Üst ekstremite rejyonel intravenöz anestezisinde volüm düşürmeye yönelik geçici ek turnike kullanımı

Abstract
Amaç: Bu çalışmada üst ekstremite cerrahisinde geleneksel rejyonel intravenöz anestezi (RİVA) yöntemine ek olarak geçici turnike uygulanmasının lokal anestezik volümünü düşürmede etkinliğini araştırmayı amaçladık.Yöntem: Elektif ortopedik üst ekstremite cerrahisi geçirmiş ASA fiziksel skoru I-II-III olan 40 erişkin hastanın gözlem formları retrospektif olarak incelenmiştir. Hastalar iki gruba ayrılmıştır: Grup 15'de (n = 20) 150 mg lidokain hidroklorür, serum fizyolojik eklenerek 15 ml'ye tamamlanmış ve Grup 20'de (n = 20) 150 mg lidokain hidroklorür serum fizyolojik ile 20 ml'ye tamamlanmıştır. Grupların peroperatif takip değerleri, turnike ağrısı oluşma süreleri ve hasta memnuniyet skorları karşılaştırıldı.Bulgular: Demografik veriler her iki grupta benzer bulunmuştur. Grup 15'de turnike ağrısı gelişen hasta sayısı ve Grup 20'de turnike ağrısı gelişen hasta sayısı arasında anlamlı istatistiksel bir fark saptanmamıştır. İki grup arasında 30. dakika saturasyon değerleri arasında istatiksel olarak anlamlı bir fark saptanmış olsa da klinik olarak anlamlı bir farklılık saptanmamıştır. Diğer takip bulguları istatiksel olarak benzerdir.Sonuç: Kısa süreli üst ekstremite cerrahilerinde düşük volümlü lokal anestezik solüsyonu ile yapılan bu RİVA tekniğinin etkili bir anestezi oluşturduğunu düşünmekteyiz.Anahtar Kelimeler: RİVA, el cerrahisi, turnike ağrısı Aim: In this study, we aimed to investigate the efficacy of temporary tourniquet in addition to traditional regional intravenous anesthesia (RIVA) in reducing the local anesthetic volume in upper extremity surgery.Methods: Observational forms of 40 adult patients with ASA physical score I-II-III who underwent elective orthopedic upper limb surgery were retrospectively reviewed. The patients were divided into two groups: Group 15 (n=20) received 150 mg lidocaine hydrochloride completed to 15 ml with saline and Group 20 (n=20) received 150 mg lidocaine hydrochloride completed to 20 ml with saline. Periods of tourniquet pain formation, peroperative follow-up values and patient satisfaction score were compared.Results: Demographic data were similar in both groups. No statistically significant difference was found between the number of patients with tourniquet pain in group 15 and group 20. A statistically significant difference was found in 30th minute saturation values between the two groups, but no clinically significant difference was found. Other follow-up findings are statistically similar.Conclusions: We think that this RIVA technique with low volume local anesthetic solution provides an effective anesthesia in short-term upper extremity surgery.Key words: IVRA, hand surgery, tourniquet pain
