dc.description.abstractGülhane Askeri Tıp Akademisi Haydarpa?a Eğitim Hastanesi Ortopedi ve Travmatoloji servisinde Mart 2003-Ocak 2007 tarihleri arasında 19 hastaya tibia plato kırığı tanısıyla cerrahi tedavi uygulanmı?tır. Kontrollere gelen 17 hasta değerlendirmeye alınmı?tır. Ortalama ya? 45.11 (18-77), ortalama takip süresi 114.2 (18-213) haftadır. Erkek kadın oranı %78?e %22?dir. Lateral tibia plato kırıkları %65 oranla mediale göre daha fazla görüldü. Tibia plato kırıkları %53 oranla solda daha fazla görüldü. En yaygın görülen yaralanma mekanizması %41?lik oranla motorlu araç kazaları idi. Schatzker sınıflamasına göre 4 vaka tip 1, 7 vaka tip 2, 5 vaka tip 4, 1 vaka tip 6 kırığı idi. Schatzker tip 3 ve tip 5 kırığa rastlanmadı. Cerrahi sonrası klinik sonuçlar Rasmussen Klinik Değerlendirme Kriterleri?ne göre değerlendirildi. Buna göre ortalama skor 26 (17-30) iken, 9 (%53) hasta mükemmel, 7 (%41) hasta iyi, 1 (%6) hasta orta olarak değerlendirildi. Kötü sonuca rastlanmadı. Klinik olarak %94 ba?arı elde edildi. Radyolojik değerlendirme için Resnic ve Niwayama kriterleri kullanıldı. Buna göre 1 (%6) hasta mükemmel, 6 (%35) hasta iyi, 4 (%24) hasta orta, 6 (%35) hasta kötü sonuç olarak değerlendirildi. Radyolojik olarak %41.1 ba?arılı sonuç elde edildi. Çalı?ma sonucunda, tibia plato kırıklarının tedavisinde cerrahi ile anatomik eklem redüksiyonu, uygun internal tespit ve mümkün olan en kısa sürede eklem hareketi sağlanmasının klinik sonuçları olumlu etkilediği görüldü. dc.description.abstractBetween March 2003-January 2007, in the Departrment of Orthopaedics and Traumatology of Gulhane Military Medical Academy Haydarpasa Training Hospital, 19 patients with tibial plateau fractures were treated surgically. Two patients were excluded because of insufficient follow-up, leaving 17 patients (13 men and 4 women) for inclusion in the study. Average patient age was 45.11 years (18-77). 9 (53%) fractures were located on the left side and 8 (47%) were on the right side. 11 fractures were located on the lateral tibial plateau, 5 were on the medial tibial plateau and 1was bicondylar. Average follow-up was 114.2 weeks (18-213). Fractures were classified according to Schatzker as types I-VI. There were 4 (23%) type I, 7 (41%) type II, 5 (30%) type IV, 1 (6%) type VI fractures. The most common mechanism of injury was motor vehicle accident (41%). According to Rasmussen functional score, average score was 26 (17-39). 9 (53%) cases had an excellent result, 7 (41%) cases had a good result, 1 (6%) case had a fair result. There was not any poor result and this study revealed that 94% of the cases had an acceptable knee function. According to Resnic and Niwayama radiological score 1 (6%) case had an excellent result, 6 (35%) cases had a good result, 4 (24%) cases had a fair result and 6 (35%) cases had a poor result. This study revealed that acceptable clinical results are acquired by surgical treatment of tibial plateau fractures by anatomical articular reduction, rigid internal fixation and followed by early range of motion as soon as possible.
