Abstract
Erkek faktörlü infertilite, infertilite vakalarının yalnızca % 20'lik kesiminden sorumlu olup, diğer % 30 ? 40'lık dilimini de etkilediği düşünülmektedir. Son 10 yıl içinde erkek infertilitesinin olası sebeplerinden biri olarak ejakulat spermdeki DNA bütünlüğü çalışılmıştır. Enfeksiyon ile ilgili oksidatif stres, kimyasal uyarana maruz kalınması gibi çok sayıda olası sperm DNA hasar sebepleri bulunur. Poli (ADP-riboz)ilasyon (PAR), hücre enerji seviyesinden etkilenen, DNA-hasar bağımlı bir süreç olup, major bir enzim olan poli (ADP-riboz) polimeraz ile kontrol edilmektedir. 18 farklı proteinden oluşan PARP ailesinde PARP-1 en iyi bilinen üyesi olup, DNA'da oluşan hasarın keşfi ve onarımında iyi tanımlanmış bir rolü vardır. Aşırı DNA hasarının oluştuğu durumlarda PARP molekülünün 24 ve 89 kDa'luk parçalara ayrıldığı yapılan araştırmalar ile kanıtlanmıştır. Bu çalışmamızda infertil hastalardan alınan semen örneklerindeki apopitoza giden sperm hücrelerinde, PARP molekülünün bölündüğünü ve infertiliteye yol açan sperm anomalileri ile doğrudan ilişkili olup olmadığını gösterebilmeyi amaçladık. Semen örnekleri infertilite şikayeti ile androloji polikliniğine başvuran 16 hastadan toplanmıştır. Yapılan spermiyogram analizleri sonucunda sperm sayısı ml'de 20 milyondan az olan 8 oligoastenoteratopermik hastayı hasta grubu, sperm sayısı ml'de 20 milyondan fazla olan 8 hastayı da kontrol grubu olarak tayin ettik. Elektron mikroskopi yöntemi hastaların sperm örneklerinde apopitozun morfolojik değişikliklerini tanımlamak için kullanıldı. Apopitoz tayini için peroksidaz in situ apopitoz tespit kiti ile boyanan örneklerin hasta grubuna ait olanlarında apopitotik hücre oranı yüksek çıkarken (p<0,004), immunositokimya yöntemi ile PARP-1 antikoru uygulanan örneklerden kontrol grubuna ait olanlarda bölünmemiş PARP seviyesi hasta grubuna kıyasla yüksek çıkmıştır (p<0,001). Bu nedenle sınırlı DNA hasarı durumunda, DNA zincir kırıklarını onararak hücreyi koruyan PARP, zararlı etkenlere maruz kalan sperm hücrelerinde parçalara ayrıldığı zaman DNA'yı onaramayacak ve hücre apopitoza gidecektir. Bu veriler doğrultusunda, semen örneklerinde sağlıklı sperm populasyonunda azalma görülen bireylerde infertilite vakalarının baş gösterebileceği düşünüldü. Male factor infertility is responsible for only 20% of infertility cases and is thought to affect in the other 30 to 40%. One possible cause of male infertility that has been studied in last decade is the integrity of DNA in the nucleus of mature ejaculate spermatozoa. There are many possible cause of sperm DNA damage including oxidative stress associated with infections and chemical exposure. Poly (ADP-ribosyl)ation is a DNA damage?dependent process which is affected by cell energy level and its intracellular level is controlled by poly (ADP-ribose) polymerase (PARP). PARP-1 is the best-known member of the PARP family which is comprised of as many as 18 distinct proteins. PARP-1 has a well characterized role in DNA damage detection and repair. Under conditions of extensive DNA damage PARP is cleaved into 89 and 24 kDa fragments was confirmed by the researchers. In this study we aimed to show PARP cleavage in sperm samples which are taken from infertile patients and relationship between PARP cleavage and male factor infertility caused by sperm anomalies. Semen samples were collected from patients who were attending andrology clinics because of infertility. According to spermiogram analysis, we detected 8 oligoasthenoretatospermatic males as patient group which have less than 20 million sperm count per ml in their semen samples and 8 male patients as control group which have more than 20 million sperm counts per ml in their semen samples. Electron microscopy was used to identify morphological features of apoptosis in these patient sperm samples.For detection of apoptosis, samples belonged to the group of patients staining with peroxidase in situ apoptosis detection kit, has a high percentage of apoptotic cells (p<0,004), in immunocytochemistry method PARP-1 antibody was applied to semen samples belonging to control group have a high percantage of whole PARP levels when compared to samples belonging to patient group (p<0.001). Therefore in the case of limited DNA damage, PARP protecting cells by repair of DNA strand breaks is cleaved into the fragments when the sperm cells are exposed to the extensive harmful effects. Because of this, sperm cells will undergo to apoptosis. In line with these data we thought that reduction in populations of healthy sperm in the semen samples from individuals can cause male factor infertility.
