Hastanelerde talep tahmini ve hasta hizmetlerinin planlanması: GATA Eğitim Hastanesinde bir uygulama

Abstract

VI. 02ET Birer ekonomik işletme olarak sağlık kurumlarının verimli ve etkili çalışabilmesi için hizmet planlamasına ağırlık vermesi gerekmektedir. Hizmet planlaması çalışmalarının temelinde ise talip tahmini bulunmaktadır. Talep tahmini ile hastane yönetimi, kapasite planlamasını ve kaynak tahsisini kolayca gerçekleştirebilir. Bu çalışmanın temel amacı, hastanelerde hizmet planlamasının ve hizmet planlamasında talep tahmini sonuçlarından nasıl yararlanıldığını ortaya koymaktır. Araştırma evrenini GATA Eğitim Hastanesi oluşturmaktadır. Bu çalışmada hastanenin, poliklinik hizmetleri, yatan hasta hizmetleri, laboratuar hizmetleri vb. ile ilgili son 5 yıllık aylık verileri, arşiv kayıtları ve bilgi işlem merkezi verilerinden yararlanılarak toplanmıştır. Geçmişe yönelik veriler regresyon analizi ile incelenerek, taiep tahmin modeli geliştirilmeye çalışılmıştır. Araştırma sonucunda, hastane poliklinikleri, klinikleri, özel hizmet birimleri ve teşhis üniteleri ile ilgili talep tahmini modelleri geliştirilmiş, bu modeller kullanılarak geleceğe yönelik öngörüler yapılmış ve hastane hizmet kapasitesi göstergesi olan yatak sayısının ne olması gerektiği ortaya konulmuştur. 127

VII. SUMMARY Health institutions are described as economic enterprises so that they should focuse on health services planning in order to serve in an efficent way. The health services planning studies are based on the prediction of service demand. By using the demand prediction, hospitals can easily achieve the highest capacity planning and resource allocation to serve their population. The purpose of this study to develop a demand forecasting model for the hospital service planning and apply the predicted model for the Turkish Army Hospitals. The study was contucted at Gülhane Military Medical Academy. For the purpose of the study, hospital outpatient records, hospital inpatient records, laboratory services' records, hospital archives, and the hospital computing services' data set were used and the selected years for the analysis was 1995-1999. The regression analysis was used to develop the demand prediction. models for each specific hospital department. At the end of the research, the hospital service demand forecasting models were developed for the outpatient clinics, the hospital inpatient clinics, the diagnostic units, and the developed models were used to predict future hospital service utilizations. Finally, the results of the study helped the hospital administrator to find out the real inpatient bed numbers to serve its army population and their dependents. 128
