Abstract
Bu çalışma doğum korkusunu azaltmada duygusal özgürleşme tekniği (Emotional Freedom Technique-EFT) ve nefes farkındalığı (NF) uygulamalarının etkinliğinin belirlenmesi amacıyla yapılan randomize kontrollü deneysel bir araştırmadır. Araştırma bir vakıf üniversite hastanesinde Nisan 2016-Mayıs 2017 tarihleri arasında gerçekleştirldi. Randomizasyonla gruplara ayrılmış doğum korkusu olan 35'er gebe EFT ve NF, 50 gebe kontrol grubunda olmak üzere toplamda 120 gebeyle yürütüldü. Gebelere öncelikle Tanıtıcı Özellikler Veri Formu ve Wijma Doğum Beklentisi/ Deneyimi Ölçeği A versiyonu uygulandı, dahil olduğu gruplara göre uygulamalar yapıldı. Doğumun birinci evresinde her bir fazın başında ve sonunda Öznel Rahatsızlık Birimi (SUD) ile doğum korkusu düzeyi belirlendi. İkinci evrede İnnatal Dönem Değerlendirme Formu uygulandı. Doğum gerçekleştikten 24 saat sonra ise Wijma Doğum Beklentisi/ Deneyimi Ölçeği B (W-DEQ B) versiyonu ve EFT ya da NF uygulaması anket formu uygulandı. Gebelerin gruplara göre sosyodemografik ve gebelik özellikleri, W-DEQ A versiyonu puan toplamları arasında farklılık saptanmadı (p>0,05). W-DEQ B versiyonundan alınan puan toplamları arasında farklılık saptandı (p<0,01). Kontrol grubunun puanı, EFT ve NF grubuna göre anlamlı düzeyde yüksek saptandı (p<0,01). W-DEQ B versiyonunun alt boyutlarında olumlu davranışların eksikliği, yalnızlık ve doğumla ilgili kaygı düzeylerinde farklılık saptandı (p<0,01). NF grubu gebelerin uygulama sonrası SUD aktif ve geçiş faz değeri, EFT grubu olgulara göre anlamlı düzeyde yüksek saptandı (p<0,01). Araştırmaya katılan grupların uygulama öncesi ve sonrası SUD farkları değerlendirildiğinde; yaşı 20-30 olan gebelerde, EFT grubu latent, aktif ve geçiş fazı farkları NF grubundan anlamlı düzeyde yüksek bulundu (p<0,01). Sonuç olarak, doğum korkusunu azaltmada EFT'nin, nefes farkındalığına göre daha etkili olduğu görüldü. This study is a randomized controlled trial research aiming at estimating the effects of Emotional Freedom Technique (EFT) and Breathing Awareness (BA). The research was conducted between April 2016-May 2017 in a foundation hospital. It was conducted on 120 expecting women separated into groups by randomization, of which 35 were treated with EFT, another 35 with BA and remaining 50 constituted the control group. Wijma Delivery Expectancy/Experience Questionnaire (W-DEQ) Version A was given to expecting patients, then techniques were applied accordingly with groups to which patients belonged. In the first stage, at the beginning and end of every phase, the level of fear was determined with Subjective Units of Distress (SUD). 24 hours after the birth, the W-DEQ-B version was applied, together with the questionnaire regarding EFT or BA technique. The lack of significant differences between groups of expecting mothers was determined as for sociodemographic and pregnancy-related characteristic (p>0,05). Similarly, no difference as for the total scores of W-DEQ-A was spotted between groups of expectant mothers (p>0,05). The difference detected between total scores of W-DEQ-B (p<0,01) and the score of the control group was determined to be significantly higher (p<0,01) than those groups. In the analysis of sub-dimensions of W-DEQ-B, the difference was detected in the levels of positive action deficiency, loneliness and birth-related anxiety (p<0,01). Statistically, after the technique was applied in BA group, the value of SUD in active and transition phase was determined to be statistically significantly higher (p<0,01) than it was for the EFT group. The differences for 20-30 years old expectant mothers between latent, active and transition phase were found to be significantly higher (p<0,01). As a result, it was determined that as for decreasing the childbirth fear, EFT is more effective than breathing awareness.
