Lipopolisakkarid verilmiş sıçanlarda (Albino wistar) çeşitli nitrik oksid sentetaz inhibitörlerinin hemodinamiğe etkileri

Abstract
Sepsisin ağır şekli olan septik şok, çoklu organ bozukluğuna ve ölüme neden olabilir. Kalbin kasılmasının azalması, damar tonusunun düşmesi ve bozuk doku perfüzyonu sepsiste görülen kalp-damar sistemindeki anormalliklerden bazılardır. Bu değişiklikler nitrik oksit gibi farklı aracılarının salınmasında da görülür. Çalışmamızda, lipopolisakkarid (LPS) verilerek endotoksemi oluşturulanyapılan sıçanların ortalama arteriyal kan basınçları (O AB) üzerine, NG-nitro-L-arjinin-metil-ester (L-NAME) (10 mg/kg i.v.), Aminoguanidin (AG) (50 mg /kg i.v.) ve Dekzametazon (DEX) (4 mg/kg i.v.)' un etkisine bakıldı. Deney hayvanlarının, spontan solunum yapmalarını sağlamak için trakelerine kamil yerleştirildi. O AB kaydı için karotid arterlere, madde enjeksiyonu için ise, jugular verilere kanül bağlandı. Hayvanlar, LPS uygulamasından 3 saat sonra yukarıda belirtilen inhibitörlere göre gruplandırıldılar. Ayrıca, hayvanlara kontrol amacı ile diğer iki gruba da sadece inhibitörler veya fizyolojik tuzlu su uygulandı. LPS uygulamasından 3 saat sonra OAB % 22 düştü. LPS uygulamasından 3 saat sonra uygulanan L-NAME ve AG, OAB' yi yükseltti. DEX ise, kan basıncını değiştirmedi. Fakat bir glukokortikoid olan DEX bir gece ve LPS uygulamasından yarım saat sonra uygulanınca, LPS' in kan basıncım düşürücü etkisi görülmedi. Sonuç olarak, iNOS, sepsisteki hipotansiyonun birinci kaynağıdır ve glukokortikoidlerin sepsis patogenezinde koruyucu etkisi vardır. Septic shock, a severe form of sepsis, can lead to multiple organ dysfunction and death. Cardiovascular abnormality in severe sepsis includes decreased cardiac contractility, reduced vascular tone and inadequate tissue perfusion, all of which are secondary to the release of various inflammatory mediators one of which is nitric oxide (NO). This study examined the effects of L-NAME (10 mg/kg i.v.), Aminoguanidine (AG) (50 mg /kg i.v.) and Dexhamethasone (DEX) (4 mg/kg i.v.) on mean arterial pressure (MAP) in rats with lipopolysaccaridde (LPS) (10 mg/kg i.v.), induced- endotoxemia. Trachea of experimental animals were cannulated to facilitate spontane respiration. Carotid artery and jugular veins were also cannulate for recording the MAP and giving the drugs. Animals were grouped into which were given LPS (10 mg/kg i.v.) for 3 hours and subsequently different NOS inhibitors above mentioned. We also applied only inhibitors to animals for 4 hours and serum physiological solution to the control group for 4 hours. MAP was decreased by %22 after LPS administration within 3 hours. AG and L-NAME increased this MAP after 3 hours LPS application. DEX did not change this pressure when it was given after LPS. But when DEX, a glucocorticoid was injected one night and half an hour before LPS, MAP did not decrease as it did in only LPS administred animals because DEX is a spesific inhibitor which affects of iNOS mRNA gen expression. As a result, iNOS is the major source of hypotension in sepsis and glucocorticoids can have protective role for sepsis pathogenesis.
