dc.description.abstractAksilla negatif meme kanserleri son yıllarda artmaktadır. Adjuvan tedavinin belirlenmesinde diğer prognostik faktörlerin bilinmesi önemlidir. Bu çalışmada aksilla negetif meme kanserlerinde 5 yıllık sağkalım ve uzak metastaz gelişimine etkili prognostik faktörler araştırıldı.1993-1995 yılları arasında Ankara Onkoloji Hastanesinde meme kanseri tanısıyla tedavileri yapılan ve aksilla diseksiyon materyalinde metastaz saptanmayan en az 5 yıl takip edilmiş 150 hasta çalışmaya alındı. Hastanın yaşı, tümör boyutu, grade'i, histolojik subtipi, lenfatik invazyon, vasküler invazyon, nöral invazyon, mononükleer hücre infiltrasyonu, aksiller diseksiyonda çıkartılan lenf bezi sayısı, adjuvan kemoterapi veadjuvan radyoterapinin uzak metastaz gelişimi ve sağkalıma etkisi univariate ve multivariate analizle değerlendirildi.5 yıllık sağkalım %92, 5 yıllık hastalıksız sağkalım %88 olarak bulundu. Univariate analizde nöral invazyon (p=0,0001), adjuvan kemoterapi (p=0,002), adjuvan radyoterapi (p=0,025) grubunda uzak metastaz, adjuvan kemoterapi ve radyoterapi grubunda (p=0,02) ölümlerin anlamlı olarak yüksek olduğu, multivariate analizdeise sadece nöral invazyonun hem uzak metastaz gelişimine (p=0,002) hem de ölüm (p=0,015) üzerine etkili bağımsız prognostik parametre olduğu saptandı.Lenf bezi metastazı olmayan meme kanserlerinde nöral invazyon sağkalımı etkileyen tek parametre olduğundan adjuvan tedavinin planlanmasında dikkate alınmalıdır
dc.description.abstractNumber of cases of breast cancer with negative axillary lymph nodes has increased in last years. It is important to define other prognostic factors for the decision of adjuvant treatment. In the study, 5 year survival rate and prognostic factors which are related with distant metastasis were evaluated. Between 1993 and 1995 years in 150 patients with breast cancer with negative axillary lymph node involment who was treated in Ankara Oncology Hospital were studied. They have been followed at least 5 years. The effect of patients' age, tumor size, grade, histologic subtype, lymphatic invasion, vascular invasion, neural invasion, mononuclear cell infiltration, number of lymph nodes in axxilary dissection material, adjuvant chemotherapy and adjuvant radiotherapy on distant metastasis and survival were evaluated by univariate and multivariate analysis. 5 year survival rate was 92%, 5 year disease free survival rate was 88%. In univariate analysis rate of distant metastasis was high in patients with neural invasion (p=0,0001), in adjuvant chemotherapy (p=0,002) and in adjuvant radiotherapy (p=0,025) groups. Death rate was high in adjuvant chemotherapy and radiotherapy (p=0,02) groups. In multivariate analysis only neural invasion was found to be an independent prognostic parameter which was related with rate of distant metastasis (p=0,002) and death rate (p=0,015). In breast cancer without axillary lymph node metastasis, neural invasion is the only parameter that effect survival, for this reason neural invasion should be taken into account in planning adjuvant treatment.en_US
