İşitsel nöropati olan ve olmayan koklear implant kullanıcılarında ince motor becerilerinin değerlendirilmesi

Abstract
İşitsel nöropati spektrum bozukluğu (İNSB), işitsel uyaranın nöral işlemleme süreçlerini etkiler. İNSB ile oluşan anormal temporal kodlama sonucu, sesler üst merkezlere bozuk bir biçimde ulaşır. Yenidoğan bebeklerde İNSB prevelansı yaklaşık %0.009 olarak belirtilmiştir. Prelingual işitme kaybı varlığında, bireylerin dil gelişimi ve motor becerilerinin olumsuz etkilendiği bilinmektedir. İnce motor beceriler, bireyin günlük yaşam aktivitelerinde ve akademik başarısında önemli rol oynar. Çalışmamızın amacı, işitsel nöropati olan ve olmayan koklear implant kullanıcılarının ince motor becerilerini değerlendirmek ve farklılıkları belirlemektir.Çalışmamıza; yaşları 25-71 ay (ort. 50,88) aralığında, 60 çocuk dahil edilmiştir. İNSB tanısı almış olan koklear implant kullanıcıları grubu, İNSB olmayan koklear implant kullanıcıları grubu ve normal işitmeye sahip grup olmak üzere, 20'şer kişilik 3 grup oluşturulmuştur. Gruplardaki yaş ve cinsiyet faktörleri eş tutulmuştur. Katılımcılara, Ankara Gelişimsel Tarama Envanteri (AGTE) uygulanmıştır. İnce motor becerileri, kutu blok testi (KBT) ve dokuz delikli peg testi (DDPT) ile değerlendirilmiştir. Değerlendirmeler sonucunda; İNSB olan koklear implant kullanıcısı bireylerin ince motor becerileri, normal işiten yaşıtlarına göre istatistiksel olarak daha zayıf bulunmuştur (p<0,05). İNSB tanısı olan ve olmayan koklear implant kullanıcıları arasında ve İNSB olmayan koklear implant kullanıcıları ile normal grubun sonuçları arasında anlamlı bir fark görülmemiştir (p>0,05). İnce motor becerilerindeki zayıflığın; işitme kaybı sonucu ortaya çıkan işitsel yoksunluğun yanı sıra, işitsel nöropati spektrum bozukluğunun patofizyolojisiyle ilgili olduğu düşünülmüştür. Auditory neuropathy spectrum disorder (ANSD) affects neural processing of auditory stimuli. ANSD results in abnormal temporal coding and sounds reach the upper centers in a distorted way. The prevalence of ANSD is reported to be about %0.009 in newborn infants. It is known that in the presence of prelingual hearing loss, language development and motor skills of individuals are negatively affected. Fine motor skills plays an important role in daily life activities and academic achievement. The aim of study is to evaluate the fine motor skills of the cochlear implant (CI) users with or without ANSD and to determine differences.60 children between the ages of 25-71 months (mean, 50,88) were included in our study. 3 groups of 20 person were formed including the group of CI users with ANSD, the group of CI users without ANSD and the group with normal hearing. Age and gender factors in the groups were identical. Ankara Developmental Screening Inventory was applied to all participants. Fine motor skills were assessed using the Box and Block Test and the Nine Hole Peg Test. As a result of evaluations; fine motor skills of the CI users with ANSD were found to be statistically weaker than the age matched normal hearing children (p<0,05). There was no significant difference between the CI users with and without ANSD and the results of CI users without ANSD and normal group (p>0,05). Weakness in fine motor skills is thought to be related to the pathophysiology of ANSD as well as the result of auditory deprivation.
