dc.description.abstractAmaç: Musküler distrofili (MD) hastalarda 25(OH)D düzeyi ve D vitamintakviyesinin fiziksel performansa (FP), kemik mineral yoğunluğuna (KMY) ve kırıkgelişimine etkisini araştırmaktı.Materyal ve Metod: Nöromuskuler Hastalık Merkezi'nde izlenen, farklı MDtiplerindeki hastalar retrospektif olarak değerlendirmeye alındı. Hastalar vitamin D düzeyi(≤20ng/ml, >20 ng/ml), ambulasyon durumu ve yaşa (≤18, >18yaş) göre 8 gruba ayrıldı ve25(OH)D düzeyine göre, kolekalsiferol ve kalsiyum karbonat verilmiş olan hastaların Dvitamin tedavi öncesindeki ve 6 ay sonraki verileri retrospektif olarak değerlendirildi. Sonuçölçütleri; kırık varlığı, FP testleri (kavrama ve pinch kuvveti, 30 sn ve 5 kez otur kalk,merdiven inme/çıkma, 10 m ve 6 dk. yürüme, Vignos, Brooke), serum Ca, P, ALP, 25(OH)Dvitamin düzeyi ve KMY olarak belirlendi.Bulgular: Çalışmaya katılan 152 hastanın %52'si ≤18 yaş, %48'i >18 yaş grubunda,%58'i ambulatuvar, %42'si nonambulatuvar düzeydeydi. Olguların %39'u Duchenne MD,%7'si Becker MD, %11'i Limb Girdle MD, %5'i fasiyoskapulohumeral MD, %22'simiyotonik MD, %15'i MD, %3'ü kongenital MD ve %1'i kongenital miyotoni tanıgrubundaydı. Başlangıçta, >18 yaş grubundaki nonambulatuvar hastalarda, serum 25(OH)Ddüzeyi ≤20 ng/dl olanlarda, >20 ng/dl olan hastalara göre kavrama ve pinch kuvveti dahadüşük bulundu. Diğer FP testleri, kırık sıklığı ve KMY yönünden gruplar arasında fark yoktu.D vitamin takviyesi sonrası 6.aydaki kontrolde gruplar arasında kırık, FP ve KMY yönündenfarklılık saptanmadı. Serum 25(OH)D>20ng/ml olan, ≤18 yaş, nonambulatuvar grupta,≤20ng/ml olan gruba göre toplam kas puanındaki azalma, daha düşüktü.Sonuç: Bu çalışma sonucunda 25(OH)D ≤20ng/ml düzeyinin, nonambulatuvar, >18yaş MD olgularında kavrama ve pinch kuvvetinde farklılığa neden olduğu; D vitamintakviyesinin FP, kırık ve KMY üzerine etkili olmadığı, ancak ≤18 yaş, nonambulatuvar gruptakas kuvvetindeki azalmaya etkili olduğu belirlendi.Anahtar Kelimeler: Muskuler distrofi, vitamin D, fiziksel performans, kemik mineralyoğunluğu
dc.description.abstractObjective: The aim of this study is to investigate the effect of 25(OH)D and Dvitamin supplementation on physical performance (PP), bone mineral density (BMD) andfracture development in patients with muscular dystrophy (MD).Material and Methods: Patients in different MD types followed at theNeuromuscular Disease Center were retrospectively evaluated. Patients were divided intoeight groups according to vitamin D levels (≤20 ng/ml,>20ng/ml), ambulatory status and age(young group ≤18, adult group >18 years) and cholecalciferol and calcium carbonate weregiven according to 25(OH)D levels. Patients were evaluated before vitamin D treatment and 6months later. Outcome measures were fracture development, FP (grip and pinch strength, situp in 30 sec and 5 times, step up and down, 10 m and 6 min walk, Vignos, Brooke), serumCa, P, ALP, 25(OH)D levels and BMD.Results: Of the 152 patients who participated in the study, 52% were ≤18 years, 48%were >18, 58% were ambulatory, and 42% were nonambulatory. 39% of the patients wereDuchenne MD, 7% were Becker MD, 11% were Limb Girdle MD, 5% werefacioscapulohumeral MD, 22% were myotonic MD, 15% were MD, 3% congenital MD and1% were congenital myotonia. At baseline, PP, fracture frequency and BMD did not differbetween the groups, but grip and pinch strength were lower in nonambulatory adult groupwho had ≤20ng/dl 25(OH)D level, than > 20ng/dl 25(OH)D level. PP, fracture frequency, andBMD level between the groups were not different after vitamin D supplementation excepttotal muscle score. The decrement of total muscle score was lower in the nonambulatoryyoung group who had 25(OH)D >20ng/ml than who had 25(OH)D ≤20 ng/ml.Conclusion: As a result of this study, it was found that 25(OH)D levels of ≤20 ng / mlresulted in differences in grip and pinch strength in nonambulatory, >18 age group. Vitamin Dsupplementation is not effective on PP, fracture and BMD, however the decrement of totalmuscle strength was lower in nonambulatory young age group who had >20ng/ml vitamin Dlevel.Key words: Muscular dystrophy, vitamin D, physical performance, bone mineral densityen_US
