Abstract
NRAMP1 geni insanda 2. kromozomun q35 bölgesine yerlesik olup 550 aminoasitlik,53 kDa büyüklügünde bir integral membran proteini kodlamaktadır. nfekte bireylerinmakrofajlarının fagolizozomlarından demiri sitoplazmaya pompalayarak,fagolizozomlardaki bakterilerin gelisimi için esas olan demiri ortamdan uzaklastırır vehücre içi parazitlerin replikasyonlarını önler. Ayrıca makrofaj aktivasyon yolunda daönemli roller üstlenmektedir: CXC kemokin (a-chemokine) regülasyonunu saglar; IL-1b, indüklenebilir nitrik oksit sentaz (iNOS), MHCII molekülleri, TNF-a ve nitrik oksit(NO) salınımını düzenler. Bu islevlerinden dolayı NRAMP1'in otoimmun veinfeksiyöz hastalıklarda aday bir gen olabilecegi düsünülmektedir.Bu çalısmada, NRAMP1'in islevi üzerine etkili olabilecegi düsünülen NRAMP1 genpolimorfizmlerinin Türk romatoid artrit ve tüberküloz hastalarındaki rollerininarastırılması amaçlandı.Bu kapsamda, 98 romatoid artrit, 112 tüberküloz hastası ve 213 saglıklı kontrolgrubunda NRAMP1 geninin 5' promotör bölgesi polimorfizmi DNA dizileme yöntemiile, INT4, D543N ve 3'UTR polimorfizmleri ise hem ARMS-PZR hem de REAyöntemleri kullanılarak analiz edildi.Çalısmamızda romatoid artrit ile NRAMP1'in 5' promotör, D543N ve 3'UTRpolimorfizmleri arasında anlamlı baglantı bulunamazken (5'promotör polimorfizmi p=0.162, D543N polimorfizmi ve 3'UTR polimorfizmi p=0.130), INT4 polimorfizmiarasında anlamlı baglantı bulundu (p:0.005, %95 CI 1.20-2.77, O.R: 1.824). Tüberkülozile NRAMP1'in 5' promotör, INT4, D543N ve 3'UTR polimorfizmleri arasında anlamlıbaglantı bulunamadı (5'promotör polimorfizmi p= 0.444, INT4 polimorfizmi p= 0.337,D543N polimorfizmi ve 3'UTR polimorfizmi p=0.286). Ayrıca romatoid artritin siddetive tüberkülozun farklı klinik tipleri ile 5'promotor, INT4, D543N, 3'UTRpolimorfizmleri arasında da anlamlı baglantı bulunamadı. 3'UTR polimorfizmi ileD543N polimorfizmi arasında baglantı dengesizligi belirlendi. D543N polimorfizmibakımından A/A genotipine sahip bireylerde 3'UTR polimorfizmi bakımındandelesyon/delesyon genotipine, G/A genotipi bulunan bireylerde delesyonsuz/delesyongenotipine ve G/G genotipine sahip bireylerde delesyonsuz/delesyonsuz genotipinesahip oldugu belirlendiSonuç olarak, Türk toplumunda tüberküloz ile NRAMP1 polimorfizmleri arasındabaglantı bulunamaz iken, romatoid artrit ile NRAMP1 geninin INT4 polimorfizmiarasında anlamlı baglantı tespit edildi. INT4 Allel C'yi tasıyan bireylerin, bu allelitasımayanlara göre 1.8 kat romatoid artrit hastası olma riskinin arttıgı belirlendi. Buverilere göre Türk romatoid artrit hastalarında Allel C'nin, hastalıgın ortaya çıkmasındaetkili bir faktör olabilecegi düsünüldü. In human NRAMP1 gene which encodes a 53kDa integral membrane protein consistingof 550 amino acids, is located on the q35 site of the 2nd chromosome. NRAMP1 pumpsthe iron ions from the phagolysosomes of the macrophages of infected individualstowards the cytoplasm; by this way it prevents the replication of intracellular parasites.Moreover it also plays important roles in the macrophage activation pathway: Itmaintains the regulation of CXC chemokine (-chemokine) and regulates the secretionof IL-1b, inducible nitric oxide synthase (iNOS), MHCII molecules, TNF- and nitricoxide (NO). Thus, NRAMP1 is considered to be a candidate gene for autoimmune andinfectious diseases due to its? important functions.This study aimed to investigate the roles of NRAMP1 gene polymorphisms that mightaffect NRAMP1 function in Turkish rheumatoid arthritis and tuberculosis patients.In this context, 5? promoter region polymorphism of NRAMP1 gene was analyzed byDNA sequencing and INT4, D543N and 3? UTR polymorphisms were analyzed byusing both of ARMS-PCR and REA techniques in 98 rheumatoid arthritis patients, 112tuberculosis patients and 213 healthy control individuals.Although a significant association was not detected between rheumatoid arthritis andthe 5? promoter, D543N and 3? UTR polymorphisms of NRAMP1 gene (5? promoterpolymorphism p= 0.162, D543N polymorphism and 3? UTR polymorphism p=0.130), asignificant correlation was detected between the INT4 polymorphism and rheumatoidarthritis (p:0.005, %95 CI 1.20-2.77, O.R: 1.824). There was no significant associationbetween tuberculosis and 5? promoter, INT4, D543N and 3? UTR polymorphisms (5?promoter polymorphism p=0.444, INT4 polymorphism p=0.337, D543N polymorphismand 3?UTR polymorphisms p=0.286). In addition, no significant association were foundbetween severity of rheumatoid arthritis, clinically different types of tuberculosis and 5?promoter, INT4, D543N and 3?UTR polymorphisms. A linkage disequilibrium wasdetected between the 3? UTR and D543N polymorphisms. Individuals with A/A, G/Aand G/G genotypes of D543N polymorphism were found to have deletion/deletion,nondeletion/deletion and nondeletion/ nondeletion genotypes of 3? UTR polymorphism,in sequence.As a result, while no association was detected between tuberculosis and NRAMP1polymorphisms, INT4 polymorphism of NRAMP1 gene was found to be significantlycorrelated with rheumatoid arthritis. Risk ratio for being a rheumatoid arthritis patientwas found to increase by 1.8 fold in individuals carrying the INT4 allele C whencompared to the others that do not carry this allele. According to these data, allele C isthought to be an important factor for the out come of rheumatoid arthritis in Turkishrheumatoid arthritis patients.
