Uç-yan periferik sinir onarımlarında, ven ve ven+kas greftleri ile elde edilen fonksiyonel iyileşmenin sinir grefti ile karşılaştırması (deneysel çalışma)

Abstract

VII. ÖZET Periferik sinir travmalarında uç-yan sinir tamiri proksimal sinir segmenti.ulunamayan vakalarda bir çözüm olarak bildirilmiş olsa da, distaldeki sinir segmentinin uç an sütüre edileceği sağlıklı ve motor-duyu komponentlerine uygun sinir uzanabileceği akmlıkta bulunamayabilir. Bu durumda araya rejenere olan aksonları taşıyacak bir greft lullanmak gerekecektir. Ağırlıkları 250-350 g. arasında değişen 30 erkek Sprague-Dawley türü sıçan ile her )iri 10 sıçan olacak şekilde 3 grup oluşturuldu. Birinci grupta peroneal sinir, siyatik sinir ;övdesi ile birleştiği yerin 1 cm distalinden kesilip, proksimal uç adduktor kasa tespit :dildikten sonra distalde kalan kısımda 2 cm lik sinir grefti olacak şekilde sinir segmenti ;esildi. Oluşturulan sinir greftinin distal ucu kesildiği yere 4 adet 10/0 naylon sütür ile uç- uca likildi. Sinir greftinin proksimal ucu ise, medialdeki tibial sinir gövdesine, 1 mm lik epinöral >encere çıkartıldıktan sonra, 4 adet 10/0 naylon sütür ile uç-yan epinöral tarzda dikildi. İkinci jrupta, oluşturulan aynı defekte greft olarak juguler venden alınan 2 cm lik ven grefti adlanıldı. Üçüncü grupta ise, ven greftinin içerisinden, adduktor kastan alınan ince bir kas ;libi geçirilip ven+kas grefti oluşturulduktan sonra dikildi. Fonksiyonel değerlendirme için 2, 4, 8, 12, 20 ve 28. haftalarda yürüme yolu analizi jygulandı. Morfolojik değerlendirme için en son yapılan yürüme yolu testinden sonra peronel sinirdeki distal anostomoz alanlarının 5 mm proksimalinden ve 5 mm distalinden, yarısı greft sarısı peroneal sinirin distal segmenti olacak şekilde eksize edildi. Elde edilen segmentlerde »inir lif sayısı ve ortalama miyelin çapı hesaplandı. Alman histolojik preperatlar elektron nikroskopi ile incelendi. Yürüme yolu analizinde gruplar arasındaki farklar anlamlı olarak saptandı ( p<0.05). Distal peroneal sinirlerin transvers kesitlerinden hesaplanan akson sayımlan carşılaştınldığında, 1 - 3 ve 2 - 3 gruplar arasındaki farklar anlamlı olarak saptanırken [p<0.05), 1-2. gruplar arasındaki fark anlamsızdı (p> 0.05). Distal peroneal sinirlerin ıransvers kesitlerinden hesaplanan ortalama akson çaplan karşılaştınldığmda, tüm gruplar arasındaki farklar anlamlı olarak bulundu(p<0.05). Uç-yan nörorafilerde, araya greft kullanılarak olumlu sonuçlann alınabileceği ve diğer yandan, greft seçenekleri arasında, klasik olarak sinir grefti uygulanabilirse de, çalışmamızda ven greftinin, sinir greftine iyi bir alternatif olduğu gösterilmiştir. Uç-uca onanmlarda oldukça iyi sonuçlar verdiği iddia edilen ven+kas greftleme yöntemi uç-yan onanmlarda hayal kınklığı yaratmıştır 38

VIII. SUMMARY HE COMPARISON OF FUNCTIONAL RESULTS OF NERVE GRAFT WITH VEIN AND VEIN+MUSCLE GRAFTS AT THE END-TO-SIDE NEURORAPHY Although it's reported that, end-to-side neuroraphy is an alternative method in the ituation, which the proximal part of the nerve cannot be found, the intact and competent to ıotor-sensory components nerve might not be close enough to perform end-to-side euroraphy. In that case, it will be needed to use a graft for transporting the regenerating xons. In this study, we try to find whether it's possible to use a graft in an end-to-side euroraphy or not, and compare the nerve graft with possible alternative grafts, vein and ein+muscle grafts. Thirty male Sprague Dawley rats were used, with average weight of 293 g (250 - 50 grams). All experiments were made on the right side. Two cm nerve graft, beginning 1 m distal to the branching level was sectioned from the peroneal nerve. A 1 mm epineural /indow was opened in the tibial nerve. In the first group, the proksimal side of this graft was utured to the tibial nerve side as an end-to-side fashion and the distal side was sutured to the istal peroneal nerve stump as an end-to-end fashion. In the second group, the right 2 cm jgular vein was harvested, and used to bridge defect instead of nerve graft used in the first joup. In the third group, 2 cm jugular vein filled with fresh skeletal muscle was used to »ridge the defect. At 2, 4, 8, 12 and 28th weeks, functional assessment of nerve regeneration was performed using walking tract analysis. The number of miyelinated fibers and fiber diameter vere measured and an electron microscopic evaluation was also carried out. Due to walking tract analysis and the fiber diameters, the differences of all three ;roups were statistically significant (p< 0.05). While the differences of miyelinated fibers >etween first and second group were not significant, the differences between the rest (1. ^roup-3. group and 2. group-3. group) were significant p<0.05). The study showed that, the usage of graft in the end-to- side neuroraphy is possible, ind while the classic nerve graft may be used, the vein graft is also a good alternative to the ıerve graft. The vein+muscle graft reported with good results in the end-to-end neuroraphy, :aused to disappointment. 39
