Abstract
Bu çalışmada; gastrointestinal şikayeti olan hastalarda D.fragilis'in farklı tanı yöntemleri ile yaygınlığının araştırılması ve tanıda kullanılan yöntemlerin duyarlılık ve özgüllüklerinin belirlenmesi amaçlanmıştır. Çalışmaya çeşitli kliniklerden gelen ve gastrointestinal şikayeti olan 101 hastanın dışkı örneklerinin incelenmesinde Robinson kültür, demir hematoksilen boyama, trikrom boyam, PCR ve Real-Time PCR yöntemleri kullanılmıştır. Hasta ve kontrol gruplarından oluşan 121 dışkı örneği nativ-lügol yöntemi ile incelenmiştir. Dışkı örneklerinin robinson besiyerine kültürü yapılmıştır. Kullanılan boyama yöntemleri, PCR ve Real-Time PCR yöntemleri ile araştırma grubunda ve kültür grubunda Dientamoeba bakımından pozitiflik oranları belirlenmiştir. Hasta grubunun 13'ü (%10,7) demir hematoksilen boyamayla, 2'si (%1,7) ise trikrom boyamayla, PCR yöntemi ile 7'si (% 5,7), Real-Time PCR ile 13'ü (%10,7) pozitif olarak tespit edilmiştir. İncelemesi yapılan kültür örneğinden 7'si (%5,8) demir hematoksilen boyamayla, 4'si (%3,3) trikrom boyamayla, PCR yöntemiyle 2'si (%1,7), Real-Time PCR'da 3'ü (%2,5) pozitif olarak tespit edilmiştir. Demir hematoksilen boyama sonucunda, ishalli dışkılarda pozitif örneğe rastlanmazken, ishalli olmayan örneklerin 13'ü (%10,7) pozitif bulunmuştur. İshalli dışkıların trikrom boyama yöntemi ile boyanması sonucunda pozitif örneğe rastlanmamıştır. İshalli olmayan dışkılarda ise trikrom boyamayla 2'si (%1,7) pozitif bulunmuştur. PCR yöntemi ile ishalli dışkıların 1'i (% 1) pozitifken, ishalli olmayan dışkıların 6'sı (% 4,9) pozitif bulunmuştur. Real-Time PCR yönteminde ise ishalli dışkılarda pozitif örneğe rastlanmazken, ishalli olmayan dışkılardan 13'ü (%10,7) pozitif olarak tespit edilmiştir. İncelenen dışkı örneklerinde Real-Time PCR yöntemi esas alınarak yapılan sensivite ve spesifite değerleri sırasıyla demir hematoksilende %46 ve %93, trikrom boyamada %0 ve %99, PCR'da %54 ve %100 olarak bulunmuştur. Real-Time PCR yöntemi esas alınarak yapılan kültür örneklerine ait sensivite ve spesifite değerleri sırasıyla; demir hematoksilende %67 ve %96, trikrom boyamada %33 ve %98, PCR'da %67 ve %100 olarak bulunmuştur.Anahtar Kelimeler: Dientamoeba, D.fragilis, Tanı, Kayseri In this study; we aimed that investigate the prevalence of D.fragilis with different diagnostic methods in patients with gastrointestinal symptoms and determine the sensitivity and specificity of this diagnostic methods have been used. 101 patients who were coming from various clinics and had gastrointestinal complaints were included and fecal samples examined with Robinson culture, iron hematoxylin staining, trichrome dye, PCR and Real-Time PCR methods. Patients and the control groups consisting of 121 stool samples were examined by native-iodine method. Stool samples that examined were cultured with Robinson medium. Positivity rate of the research group and culture group was determined using the dyeing methods, PCR and Real-Time PCR. 13 patients (10.7%) with iron hematoxylin staining, 2 patients (1.7%) with trichrome staining, 7 patients (5.7%) by PCR and 13 patients (10.7%) by Real-Time PCR were positive. Culture samples were positive with iron hematoxylin staining, trichrome staining, PCR and Real-Time PCR 7 (5.8%),4 (3.3%), 2 (1.7%), 3 (2.5%) rates, respectively. While diarrheal stool samples were not found as positive with iron hematoxylin staining, 13 samples of non-diarrheal stool samples (10.7%) were positive. Diarrheal stool samples with trichrome staining method were not identified any positive samples, as 2 (1.7%) of non-diarrheal stool sapmles were found positive. 1 (1%) of the diarrheal stools were identified as positive, while 6 (4.9%) of non-diarrheal stools were positive by PCR. The diarrheal stools were not positive, while 13 (10.7%) of non-diarrheal stools were positive by Real-Time PCR. Sensitivity and specificity were found to be values of 46% and 93% in iron hematoxylin staining, 0% and 99% in trichrom staining, 54% and 100% by PCR at examined stool samples based on Real-Time PCR method. Sensitivity and specificity were found to be values of 67% and 96% in iron hematoxylin staining, 33% and 98% in trichrom staining, 67% and 100% by PCR at culture samples based on Real-Time PCR method.Key Words: Dientamoeba, D.fragilis, Diagnosis, Kayseri
