Torakolomber bölgede transpediküler fiksasyon enstrümantasyon sisteminin kullanılması ile alınan sonuçlar ve oluşan komplikasyonlar

Abstract

ÖZET Bu çalışmada, spinal kolon stabilizasyon bozukluklarında, stabilizasyonu sağlamak amacıyla kullanılan TF enstrumantasyon sisteminin her yönüyle değerlendirilmesi planlandı. Çalışmada TF enstrumantasyon sistemi uygulanan hastalarda preoperatif, operatif ve postoperatif değerlendirmeler yapıldı. Tüm hastalarda instabilite tanısı, klinik instabilite tablosuna göre kondu. Bu amaçla her hastada preoperatif dönemde direkt grafiler, myelografi, BT ve MRI tetkikleri elde edildi ve bu sonuçlar nörolojik tablo ile birleştirilerek hastaların instabil olup olmadığına karar verildi. 25 hastanın tamamı buna göre instabil kabul edildiler. Postoperatif dönemde elde edilen tetkiklerin sonuçları nörolojik tablo ile birleştirilip preoperatif dönemdekilerle karşılaştırılmalar yapıldı. Hastaların hepsi postoperatif dönemde stabil kabul edildiler. Hastaların hepsinde stabilizasyonu sağlamak amacıyla, posterior girişimle Diapazon TF enstrumantasyon sistemi uygulandı, Uygulamanın ilk dönemlerinde çeşitli zorluklar ve komplikasyonlarla sıkça karşılaşmamıza rağmen, son dönemlerde sisteme gittikçe adapte olduk. Bu çalışmada, TF vida- rod enstrumantasyon sisteminin uygulanan diğer sistemlerle karşılaştırıldığında, literatürlerde sunulanlara paralel olarak belirgin üstünlüklere sahip olduğu ortaya kondu. Biz ek olarak, vidanın fraktür mevcut ama pedikülleri sağlam vertebralarada uygulanabileceği ve sistemin tüm standartlara uygun kullanıldığında deformitelerin düzelmesi için ek bir reduksiyona gerek olmadığı sonucunu çıkardık. 60

SUMMARY In this study, it is planned to evaluate the TF instrumentation system which is used to provide the stabilization of the spine in the conditions of the instabilizations. All the patients who are applied the TF instrumentation system are observed during the preoperative, operative and postoperative periods. Instability were determined by using the checklist of the clinic instability scale for all patients. For this purpose in the preoperative period, direct graphies, myelography, CT and, or MRI were obtained and by combining those with the neurological situations of the patients were instable or not. According to this procedure all the 25 patients were accepted to be instable. In the postoperative period, we again evaluated our patients by the results of radiological procedures and neurological situations. According to this procedure, all of the patients were accepted to be stable. Diapazon TF instrumentation system was applied to gain the stabilization of all the patients via posterior approach. Altough, we faced several difficulties and complications in the initial part of the application period, we adapted the system completely in the later period. It was observed in this study that in parallel with literature TF screw-rod instrumentation system has very superiority to the other systems. In addition to this, we saw the screws also can be used in vertebra which contain fracture but have healty pedicles and that an additional reduction is not required to regulate the deformities when the system is used properly with all the standards. 61
