Abstract
Parkinson hastalığı (PH) en yaygın nörodejeneratif hastalıklardan biri olarak Substantia Nigra pars compacta (SNpc) bölgesindeki dopaminerjik nöronların kaybı sonucu meydana gelmektedir. PH'nın altında yatan mekanizmalar tam olarak açıklanamamasına rağmen son yapılan çalışmalarda, mitokondriyal fonksiyon bozukluğu ve kronik inflamasyon mekanizmalarının hastalığın ilerlemesine katkıları olduğu gösterilmiştir. PH'nın SNpc bölgesinde reaktif glial hücrelerde Tümör nekrozis faktör-alfa (TNF-alfa) immünoreaktivitelerinin artmış olduğu bildirilmiştir. Eşleşme bozucu proteinler (UCP-4) mitokondrinin iç membranında yerleşmiştir, bu proteinler membran potansiyelinde azalmaya neden olarak oksidatif fosforilasyonda elektron transport zincirinde ATP sentezinde azalmaya sebep olmaktadırlar. Mitokondri sitozolünde yerleşik Mangan Süperoksit Dismutaz (MnSOD) oksidatif stresin oluşturduğu hasara karşı hücre içerisinde koruyucu görev alarak, süperoksit radikalinin hidrojen peroksite dönüştürülmesinde görevlidir. NF-kB'nin TNF-alfa tarafından aktif hale getirilmesiyle eşleşme bozucu protein 4'ün aktivitesini baskıladığı rapor edilmiştir. Bu literatür bilgilerinin ışığında, PH'da UCP-4 C/T (rs 10807344), TNF-alfa -308 G/A (rs 1800629), Nf-kB -94 İns/Del ve MnSOD Ala16Val polimorfizmleri arasındaki ilişkiyi araştırmayı amaçladık. Çalışmamızda, yaş ve cinsiyetleri eşleştirilmiş olarak 85 sağlıklı kontrol ve 89 İdiyopatik Parkinson Hastası dahil edildi. Genomik DNA periferik kandan tuzla çöktürme yöntemiyle izole edildi. UCP-4 C/T (rs 10807344), TNF-alfa -308 G/A (rs 1800629) Taqman Prob ve primerler kullanılarak gerçek zamanlı Polimeraz Zincir Reaksiyonuyla gerçekleştirildi. MnSOD Ala16Val, Nf-kB -94 İns/Del genotip tayinleri ise polimeraz zincir reaksiyonunu takiben kesim enzimi uzunluk polimorfizmi yöntemiyle yapıldı. Genotip ve allel frekansları SPSS 21 programında ki-kare yöntemiyle hesaplandı. p<0,05 istatistiksel anlamlılık sınırı olarak alındı. PH grubunda, UCP-4 TT genotip frekansının kontrol grubununkine göre anlamlı olarak düşük olduğu bulundu (p<0,01). Gruplar arasında, TNF-alfa -308 G/A, Nf-kB -94 İns/Del ve MnSOD Ala16Val genotip ve allel frekanslarında anlamlı farklılık bulunmadı (p>0,05). Eşleşme bozucu protein 4 TT genotipini taşımanın Parkinson hastalığı için koruyucu olarak önemli bir faktörü olabileceğini öne sürmekteyiz. Parkinson's disease (PD) is one of the most common neurodegenerative disease which occurs by the loss of dopaminergic neurons in substantia nigra pars compacta (SNpc). Although the underlying mechanisms of PD is not clearly explained, it is demonstrated in the recent studies that mitochondrial dysfunction and chronic inflammation mechanisms have also contributed to the progression of disease. It was shown that immunoreactivities of Tumour necrosis factor-alpha (TNF-alpha) were increased in reactive glial cells of SNpc in patients with PD. Uncoupling proteins are localized in mitochondrial inner membrane, which observed to decrease mitochondrial membrane potential led to decrease of ATP synthesis in electron transport chain during oxidative phosphorylation. Manganese Superoxide Dismutase (MnSOD) which is located in the mitochondria cytosol has a protective role against oxidative stress' damage in the cytoplasm and MnSOD converts superoxide radical to hydrogen peroxide. It has been reported that the supression of UCP4 activity was due to activated NF-kappa B binding that was stimulated by TNF-alpha. In light of this literature knowledge, we aimed to research the relationship between UCP-4 C/T (rs10807344), TNF-alpha -308 G/A (rs1800629), MnSOD Ala16Val, Nf-kB -94 Ins/Del polymorphisms in PD. In this study, we have enrolled age-and gender matched 85 healthy controls and 89 patients with Idiopathic Parkinson's Disease. Genomic DNA was isolated from peripheral blood by using salting out method. Genotypes of UCP-4 C/T TNF-alpha -308 G/A were performed using designed TaqMan Probes and primers in Real Time PCR (RT-PCR). MnSOD Ala16Val, Nf-kB -94 Ins/Del were genotyped polymerase chain reaction following restriction fragment length polymorphism. The frequencies of genotypes and alleles between groups were evaluated chi square test with SPSS 21 Software Programme. p <0.05 was the limit of significance. In the PD group, it was found that TT genotype of UCP4 was found statistically lower than that of controls (p<0.05). There were no significancies in the frequencies of TNF-alpha -308 G/A, MnSOD Ala16Val, Nf-kB -94 Ins/Del genotypes between groups (p>0.05).We proposed that TT genotype of UCP-4 gene may be an important protective factor for PD.
