Radyoiyot tedavisi almış tiroid kanseri hastalarında hastalık algısının anksiyete, depresyon ve yaşam kalitesi ile ilişkisi

Abstract
Çalışmamızda kliniğimizde son bir yıl içerisinde total tiroidektomi sonrası radyoaktif iyot-131 tedavisi görmüş iyi diferansiye tiroid kanseri hastalarında hastalık algısı, anksiyete, depresyon ve yaşam kalitesi değerlendirilmesi yapıldı. Yeni tanı ve tedavi sürecinde olan ve tedavisi tamamlanıp 6. ay iyot-131 ile tüm vücut taraması yapılmış TSH supresyonu ile kontrolleri devam eden hastaların hastalığa uyumunun, anksiyete ve depresyon semptomlarını tetikleyip, yaşam kalitesini olumsuz etkileyip etkilemediği araştırıldı. Tüm hastalara değerlendirildikleri sırada Hastalık Algısı Ölçeği (HAÖ), SF-36 Yaşam Kalitesi Ölçeği, Beck Depresyon Ölçeği ve Beck Anksiyete Ölçeği ve sosyo-demografik form uygulandı. Çalışmaya katılan olguların yaş ortalaması 40.62±10,28 yıl idi. 84 (%84,0) kadın,16 (%16,0) erkek olgu vardı.72 olgu güç kaybı yaşadığını belirtmiş ve bunun %60'ı bu belirtiyi hastalıkla ilişkilendirmiştir.60 hasta soluk almada güçlük yaşadığını ifade ederken yüzde 48'i bunu hastalıkla ilişkilendirmiştir. Ağrı belirtisi %40'ta kalmıştır, sonuç differansiye tiroid kanserli olgularda ağrı şikayetinin hastalığın belirleyici bir semptomu olmadığı yönündedir. Kilo kaybı bulgusu hastalıkla ilişkilendirilen en az sayıda belirti olurken çoğu kadın olan olguların asıl şikâyeti 'kilo alma' oldu. Ayrıca, hastalarda hastalığı anlayabilme boyutunda dikkat çekici düşüklük bulundu. Yaşam kalitesi ölçeklerinde puanlar hafif düşük izlendi. Bunun yanında olgularımızda belirgin olarak yüksek yorgunluk ve güç kaygısı, düşük yaşam kalitesi, emosyonel rol güçlüğü ve fiziksel rol güçlüğü tespit edildi. In our study, we evaluated disease susceptibility, anxiety, depression and quality of life in well differentiated thyroid cancer patients who had received radioactive iodine 131 treatment after total thyroidectomy in our clinic in the last one year. It was researched whether patients, who were in the process of new diagnosis and treatment, whose treatment was completed and the whole body scan was performed with iodine-131 for 6 months were controlled by TSH, triggered symptoms of anxiety and depression and adversely affected quality of life. Sickness Perception Scale (SFI), SF-36 Quality of Life Scale, Beck Depression Scale, Beck Anxiety Scale and socio-demographic form were applied when all diseases were evaluated. The mean age of the participants was 40.62 ± 10,28 years. There were 84 (84.0%) female and 16 (16.0%) male cases. 72 reported that they had lost power, and 60% of them attributed this symptom to the disease. 60 patients stated that they had difficulty in breathing, and 48 percent of them said it was related to the disease. The symptom of pain remained at 40%, the result is that the symptoms of pain in the differantiated thyroid cancer cases are not a defining symptom of the disease. While the symptoms of weight loss were the least number of symptoms associated with the disease, the actual complaint of most women was 'weight gain'. In addition, there was a noticeable decrease in the patient's ability to understand the disease. The scores on the quality of life scale were slightly lower. However, significantly higher fatigue and power anxiety, lower quality of life, emotional role strength and physical role weakness were found in our cases.
