Abstract
Otoimmün tiroid hastalıkları (OİTH), bağışıklık sisteminin regülasyon bozukluğundan kaynaklanan ve tiroid bezi üzerine bir immün saldırısı başlatır. Hashimoto tiroiditi (HT) tiroid bezinin kronik bir enflamasyonudur. En sık görülen otoimmün hastalığın yanı sıra hipotiroidinin en yaygın nedeni olarak kabul edilir. Bu çalışmanın amacı, Hashimoto tiroiditi (HT) ve tiroid otoantikorları, serum selenyum eser element düzeyleri ve ötiroid ve subklinik hipotiroid aşamalarındaki oksidatif stres parametreleri arasındaki ilişkinin incelenmesidir. Çalışmaya toplam 100 hasta dahil edildi; Bunlardan 71'i yeni HT tanısı almış (ötiroidte 51 hasta ve subklinik hipotiroid alt grupta 20 hasta), 18 yaş üstü ve daha önce herhangi bir tedavi almayan ve 29'u sağlıklı gönüllü kişilerden oluşmaktadır. Ortalama total oksidan durum (TOS), kontrol grubuna göre daha yüksek seviyede bulundu. TAS düzeyleri ötiroid ve subklinik hipotiroid grupları arasında belirgin bir farklılık göstermezken, kontrol grubuna göre düşük seviyelerde olup anlamlı bulunmuştur. Çalışma grupları arasında serum selenyum düzeleri arasında anlamlı bir fark gözlenmemiştir. Bulgularımız, HT'li hastalarda hipotiroidin gelişimi sırasında oksidatif stresin sürekli arttığını göstermektedir. Selenyum eser elementi HT hastalarında doğrudan tiroid fonksiyonunu etkileyebilir.Anahtar kelimeler: Otoimmün tiroid hastalıkları, Hashimoto tiroiditi, oksidatif stres, Selenyum Autoimmune thyroid diseases (OİTH) result from a deregulation of the immune system leading to an immune attack on the thyroid gland. Hashimoto thyroiditis (HT) is a chronic inflammation of the thyroid gland. It is considered to be the most common autoimmune disease, as well as the most common cause of hypothyroidism. The aim of this study was to study the relationship between Hashimoto's thyroiditis (HT) and thyroid autoantibodies, serum selenium trace element levels and oxidative stress parameters in euthyroid and subclinical hypothyroid stages. A total of 100 patients were included in the study; 71 of whom were newly diagnosed with HT (51 patients in the euthyroid, and 20 patients in subclinical hypothyroid subgroups), aged over 18 and had not received any prior treatment and 30 of whom were healthy volunteers. The total oxidant status (TOS) levels were higher in patient groups compared to controls. The mean serum selenium levels were similar between the control, euthyroid and subclinical hypothyroid groups. The total antioxidant status (TAS) levels were lower in Euthyroid and subclinical hypothyroidism patients in comparison to control group. Our results suggest that oxidative stress increases continuously during the development of hypothyroidism in patients with HT. Selenium trace element may directly influence the thyroid function in patients with HT.Keywords: Autoimmune thyroid diseases, Hashimoto thyroiditis, oxidative stress, Selenium
