variant translocations, the mechanisms of translocations could be explained in detail through FISH studies.We concluded that the presence of Ph1 chromosome should be investigated via FISH technique for CML patients determined cytogenetically as Ph1 negative, since the presence of Ph1 chromosome is the criteria with utmost importance for the selection of the treatment and the determination of the prognosis. We also came through that FISH technique should be used in conjunction with conventional cytogenetics, for cases with variant translocation, to explicate the translocation mechanism and also to investigate the presence of further chromosomes participating to the translocation which couldn?t be seen cytogenetically but may be important for prognostic evaluation.
