Estetik restoratif materyallerde farklı koşullarda oluşan aşınmanın in vitro olarak incelenmesi

Abstract
Bu çalışmada geleneksel cam iyonomer simanı (Fuji IX GP, GC), reçine modifiye cam iyonomer simanı (Fuji II LC Improved, GC), kompomer (Compoglass F, Ivoclar Vivadent), hibrit kompozit (Aelite LS Posterior, Bisco), mikrohibrit kompozit (Gradia Direct, GC) ve nanohibrit kompozit (IPS Empress Direct, Ivoclar Vivadent) olmak üzere altı farklı restoratif materyalin, iki farklı pH değerine sahip ortamda gösterdikleri aşınma direnci, yüzey sertliği ve yüzey pürüzlülüğünün karşılaştırılmalı olarak incelenmesi amaçlanmıştır. Normal ağız ortamı için pH'sı 7 olan yapay tükürük, reflü etkisindeki ağız ortamı için pH 2,6 değerindeki hidroklorik asit kullanılmıştır. Aşındırma işlemi uygulanmadan önce, tüm materyallerin yüzey pürüzlük değerleri ölçülmüştür. Aşındırma işlemi sonrası, aşınma miktarı hesaplanmış, ayrıca yüzey pürüzlülüğündeki değişim değerlendirilmiştir. Örnekler taramalı elektron mikroskobunda (X50 ve X500 büyütmede) incelenmiştir. Verilerin istatistiksel analizinde, Oneway Anova test, Tukey HDS test, Student t test ve Paired Sample t test kullanılmıştır.Geleneksel cam iyonomer simanının (Fuji IX GP, GC) aşınma direnci, diğer restoratif materyallere göre anlamlı derecede düşük bulunmuştur. (p<0.01). Asidik ortamdan en az kompozit reçineler etkilenmiştir. Reçine modifiye cam iyonomer simanının (Fuji II LC Improved, GC) yüzey pürüzlülük değerinin diğer restoratif materyallere göre yüksek olduğu saptanmıştır. (p<0.05, p<0.01).Araştırma sonuçlarına göre; cam iyonomer simanlarının ve reçine modifiye cam iyonomer simanlarının özellikle aşınma kuvvetlerinin yoğun olduğu bölgelerde ve mide asidinin ağız boşluğunu etkilediği, reflü sorunu olan hastalarda restoratif materyal olarak kullanımının uygun olmadığı belirlenmiştir. Bu tip olgularda ve estetiğin önem taşıdığı bölgelerde kompozit reçinelerin tercih edilmesi restorasyonun ömrü ve başarısı açısından daha güvenilir ve uygundur. The purpose of this study was to evaluate and compare the wear rates, surface hardness and surface roughness of conventional glass ionomer cement (Fuji IX GP, GC), resin-modified glass ionomer cement (Fuji II LC Improved, GC), compomer (Compoglass F, Ivoclar Vivadent), hybrid composite (Aelite LS Posterior, Bisco), microhybrid composite (Gradia Direct, GC) and nanohybrid composite (IPS Empress Direct, Ivoclar Vivadent) under two different pH levels. Artificial saliva at pH 7,0 was used to simulate a serous oral environment. Hydrochloric acid at pH 2.6 was chosen to simulate acidic condition related to gastric reflux. Before wear procedure, surface roughness of all restorative materials were measured. And after wear procedure amout of material loss, changes of surface roughness were evaluated by profilometer. Samples were observed by scanning electron microscope (X50, X500). The statistical analyses were performed by Oneway Anova test, Tukey HDS test, Student t test and Paired Sample t test.The wear resistance of glass ionomer cement (Fuji IX GP, GC) was found the lowest among the other tested restorative materials (p<0.01). Composite resins was affected by acid minimally. Resin-modified glass ionomer cement (Fuji II LC Improved, GC) showed the highest surface roughness comparison to the other materials (p<0.05, p<0.01).The results of this study suggested that making restorations by using glass ionomer cement and resin-modified glass ionomer cement should be avoided from occlusal contacting surfaces, especially in patient with reflu problems. In such cases, composite resins would be more suitable and reliable for esthetics areas.
