Lomber disk hernisi cerrahisinde önemli olabilecek yeni bir test: Kramp bulgusu

Abstract

VI - ÖZET Günümüzde yeni görüntüleme tekniklerinin gelişmesiyle birlikte lomber disk hastalığında gerçek cerrahi endikasyon sınırlanda genişlemiştir. Diagnostik açıdan fizik muayene bulgularının ve cerrahi endikasyon sınırlarının net olması gerekir. Bu prospektif çalışmada; Dr. M. Nusret Demircan tarafından 1987 den beri kullanılmakta olan, geçerli ve önemli tanısal yeri olduğu deneyimi olan `Kramp bulgusu` önemini daha gerçekçi yöntemle ortaya koymayı amaçladık. Lomber disk hastalıklarında; cerrahi sonrasında da bu yeni bulgu araştırıldı. `Kramp bulgusu` testi hasta prone pozisyonda iken yapılır. Hastanın kuvvetli diz ekstensiyonuna karşı, muayene eden doktor bir eliyle hastanın ayağım tutarak karşı yönde kuvvet uygular. Eğer muayeneyi yapan doktor uyluk ve baldır üzerinde elinin altında kramp veya gerginlik hissederse test pozitiftir. Aynı anda hastada bunu `bacağımda ya da baldırımda kasılma oldu veya ağrı girdi` şeklinde yakınmayla belirtebilir. Çalışma Mayıs 1999 - Temmuz 2000 yıllan arasında yapıldı. Çalışmada kramp bulgusundan başka; klasik disk hastalığı muayenesinde mevcut olan mekanik ve nörolojik bulgular, MRG tetkikleri ve laboratuar bulguları da preoperatif ve postoperatif dönemlerde kontrol edildi. Testin pozitifliği postoperatif dönem olarak; erken dönem (l.gün), l.ay, 3. ay, 6.ay ve 12.ay da yapıldı. Sunmuş olduğumuz bu yeni bulgu; DBK ( düz bacak kaldırma ) testi ve kontrlateral DBK testi kadar değerli gözüktü ve radyolojik incelemelerle de arasında korelasyon saptandı. 35

VII - YABANCI DİLDE ÖZET SUMMARY A Potentially Important Test For Lumbar Disc Herniation Surgery: Cramp Finding Today, with the development of new imaging techniques, the limits of relative indications in lumbar disc surgery are widened. The findings of physical examination for the diagnosis and the surgical indication criteria are needed to be clearer. In this prospective study, the validity and the importance of a new finding (cramp finding) in the person in prone position. Against to the forceful knee flexion, the exeminer holds the leg with one hand and applies a force to overcome the knee flexion. The finding is positive if the exemined person feels a disturbing cramp in the leg or thigh. The study was performed between May 1 999 - July 2000. Besides the cramp finding, the classical disc herniation examination including mechanical and neurological finding, MRI imaging tests, laboratory findings were checked in the pre-postoperatively. The positive cramp finding in operated group was 64 % (n:97) preoperatively and straight leg raising (SLR) test was positive in all. It was positive in 74%, 49%, 31%, 12%, and 5% of patients postoperatively in the postoperative first day, l^^^ö^and 12th months respectively. The presented finding seemed valuable as much as the SLR and especially contrlateral SLR tests in lumbar disc surgery. There are positive correlations with radiological studies. Cramp finding is also important in outcome evaluation. 36
