Sınıf II bölüm I maloklüzyonu olan bireylerin tedavisinde kullanılan `forsus nitinol flat spring` apareyinin diş-çene-yüz sistemi üzerindeki etkilerinin incelenmesi

Abstract

ÖZET: Çalışmamızın amacı, Forsus Nitinol Flat Spring apareyinin etkinliğinin Jasper Jumper apareyi ile karşılaştırılmasıdır. Çalışmamız, aşağıdaki özelliklere sahip 20 hasta üzerinde yürütülmüştür:. İskeletsel ve dişsel Sınıf II Bölüm I malokluzyon.. Retrognatik mandibula.. Horizontal ya da normal büyüme paterni.. Aktif büyüme döneminde olan hastalar. Birinci gruptaki 10 bireye Forsus Nitinol Flat Spring apareyi, ikinci gruptaki 10 bireye ise Jasper Jumper apareyi uygulanmış ve Sınıf I azı ilişkisi elde edildikten sonra apareyler çıkartılmıştır. Dişlerin seviyelenme aşamasında sonra ve apareyler çıkartıldıktan sonra lateral sefalometrik röntgenler ve alçı modeller alınmıştır. Grup içi farklılıklar Wilcoxon analizi ile belirlenmiş, gruplar arası farklar ise Mann Whitney U testi ile tespit edilmiştir. Her iki apareyinde üst çene üzerinde retrüziv bir etkisinin olmadığı saptanmıştır. Diğer yandan iki apareyinde alt çenenin gelişimini stimule ettiği görülmüştür. İki grupta da ön ve arka yüz yükseklikleri artmış, üst kesici dişlerde uzama ve retrüzyon görülürken alt kesiciler de gömülme ve protrüzyon olduğu tespit edilmiştir. Ayrıca, üst büyük azı dişlerindeki gömülme ve distalizasyonun aksine alt büyük azılarda, uzama ve meziale hareket belirlenmiştir. Forsus Nitinol Flat Spring ve Jasper Jumper apareyleri ile tedavi sırasında alt ve üst dental arklarda genişleme olmuştur. Araştırmamızın sonunda, Forsus Nitinol Flat Spring ve Jasper Jumper apareylerinin benzer dişsel ve iskeletsel etkilerinin olduğu tespit edilmiş, Forsus Nitinol Flat Spring apareyinin, distal kapanışın ve artmış overjetin düzeltilmesinde diğer sabit fonksiyonel apareylere bir alternatif olarak uygulanabileceği sonucuna varılmıştır. 107

SUMMARY: The aim of this study is to compare the efficiency of the Forsus Nitinol Flat Spring, with the Jasper Jumper appliance. Our study is carried out on 20 patients with following characteristics;. Skeletal and dental Class II Division I malocclusion.. Retrognatic mandible.. Horizontal or normal growth pattern.. Patients in active growth period. The Forsus Nitinol Flat Spring was applied to the first group and the Jasper Jumper appliance to the second group of 10 patients and the appliances were taken out when the Class I molar relationship was obtained. Lateral cephalometric radiograms and cast models were taken after levelling phase and after the appliances were taken out. The variations in the same group are determined with Wilcoxon analysis and the differences between the two groups are evaluated with Mann Whitney U test. It is determined that, neither the Forsus Nitinol Flat Spring, nor the Jasper Jumper appliance has a retrusiv effect on the maxilla. On the other hand, both of the appliances stimulated the mandibular growth. Vertical anterior and posterior face heights and Y-axis increased, in both of the groups. It is evaluated that, upper incisors are intruded and retruded while lower incisors are extruded and protruded. And also, upper molars are intruded and drifted distally, contrary to lower molars which are extruded and drifted mesially in both of the groups. During the treatment with Forsus Nitinol Flat Spring and with Jasper Jumper appliances, broadening occured in upper and lower dental arches. It is determined that, both of the appliances have similar skeletal and dental effects. Forsus Nitinol Flat Spring provides an alternative to other fixed functional appliances and it can be used to improve distoocclusion and excessive overjet. 108
