dc.description.abstractÖZET Bu çalışmanın amacı, erişkin femur kırıklarının kilitli intramedüller çivileme ile tedavisinin klinik olarak sonuçlarını tartışmaktır. Gülhane Askeri Tıp Akademisi Haydarpaşa Eğitim Hastanesi, Ortopedi ve Travmatoloji Kliniğinde, Temmuz 1996 - Temmuz 1999 tarihleri arasında, 21 hastadaki, 21 femur kırığı primer veya sekonder tedavi yöntemi olarak kilitli intramedüller çivileme ile tedavi edildi. Bu olguların 17'sinin ortalama 26.9 hafta (en kısa 20- en uzun 56 hafta) takipleri yapıldı. Yirmibir olgumuzdan 3 tanesi bayan, 18 tanesi erkekti. Yaşlan ortalama 30.5 ti (20- 69 yaş). Olgularımızdan 1 tanesi ateşli silah yaralanmasına bağlı açık kırık diğerleri kapalı kırıktı. Takibi tamamlanan 17 olgunun ortalama kaynama süresi 16.5 hafta, varus- valgus açılanması ortalama 5.3°, ortalama ön- arka açılanması tek olguda 10° olarak oldu. Kırık iyileşmesinin ardından sonuçlan Thoresen kriterlerine gör değerlendirildi. Buna göre, 10 olgu mükemmel, 5 olgu iyi, 2 olgu orta olarak değerlendirildi. Dört hastada diz ekleminde kısmi hareket kaybı oldu. İki olgumuzda 5° lik rotasyonel deformite oldu. İki olgumuzda lcm. kısalık oldu. Takibi tamamlanan tüm olgulanmızın kınğı kaynadı. Bir olgumuzda kronik osteomyelit gelişti. HBO ve antibioterapi uyguladık. Geniş endikasyon alam, yüksek basan oranlan nedeniyle erişkin femur tanklarının tedavisinde kilitli intramedüller çivilemenin uygun bir tedavi seçeneği olduğu sonucuna vanldı. 69
dc.description.abstractSUMMARY The purpose of this study was to discuss the clinical results of the femoral fractures which were treated by interlocking intramedullary nailing. Between July 1996- July 1999, locking intramedullary nailing were performed as a primary or secondary treatment for 21 femoral fractures in the Orthopaedics and Traumatology Department of Gülhane Military Medical Academy Haydarpaşa Training Hospital. The average follow up period was 26.9 weeks (range, 20-56 weeks) in 17 patients. There were 3 females and 18 males. The average age of patients was 30.5 years (range, 20- 69 years). All the fractures but one were closed type fracture, open fracture was occurred by gun shoot trauma. The average union time the followed 17 patients was 16.5 weeks, the average varus- valgus angulation was 5.3°. Only one fracture was detected with 10° anterior- posterior angulation. The results were evaluated according to Thoresen criteria. According this evaluation system, 10 patients were excellent, 5 patients good, 2 patients moderate. There were minimal knee stifhess in 4 patients, 5° rotational deformity was detected in only 2 patients. One centimeter limb length discrepancy, which did not affect to the patients' functional situation, was detected in two patients. Complete fracture union was obtained. Only 1 patients had chronic osteomyelitis. He has been treating with hyperbaric oxygen and antibiotherapy. Many treatment protocols have been performing in the adult femur fractures one of these methods is locking intramedullary nailing which has advantages to other techniques. Since this method surves high successful rates in many indications of femur shaft fractures, we concluded that locking intramedullary nailing was useful surgical technique in adult femur fractures. 70en_US
