Laparoskopik kolesistektomi ile açık kolesistektominin karşılaştırılması

Abstract

ÖZET GATA Haydarpaşa Eğitim Hastanesi Genel Cerrahi Servisinde Kasım 1992 - Mayıs 1995 tarihleri arasında laparoskopik kolesistektomi uygulanan 50 hasta ve kontrol grubu olarak açık kolesistektomi uygulanan 50 hasta olmak üzere toplam 100 hasta prospektif olarak araştırılmıştır. Bu araştırmada laparoskopik kolesistektomi ile açık kolesistektomi ameliyat süreleri, postoperatif hastanede yatış süreleri, postoperatif solunum fonksiyonları ve morbidite - mortalite oranlarının irdelenmesi suretiyle karşılaştırılmışlardır. Operasyon süresi açısından laparoskopik kolesistektomi bir dez avantaja neden olurken, hastanede yatış süresinin kısalığı açısından açık yönteme göre belirgin derecede üstündür. Morbidite ve mortalite oranları her iki yöntemde de hemen hemen aynıdır ve aralarında istatistiksel olarak anlamlı bir fark bulunamamıştır. Postoperatif solunum fonksiyonları laparoskopik kolesistektomi uygulanan hastalarda daha iyidir ve alınan kozmetik sonuçlar çok başarılıdır. Laparoskopik kolesistektominin iyi bir hasta seçimini takiben tecrübeli bir ekip tarafından uygulandığında semptomatik kolelitiazisli hastalarda en geçerli tedavi olan açık kolesistektomiye alternatif bir cerrahi seçenek olduğu kanısına varılmıştır.

SUMMARY In this study, 50 patients who were subjected to laparoscopic cholecistectomy and 50 patients who were subjected to open cholecistectomy were analyzed prospectively between November 1992 - May 1995 in Surgical Departmant of Gülhane Military Medical Academy Haydarpaşa Training Hospital. The patients with laparoscopic cholecistectomy and open cholecistectomy were compared with each other according to the duration of the operation and postoperative hospital stay, postoperative respiratory functions and morbidity - mortality rates. Although laparoscopic cholecistectomy has a disadvantage about the duration of operation, it is superior to open cholecistectomia about the duration of hospital stay. The morbidity and mortality rates between two techniques are approximately the same and there is no statistical difference between them. Postoperative respiratory functions are better and cosmetic results were highly successful in laparoscopic cholecistectomy. It has been concluded that laparoscopic cholecistectomy which is gold standard therapy in cholelithiasis if appropriate patients and skilled surgical team get together is an alternative surgical technique to open cholecistectomy. 46
