Abstract
Tip 2 diyabet insülin direnci ve hiperglisemi ile karakterize olan multifaktöriyel bir hastalıktır. Oksidatif stres T2DM gelişiminde önemli bir faktör olmakla birlikte, diyabette oluşan hiperglisemi de oksidatif stresi arttırmaktadır. T2DM patogenezinde yer aldığı düşünülen SIRT6 glukoz metabolizması ve oksidatif stres ile ilişkili bir deasetilaz ve mono-(ADP)-ribozil transferazdır. Bu nedenle çalışmamızda prediyabet ve T2DM olgularında periferal lökositlerde SIRT6 ekspresyonunun mRNA ve protein düzeyinde araştırılması, SIRT6 ekspresyonunun açlık kan şekeri, hemoglobinA1c (HbA1c), C-peptid, insülin direnci, lipidler ve C-reaktif protein (CRP) düzeyi ile ilişkisinin incelenmesi amaçlanmıştır. Ayrıca DNA onarımında görev alan SIRT6'nın oksidatif DNA hasarına karşı koruyucu olabileceği düşünülerek SIRT6 ekspresyonunun üriner 8-OHdG düzeyi ile olası ilişkisinin incelenmesi hedeflenmiştir.Çalışmamızda tip 2 diyabetli 70 hasta, prediyabetli 50 hasta ve 40 sağlıklı bireyin periferal lökositlerinde SIRT6 gen ekspresyonu düzeyleri qRT-PCR, SIRT6 protein seviyeleri immünositokimya yöntemi ile belirlendi. Ayrıca bu kişilerden alınan spot idrar örneklerinde üriner 8-OHdG düzeyleri ELISA yöntemi ile belirlendi. Periferal lökositlerde SIRT6 mRNA ekspresyon düzeyleri karşılaştırıldığında gruplar arasında istatistiksel olarak anlamlı bir farklılık bulunmadı. İmmünositokimyasal analizde SIRT6 (+) hücre sayısı bakımından T2DM ve preDM gruplarında kontrol grubuna göre istatistiksel olarak anlamlı bir azalma olduğu tespit edildi (P<0,01). T2DM grubunda SIRT6 (+) hücre sayısının preDM grubuna göre daha düşük olduğu belirlendi (P<0,05). Üriner 8-OHdG düzeyleri T2DM grubunda, preDM grubuna göre anlamlı olarak yüksek bulundu (P<0,05). Periferal lökositlerdeki SIRT6 protein düzeylerinin diyabet ve prediyabet ile ilişkili olabileceği sonucuna varıldı. Type 2 diabetes is a multifactorial disorder characterized by insulin resistance and hyperglycemia. Oxidative stress, while being an important factor in the development of T2DM, hyperglycemia seen in diabetes also increases oxidative stress. SIRT6 is a deacetylase and mono-(ADP)-ribosyl transferase associated with glucose metabolism and oxidative stress which is thought to be involved in the pathogenesis of T2DM. For this reason we aimed to investigate SIRT6 expression at mRNA and protein levels in peripheral leukocytes of patients with prediabetes and T2DM, and the relationship between fasting blood glucose, hemoglobin A1c (HbA1c), C-peptide, insulin resistance, lipids and C-reactive protein (CRP) levels. In addition, since SIRT6 is involved in DNA repair it was thought that it may be protective against oxidative DNA damage, therefore it was aimed to investigate the potential relationship between SIRT6 expression and urinary 8-OHDG level.In the present study, SIRT6 gene expression levels in peripheral leukocytes of 70 patients with type 2 diabetes, 50 patients with prediabetes and 40 healthy subjects were determined by qRT-PCR, SIRT6 protein levels were determined by immunocytochemistry. In addition, urinary 8-OHDG levels were measured by ELISA method in spot urine samples taken from the subjects.When SIRT6 mRNA expression levels in peripheral leukocytes were compared, no statistically significant difference was found between the groups. Immunocytochemical analysis revealed a statistically significant reduction in the number of SIRT6 (+) cells in T2DM and preDM groups in comparison to the control group (P<0,01). The number of SIRT6 (+) cells in the T2DM group was found to be lower than that of in the preDM group (P<0,05). Urinary 8-OHDG levels were significantly higher in the T2DM group than that of in the preDM group (P<0,05). It was concluded that SIRT6 protein level in peripheral leukocytes may be associated with diabetes and prediabetes.
