dc.description.abstractAmaç: Acil servise başvuran toraks travmalı hastalarda primer bakıdabakılan Entegre Pulmoner İndex değerinin; toraks travma ayırıcı tanılarındakullanılabilirliğini değerlendirerek Entegre Pulmoner İndex (EPİ) değerinin hayatıtehdit edici göğüs yaralanmalarının tespitindeki rolünün araştırılması amaçlandı.Materyal ve Metod: Prospektif gözlemsel kontrollü tanı testi performansçalışması olarak dizayn edilmiş çalışmaya, Bakırköy Dr. Sadi Konuk Eğitim veAraştırma Hastanesine 25.03.2019 ile 11.06.2019 tarihleri arasında başvurmuş, 18yaş ve üstü, izole toraks travmalı 134 hasta ve aktif şikayeti olmayan gönüllü 101sağlıklı kontrol grubu alındı. Primer bakı aşamasında hastaların Entegre Pulmonerİndeks Skoru ölçüldü.Bulgular: Çalışmaya alınan izole toraks yaralanmalı 134 hastanın yaşortalaması 40,16±18,996, kontrol grubunun yaş ortalaması 52,50±21,718 idi.Çalışmaya alınan travma hastalarının EPİ skoru ortalaması 8,10±1,560, kontrolgrubunun EPİ skoru ortalaması 9,30±0,920 bulunmuş olup; istatistiksel olarakanlamlı fark bulundu (p=0,000). EPİ skoru ile end tidal karbondioksit basıncı,periferik oksijen saturasyonu, sistolik kan basıncı, diastolik kan basıncı, ortalamaarteryel kan basıncı ve GKS ile arasında istatiksel olarak anlamlı pozitif korelasyon;solunum sayısı, nabız sayısı ve laktat düzeyi ile arasında istatiksel olarak anlamlınegatif korelasyon saptandı. EPİ skoru cut off >7 alındığında tüp torakostomiişlemini %91,62 sensitivite ve %42,86 spesifite ile hemotoraks tanısını %91,76sensitivite ve %45,28 spesifite ile bulmuştur (p<0.001, p<0.001).Sonuç: Travma hastalarının vital değerleri ve laktat düzeyi ile korele olan cutoff değeri >7 alındığında tüp torakostomi işlemini %91,62 sensitivite ve %42,86spesifite ile cut off değeri >7 alındığında hemotoraks tanısını %91,76 sensitivite ve%45,28 spesifite ile bulan EPİ skoru kritik toraks travmalı hastalarındeğerlendirilmesinde, tespitinde ve izleminde kullanılabilir.Anahtar Kelimeler: Acil Servis, Toraks Travması, Entegre Pulmoner İndek
dc.description.abstractAim: The aim of this study was to investigate the role of IntegratedPulmonary Index (IPI) in diagnosis of life threatening thorax injuries during primarysurvey by assessing its usefulness in differential diagnosis of thoracic pathologies inpatients with thoracic trauma admitted to the emergency service.Materials and Methods: Our study was designed as a prospectiveobservational controlled diagnostic test performance study in which 134 patientsaged 18 years and older with isolated thoracic trauma and 101 healthy volunteers ascontrol group who admitted to Dr. Sadi Konuk Training and Research Hospitalbetween the dates 25.03.2019 and 11.06.2019 were included. IPI score was measuredduring primary survey phase.Results: The mean age of 134 patients with isolated thorax injury was40,16±18,996 and mean age of control group was 52.50±21.718. The mean IPI scoreof trauma patient included in the study was 8.10±1.560 and the mean IPI score of thecontrol group was 9.30±0.920 and statistically significant difference was found(p=0.000). Statistically significant positive correlation was found between IPI scoreand end-tidal carbon dioxide pressure, peripheral oxygen saturation, systolic bloodpressure, diastolik blood pressure, mean arterial blood pressure and Glasgow ComaScale. There was a statistically significant negative correlation between IPI score andrespiratory rate, pulse rate and serum lactate levels. When cut off value of IPI scorewas taken >7, the sensitivity of tube thoracostomy procedure was 91,62% and thespecificity of tube thoracostomy procedure was 42,86% and the sensitivity ofhemothorax diagnosis was 91,62% and the specificity of tube thoracostomydiagnosis was 42,86% by means (p<0.001, p<0.001).Conclusion: In our study IPI score was correlated with vital signs and serumlactat levels and when cut off value of IPI score was taken >7, the sensitivity was91,62%, the specificity was 42,86% for tube thoracostomy procedure and thesensitivity was 91,76%, the specificity was 45,28% for hemothorax diagnosis. As aresult IPI score can be used in the evaluation, detection and follow-up of patientswith critical thoracic trauma.Key words: Emergency Service, Thoracic Trauma, Integrated PulmonaryIndexen_US
