Abstract
Amaç: Rejyonel anestezi teknikleri, çoğu cerrahi operasyonlarda yaygın oranda kullanılmaktadır. Çalışmamızda varis cerrahisi yapılan hastalarda uygulanan spinal anestezi ile femoral blok anestezisinin preoperatif ve postoperatif hemodinamik değerler, Mini Mental Test Skoru (MMTS), Spielberger Durumluk Kaygı Ölçeği (SDKÖ), intraoperatif sedasyon ihtiyacı, hasta, anestezist ve cerrah konfor skoru, postoperatif Vizüel Analog Skala (VAS) değerleri, ilk mobilizasyon ve taburcu olma süresi bakımından karşılaştırılmasını amaçladık.Yöntem: Elektif olarak varis cerrahisi yapılan 18-65 yaş arası ASA I-II-III olan, intraoperatif analjezi sağlamak üzere femoral blok veya spinal anestezi tekniklerinden biri uygulanan 100 hasta çalışmaya alındı. 50 hastaya femoral blok (Grup F) ve 50 hastaya spinal anestezi (Grup S) uygulandı.18-65 yaş aralığı dışındaki hastalar, gebeler, lokal anestezi ile operasyon yapılacak olanlar, acil operasyona alınan hastalar, çalışmaya katılmak istemeyen hastalar, ASA IV ve üzeri hastalar çalışmaya dahil edilmedi. Grupların preoperatif ve postoperatif hemodinamik değerleri, MMTS ile kognitif fonksiyonları, SDKÖ ile anksiyete skoru, intraoperatif midazolam, efedrin ve atropin ihtiyacı; hasta, anestezist ve cerrah konfor skoru, postoperatif VAS değerleri, ilk mobilizasyon ve taburcu olma süresi bakımından istatistiksel olarak karşılaştırıldı. Ayrıca hastalar 50 kadın ve 50 erkek olarak ikiye ayrılarak (kadın hastaların 27 tanesine spinal anestezi, 23 tanesine femoral blok yapılmış olup, erkek hastaların 23 tanesine spinal anestezi, 27 tanesine de femoral blok uygulanmıştır). Postoperatif 5 saatlik VAS skoru ve hastaların postoperatif ilk analjezik ihtiyacı süreleri karşılaştırıldı. Ayrıca hastalarınpostoperatif VAS skoru ortalamaları, ortalama taburculuk süreleri, postoperatif ilk analjezik ihtiyaç süresi ortalamaları cinsiyete göre kıyaslandı.Bulgular: Demografik veriler (yaş, cinsiyet, VKİ, ASA) bakımından her iki grup arasında istatistiksel olarak anlamlı bir fark yoktu. Ortalama arteriyel kan basıncı, intraoperatif 0, 10, 20, 30 ve 40. dakikada Grup S'de Grup F'ye göre istatistiksel olarak daha düşük bulunmuştur (p < 0,05). İntraoperatif efedrin (mg) ve atropin (mg) ihtiyacı Grup S'de Grup F'ye göre istatistiksel olarak daha yüksek bulunurken midazolam (mg) ihtiyacı Grup F'de grup S'ye göre istatistiksel olarak daha fazla bulunmuştur (p < 0,05). Hasta ve cerrah konfor skoru Grup S'de grup F'ye göre istatistiksel olarak daha yüksek, anestezist konfor skoru ise Grup F'de grup S'e göre istatistiksel olarak daha yüksek bulunmuştur (p < 0,05). Postoperatif VAS değerleri operasyon sonrası 1. ve 2. saatlerde Grup S'de Grup F'ye göre istatistiksel olarak daha düşük iken 3, 4 ve 5. saatlerde Grup F'de Grup S'ye göre anlamlı düzeyde daha düşük bulunmuştur (p < 0,05). Postoperatif SDKÖ skorlaması, her iki grupta da preoperatif değerlere göre istatistiksel olarak anlamlı derecede düşük bulunmuştur (p< 0,05). Postoperatif ve preoperatif MMTS değerleri her iki grupta da istatistiksel olarak benzer bulunmuştur (p > 0,05). Postoperatif mobilizasyon süresi ve taburculuk zamanı femoral sinir bloğu grubunda istatistiksel olarak anlamlı derecede daha kısa bulunmuştur (p<0,05).Sonuç: Spinal anestezi (SA) uygulanan grupta femoral blok (FNB) uygulan gruba göre; hasta ve cerrah konforu ve intraoperatif sedasyon daha yüksek, postoperatif 1. ve 2. saatlerde VAS skorları daha düşük bulunarak bu konuda femoral bloğa göre daha yüksek avantaj sağlanmıştır. Ayrıca postoperatif ilk analjezik uygulanma süre ortalaması kadınlarda erkeklere göre daha kısa bulundu. Varis cerrahisi yapılacak olan hastalarda, intraoperatif analjezi amacıyla femoral blok uygulanan grupta spinal anestezi uygulanan gruba göre; hemodinamik açıdan ortalama arter basıncı, nabız (Nb) ve satürasyon (SpO2) daha stabil, anestezist konforu daha yüksek, postoperatif analjezi süresi daha uzun, intraoperatif atropin ve efedrin ihtiyacı daha düşük, postoperatif mobilizasyon süresi ve taburculuk zamanı daha kısa bulunmuştur.FNB ve SA gruplarının her ikisinde de postoperatif MMTS ve postoperatif SDKÖ değerleri preoperatif değerlere göre daha düşük olmakla birlikte, iki grup arasında istatistiksel olarak herhangi bir farklılık görülmemiştir.Anahtar Kelimeler: Varis cerrahisi, Femoral Blok, Spinal Anestezi, Hemodinamik Değerler, Mini Mental Test, Spielberger Durumluk Kaygı Ölçeği, Midazolam, Efedrin, Atropin, Hasta Konfor Skoru, Cerrah Konfor Skoru, Postoperatif VAS, Anestezist Konforu, Postoperatif İlk Mobilizasyon Süresi, Postoperatif Taburculuk Süresi. Objective: Regional anesthesia techniques are used increasingly in widespread use for most surgical operations. In this study, we aimed to compare spinal anesthesia and the femoral nerve block in patients with variceal surgeries in terms of preoperative and postoperative hemodynamic values, Mini-Mental State Examination (MMSE) score, Spielberger State-Trait Anxiety Inventory (STAI), intraoperative sedation requirement, patient, anesthesiologist and surgeon comfort score, postoperative visual analog scale (VAS) values, and early mobilization and discharge time.Method: One hundred patients between 18-65 years of age with ASA I-II-III who underwent elective variceal surgeries and was used either femoral block or spinal anesthesia techniques to provide intraoperative analgesia were included in the study. 50 patients received femoral block (Group F) and 50 patients received spinal anesthesia (Group S). The patients outside the age range of 18-65 years, pregnant women, the patients who would underwent local anesthesia operations, emergency surgery patients, patients who did not want to participate in the study, ASA IV and above patients were not included in the study. Preoperative and postoperative hemodynamic values, Mini-Mental State Examination (MMSE) score and cognitive functions, Spielberger State-Trait Anxiety Inventory (STAI), intraoperative midazolam, ephedrine and atropine requirement, patient, anesthesiologist and surgeon comfort score, postoperative visual analog scale (VAS) values, and early mobilization and discharge time of the both groups were statistically compared.Results: There was no statistically significant difference between the two groups in terms of demographic data (age, sex, BMI, ASA). The mean arterial blood pressure was statistically lower in Group S in the first intraoperative 40 minutes compared to Group F (p <0.05). Intraoperative ephedrine and atropine requirement were statistically higher in Group S than Group F, while midazolam requirement was statistically higher in Group F than group S (p <0.05). Patient and surgeon comfort scores were statistically higher in Group S than group F and anesthesiologist comfort score was statistically higher in Group F than group S (p <0.05). Postoperative VAS values were statistically lower in Group S at 1 and 2 hours postoperatively than in Group F, but significantly lower at 3, 4 and 5 hours than in Group F compared to Group S (p <0.05). Postoperative Spielberger State-Trait Anxiety Inventory (STAI) score was found to be statistically significantly lower than preoperative values in both groups (p <0.05). Postoperative and preoperative MMSE values were statistically similar in both groups (p> 0.05). Postoperative mobilization duration and discharge time were found to be shorter in the femoral nerve block group at statistically significant level (p <0.05).Conclusion: Postoperative cognitive functions and anxiety scores (MMSE and STAI) were lower in both groups of femoral nerve block and spinal anesthesia than in preoperative values, but no statistically significant difference was observed between the two groups.Hemodynamic values (mean arterial pressure, pulse and saturation) were more stable, anesthesiologist comfort was higher, duration of postoperative analgesia was longer, intraoperative atropine and ephedrine requirements were lower, postoperative mobilization period and discharge time were shorter in the femoral block group for intraoperative analgesia compared to the spinal anesthsia group in patients who underwent variceal surgeries,Patient and surgeon comfort and intraoperative sedation were higher, VAS scores were lower in postoperative 1st and 2nd hours in the spinal anesthesia group for intraoperative analgesia compared to the femoral block group in patients who underwent variceal surgeries, and this means that a higher advantage was obtained with spinal anesthesia compared to the femoral nerve block.Keywords: Variceal surgery, Femoral nerve block, Spinal anesthesia, Hemodynamic values, Mini Mental State Examination, Spielberger State-Trait Anxiety Inventory (STAI), Midazolam, Ephedrine, Atropine, Patient comfort score, Surgeon comfort score, Postoperative VAS, Anesthesiologist comfort, Postoperative early mobilization period, Postoperative discharge time.
