Modifiye pendulum apareyi yardımı ile üst molar distalizasyonunun incelenmesi

Abstract

Bu çalışmamızın amacı; İskeletsel I, Dişsel II. Sınıf malokluzyona sahip bireylerde ağız dışı kuvvetleri ve Modifiye Pendulum apareyini üst molar dişlerin distalize edilmesinde ayrı ayrı kullanarak, yöntemlerin birbirlerine göre avantaj ve dezavantajlarını ortaya koymaya çalışmaktır. Araştırma grubunu 40 birey oluşturmuştur. 20 bireyin sağ ve sol üst molarları ağız dışı kuvvetler, diğer 20 bireyin sağ ve sol üst molarları Modifiye Pendulum apareyi ile distalize edilmiştir. Ağız dışı kuvvetler yardımı ile molar distalizasyonu yapılan grupta yaş ortalaması 11 yıl 9 ay, Modifiye Pendulum grubunda ise 12 yıl 3 aydır. 80 adet lateral sefalogram üzerinde apareylerin oluşturduğu distalizasyon miktarı, iskeletsel ve dişsel değişimler incelenmiştir. İstatistiksel değerlendirmede; aynı gruba ait değişimler Wilcoxon testi, gruplar arası değişimler ise Mann- Whitney `U` testi ile karşüastirılmıştir. Her iki grupta da üst molar dişler önemli derecede distalize edilmişlerdir. Modifiye Pen dulum apareyi uygulanan grupta üst motorlar üç aylık sürede daha hızlı distalize olmuş, Angle Sınıfl molar ilişkisine oturmuşlardır. Ağız dışı kuvvet uygulanan grupta hasta kooperasyonu sorun yaratmış, Modifiye Pendulum apareyi uygulanan grupta ise böyle bir problemle karşılaşılmamıştır. 65-

SUMMARY The purpose of our study is; to use extra-oral forces and Modified Pendulum appli ance in the distalization of the upper molars in the cases with Skeletal Class I, Dental Class II malocclusions, in order to compare the advantages and disadvantages of the methods. The research group is existed of 40 individuals. Upper right and left molars of 20 individuals are distalized using extra-oral forces and the upper right and left molars of the other 20 individuals are distalized with the use of Modified Pendulum appliance. The average age; in the extra-oral force group is, 1 1 years 9 months and in the group using Modified Pendulum appliance is, 12 years 3 months. On the lateral cephalograms which are taken before and after distalization; the degree of distalization, skeletal and dental variations are studied. In the statistical evaluation, the variations in the same group are compared with Wilcoxon test and the variations between groups are compared with Mann- Whitney `U` test. The upper molars are distalized with a valuable degree of importance in both of the groups. The molars in the Modified Pendulum appliance group are distalized faster and more in three months time and settled in Angle Class I molar relationship. In the group using extra-oral force, patient cooperation is seen as a problem influencing orthodontic treatment however there is no such problem in Modified Pendulum group. -66
