Alt ekstremite variköz venlerinde endovenöz lazer ablasyon tedavisi

Abstract

Alt ekstremite variköz venlerinde endovenöz lazer ablasyon tedavisi ile ilgili ilk deneyimlerimizi içeren bu çalışma GATA Haydarpaşa Eğitim Hastanesi Radyodiagnostik servisinde yapılmıştır.Bu çalışmadaki amacımız varis tedavisinde güncel yaklaşımda cerrahi tedaviye alternatif olarak kullanımı gittikçe artan endovenöz lazer ablasyon tedavisinin kısa ve orta peryodda tedavi etkinliğinin araştırılması ve kliniğimizde bu tedavinin güncel pratik uygulamaya geçirilmesidir.Safen ven yetmezliğine bağlı varis şikayetleri bulunan 15 olgu, alt ekstremite venöz yetmezliğinde lazer ablasyon tedavisi için çalışma grubumuzu oluşturdu. İşlem öncesi dupleks ultrasonografi ile detaylı venöz haritalama yapıldı. Tüm olgulara tümesant anestezi uygulandı. İşlem sonrası tüm olgulara elastik bandaj ve class ?? varis çorabı giydirildi. Varis çorabı uygulaması enaz üç ay önerildi. Olgular işlem sonrası üç-altı ve onikinci ay olmak üzere dupleks ultrasonografi kontrolüne çağrıldı. Geniş bir büyük safen vene sahip perforan ven yetmezliği bulunan bir olguda üçüncü ayda parsiyel rekanalizasyon saptandı. Majör komplikasyon gelişmedi. Ciltte morarma ve ven trasesinde blister oluşumu gibi minör komplikasyonlar görülmüş olup takip ve günlük pansuman ile iyileşme sağlandı.Endovenöz lazer ablasyon tedavisi, güncel yaklaşımda cerrahi tedaviye alternatif olmanın yanında kullanımı giderek yaygınlaşarak çoğu olguda cerrahi tedavinin yerini almaya başlamıştır. Daha az komplikasyon ve rekürrens oranı, daha kısa işlem süresi, genel anestezi gereksiniminin olmaması, toplamda daha az maliyet ve yüksek hasta memnuniyeti işleme ait belirgin avantajlardır. Tüm işlem esnasında ultrasonografi kullanıldığından, işlem için vasküler ultrasonografiyi iyi bilen radyologlara gereksinim vardır.Sonuç olarak alt ekstremite variköz venlerinde lazer ablasyon tedavisi etkili, güvenli ve maliyet-etkin bir yöntemdir. Our study about endovenous laser ablation treatment in lower extremity venous insufficiency was done at radiodiagnostic department of GATA Haydarpasa Teaching Hospital.The aim of this study is to investigate the efficiency of the endovenous laser ablation treatment that has recently become popular alternative to surgery, in short and long term period. Also we aimed to use this treatment in daily practise in our department.A total of 15 patients with varices secondary to greater and small saphenous vein insufficiency were our study group in endovenous laser ablation treatment in lower extremity venous insufficiency. Prior to ablation, all patients were examined with color Doppler ultrasnography and detailed venous mapping was made. Tumescent anesthesia was performed in all case before ablation. After ablation, all patients were mobilized with elastic bandage wrapped around their leg and class ?? compression stocking. We recommended using compression stocking three months at least. The patients were recalled for color Doppler examination at 3 rd, 6 th and 12 th month following the ablation day. We observed recanalization in a patient who had a insufficient greater saphenous vein with diameter of 15 mm at 3 rd month. He had additionally a incompetent perforating vein at the level of the proximal cruris. No major complication developed. Minor complications like ecchymosis and blisters along the ablation trace were seen and cure was achieved by daily care and follow-up.In daily practice, endovenous laser treatment is an alternetive treatment choise in lower extremity venous insufficiency and varices. Even it has generally in higher proportion than surgery in many department of cardiovascular medicine. Advantages related to ablation are; fewer complications and recurrences, smaller procedure duration, no need for general anesthesia, cost-effectiveness in total price, and significant patient comfort. Because of real-time monitorizing during the procedure, experienced radiologists in color Doppler ultrasonography are needed.In conclusion, laser ablation treatment in lower extremity venous insufficiency and varices is an effective, safe and cost-effective treatment choise. In near future, it is expected that, surgical treatment in varices related to the venous insufficiency will be replaced by endovenosus laser ablation treatment.Keywords: Venous insufficiency, Varix, Laser ablation treatment
