Basit böbrek kistli hastalarda böbrek fonksiyonları, hipertansiyon yaygınlığı ve kan değerlerinin araştırılması

Abstract

ÖZET Bu çalışmada hastanemize başvuran 1291 hastadan Radyoloji servisi ultrasonografi merkezinde basit böbrek kisti saptanan 57 hasta ve 20 sağlıkıl bireyde, hastaların başvuru nedenleri, böbrek fonksiyonlarının durumu, hipertansiyon yaygınlığı, kan değerleri ile serum er- itropoietin seviyeleri araştırıldı. Hastaların 25'i erkek, 22'si kadındır (1/1,6) Yaş ortalaması 62,03+5,87 olan hastaların hepsi 40 yaşın üzerindedir. Yaşın ilerlemesi ile kist görülme sıklığı artmıştır. Başvuru semptomları; Ağrı % 28,07 miksiyon bozukluğu % 21,05, Hematüri :% 10,52, miksiyon bozukluğu ile ağrı beraberliği % 8,77, karında kitle % 5,26, Hipertansiyon % 5,26 olarak saptanmıştır, % 21,05 hasta asemptomatiktir. Hastalarda üre değerleri normal sınırlar içerisindedir. Kreatinin değeri de normal sınır lar içerisinde olmasına rağmen, 5 cm üzerinde kistli hasta grubu(Grup m), kontrol grubu ve 1-3 cm çaplı kistli hasta grubu (Grup I) ile kıyaslandığında kreatinin değerlerinin anlamlı olarak arttığı gözlenmiştir. (P < 0,05) Kreatinin klirens değerleri normal sınırlarda olup, IH grup; kontrol ve 1. gruba göre anlamlı azalma göstermiştir. (P<0,05) 24 saatlik proteinüri değerleri normal sınırlarda bulunmuştur. Hipertansiyon sıklığı % 45,61 olarak bulunmuş ve kist boyutu arttıkça hipertansiyon görülme sıklığının arttığı gözlenmiştir. 57 basit böbrek kistli hastada ve kontrol grubunda eritrosit, hemoglobin ve hematokrit değerleri ile serum eritropoietin seviyeleri normal sınırlarda saptanmış ve serum eritropoietin seviyelerinin kistli hastalarda kontrol grubuna göre anlamlı olarak yüksek bulunduğu ortaya konulmuştur. SUMMARY In this study, 57 simple renal cyst patients and 20 healthy subjects of the total of 1291 patienst, referred to the Ultrasonography center of Radiology Department of our hospital, were investigated for the complaints, conditions of renal functions, hypertension prevalence, hematologic values and serum erythropoietin levels. 57 patients (35 male, 22 female; 1/1,6) mean age 62,03±5,87, were all older than 40. There ascertained a good correlation betwen the aging and the augmentation of the frequency of simple cysts. The symptoms and signs of the applied patients were pain (% 28,07), micturation disor der (%21,05), hematuria (% %10,52), micturation disorder with pain (%8,77), abdominal mass (% 5,26) and hypertension (% 5,26). The percentage of the asymptomatic patients was 21,05. The urea levels were found to be normal. Despite the normal creatinine levels, when comparing the group of patients having the cyst diameter greater than 5 cm (Group DI) with the control group and the group of the patients having the cyst diameter between 1 and 3 cm(Group I); the serum creatinine levels of Group IE were found to be significantly higher than the levels of both the control group and group I (P<0,05). Creatinin clearance values were found to be normal for all groups. But the levels of creatinine clearance of (Group IE) were found to be significantly lower than the control group and Group I (P< 0,05) The 24 - hour proteinuria levels were normal for all groups. The hypertension frequency was determined as % 45,61 in parallel to the increment of the cyst diameters, the hypertension frequency was found to be increased. The levels of red blood cell hemoglobine, hematocrit and the serum erythropoietin were all in the normal range for both control and the cyst groups.
