II. sınıf adeziv restorasyonlarda farklı materyallerin ve restorasyon yöntemlerinin mikrosızıntı açısından in vitro değerlendirilmesi

Abstract
Bu in vitro çalışmanın amacı, II.Sınıf adeziv kavitelerde farklı tabakalama tekniklerinin, akışkan kompozit materyalinin, farklı fiber ağların ve kompozit materyalinin indirekt yöntem ile uygulanmasının mikrosızıntıya etkilerinin, şeffaflaştırma yöntemi ile incelenmesidir. 110 adet sürmemiş üçüncü büyük azı dişinde II.Sınıf slot kaviteler hazırlanmış, ScotchBond MP etch&rinse dentin bonding uygulanarak 11 farklı grup oluşturulmuştur: GK Filtek Ultimate Universal, tek tabaka; GHK Filtek Ultimate Universal, horizontal; GHAK Filtek Ultimate Universal+Filtek Ultimate Flowable, horizontal; GHCF Filtek Ultimate Universal+Filtek Ultimate Flowable+everStick NET, horizontal; GHPF Filtek Ultimate Universal+Filtek Ultimate Flowable+Ribbond THM, horizontal; GHI Filtek Ultimate Universal, indirekt, horizontal; GOK Filtek Ultimate Universal, oblik; GOAK Filtek Ultimate Universal+Filtek Ultimate Flowable, oblik; GOCF Filtek Ultimate Universal+Filtek Ultimate Flowable+everStick NET, oblik; GOPF Filtek Ultimate Universal+Filtek Ultimate Flowable+Ribbond THM, oblik; GOI Filtek Ultimate Universal, indirekt, oblik. Bitirme ve cila işlemlerinden sonra, 500 tur ısı banyosu işlemi (5oC-55oC) uygulanmış ve dişler %50'lik gümüş nitrat çözeltisinde 24 saat bekletilmiştir. Dekalsifikasyon ve şeffaflaştırma yöntemi uygulandıktan sonra gingival, aksiyal ve proksimal duvarlarda oluşan mikrosızıntı değerleri stereomikroskop ile incelenerek alan olarak hesaplanmış; ?Kruskal Wallis? ve ?Mann Whitney U? istatistiği kullanılarak değerlendirilmiştir (p<0,001). Tüm gruplarda mikrosızıntı belirlenmiş ve en yüksek mikrosızıntı gingival duvarlarda saptanmıştır. Yerleştirme teknikleri, akışkan kompozit materyali, fiber liflerin akışkan kompozit ile birlikte kullanılması ve kompozit materyalinin indirekt olarak uygulanması mikrosızıntıyı anlamlı derecede azaltmıştır. Horizontal ve oblik tabakalama teknikleri arasında istatistiksel olarak anlamlı bir fark bulunmamış ancak oblik tabakalama tekniği ile rakamsal olarak daha düşük sızıntı değerleri bulunmuştur. Mine ile sonlanan duvarlarda, istatistiksel olarak anlamlı derecede en düşük mikrosızıntı değerleri kompozit materyalinin indirekt olarak uygulandığı gruplarda saptanmıştır. Dentin ile sonlanan duvarlarda ise istatistiksel olarak anlamlı derecede en düşük mikrosızıntı değerleri akışkan kompozit ile birlikte fiber lif uygulanan gruplarda elde edilmiştir. The aim of this in vitro study was to evaluate the effects of different placement techniques, flowable composite materials, different fiber nets and application of composite material with the usage of indirect technique to the microleakage with tooth clearing method in Class II adhesive cavities. Class II slot cavities were prepared in 110 non erupted third molar teeth and 11 different groups were formed by using ScotchBond MP etch&rinse dentine bonding agent: GK Filtek Ultimate Universal, bulk; GHK Filtek Ultimate Universal, horizontal; GHAK Filtek Ultimate Universal+Filtek Ultimate Flowable, horizontal; GHCF Filtek Ultimate Universal+Filtek Ultimate Flowable+everStick NET, horizontal; GHPF Filtek Ultimate Universal+Filtek Ultimate Flowable+Ribbond THM, horizontal; GHI Filtek Ultimate Universal, indirect, horizontal; GOK Filtek Ultimate Universal, oblique; GOAK Filtek Ultimate Universal+Filtek Ultimate Flowable, oblique; GOCF Filtek Ultimate Universal+Filtek Ultimate Flowable+everStick NET, oblique; GOPF Filtek Ultimate Universal+Filtek Ultimate Flowable+Ribbond THM, oblique; GOI Filtek Ultimate Universal, indirect, oblique. The teeth were immersed in 50% silver nitrate solution for 24 hours and thermocycled for 500 cycles (5oC-55oC) after finishing and polishing. The microleakage scores of gingival, axial and proximal walls were evaluated with a stereomicroscope and calculated in the form of area after the application of decalcification and clearing method; they were analyzed with Kruskal-Wallis and Mann-Whitney U-tests (p<0,001). In all groups, microleakage were determined and the highest microleakage scores were identified in gingival walls. Placement techniques, flowable composite material, the usage of fiber nets combined with flowable composite and application of composite material by using indirect technique significantly reduced microleakage. There was no significant difference between horizontal and oblique placement techniques but less microleakage scores were determined by using oblique placement tecniques. In cavity walls with enamel margins, the significant fewer microleakage scores were determined in the groups which the composite material was applied by using indirect technique. In cavity walls with dentine margins, the significant fewer microleakage scores were determined in the groups which fiber nets combined with flowable composite were applied.
