Kemik iliği transplantasyonu yapılan hastalarda cytomegalovırus (CMV) antijeni ve CMV DNA`nın araştırılması

Abstract
Amaç: CMV immün sistemi normal kişilerde genellikle asemptomatik hastalık oluşturmasına rağmen, transplant alıcıları ve HIV/AIDS hastaları gibi immün sistemi baskılanmış hastalarda ciddi mortalite ve morbidite nedeni olmaktadır. Bu çalışmada kemik iliği transplantasyonu (KİT) yapılan hastalardan elde edilen kan örneklerinde elde edilen CMV DNA sonuçları ile CMV pp65 antijen test sonuçlarının karşılaştırılması amaçlandı.Materyal ve Metod: Erciyes Üniversitesi Tıp Fakültesi M. Kemal Dedeman Hematoloji-Onkoloji Hastanesi, Hematoloji bölümü poliklinik ve servislerinde takip edilen KİT yapılan hastalar çalışmaya dahil edildi. Çalışmaya alınan 156 hastaya ait örnek pp65 antijenemi testi indirekt immunofloresan yöntemi (CINA kit, Argene-Biosoft, Fransa) ile CMV DNA ise real time PCR (RT-PCR) (Artus® CMV QS-RGQ Kit, QIAGEN, Almanya) yöntemi ile araştırıldı. Antijenemi testinde ≥ 1 hücre/200.000 pozitifliğine karşılık gelen viral yük düzeyi ise ROC analizi ile belirlendi.Bulgular: Çalışmaya dahil edilen 174 örneğin; 121 (%69.5)'inde antijenemi ve CMV DNA negatif, 33 (%19) örnekte her iki test sonucu pozitif bulundu. pp65 antijenemi testi negatif 20 hastaya ait örnekte (%11.5) CMV-DNA pozitif saptandı. Her iki test ile pozitif saptanan örnekler değerlendirildiğinde, plazma CMV-DNA düzeyi ile pp65 pozitif hücre sayısı arasındaki korelasyon istatistiksel olarak anlamlı bulundu (r= 0.660). ROC analizi ile ≥1 pozitif hücre/200.000 hücre antijenemi pozitifliğine karşılık gelen CMV-DNA düzeyi 152 IU/ml olarak tespit edildi.Sonuç: Sonuç olarak antijenemi ve CMV DNA test sonuçlarının uyumlu olduğu ve bir antijen pozitifliğine karişılık gelen eşik CMV DNA düzeyinin 152 IU/ml olduğu bulundu. Ancak her merkezin kendi koşullarına uygun eşik değerleri belirlemesi gerektiği kanaatine varıldı.Anahtar Kelimeler: Cytomegalovirus, CMV antijenemi, CMV DNA, kemik iliği transplantasyonu Aim: Although CMV usually causes asymptomatic disease in immuncompetent person, it causes serious mortality and morbidity in immunocompromised patients such as transplant recipients and HIV/AIDS patients. In this study, we aimed to compare CMV DNA RT-PCR results and CMV pp65 antigen test results in blood samples obtained from bone marrow transplant recipients.Materials and Methods: The patients who underwent bone marrow transplantation and being followed at hematology policlinics and clinics of Erciyes University Medical Faculty, M. Kemal Dedeman Hematology-Oncology Hospital were included in the study. CMV DNA was investigated by real-time PCR (RT-PCR) (Artus CMV QS-RGQ Kit, QIAGEN, Germany) and CMV antigen was investigated by indirect immunofluorescence assay (CINA kit, Argene Biosoft, France) in blood samples obtained from 156 patients. The viral load level corresponding to ≥ 1 cell / 200.000 positivity in the antigenemia test was determined by ROC analysis.Results: Totally 174 samples were included in the study; 121 (69.5%) were negative for indigenous antigen and CMV DNA, and 33 (19%) were positive for both tests. CMV-DNA was found positive in 20 patients that were found antigen negative (11.5%). When positive samples were evaluated with both tests, the correlation between plasma CMV-DNA level and number of pp65 positive cells was statistically significant (r = 0.660). ROC analysis revealed a CMV-DNA level of 152 IU / ml, corresponding to a positive of ≥1 positive cells / 200,000 cell antigens. Conclusion: As a result, it was found that antigenemia and CMV DNA test results were compatible and that the threshold CMV DNA level, which corresponds to one antigen positivity, was 152 IU/ml. However, it was concluded that each center should determine CMV DNA threshold values appropriate to its conditions.Key words: Cytomegalovirus, CMV antigenemia, CMV DNA, bone marrow transplantatio
