Foramen infraorbitale'nin anatomik ve morfometrik özellikleri

Abstract

Maxillofacial ve plastik cerrahi uygulamalarında lokal anestezi başarısı nervus infraorbitalis ve aksesuar nervus infraorbitalis lokalizasyonunun doğru bir şekilde saptanmasına bağlıdır. Bu amaçla çalışmamızda nervus infraorbitalis'in çıktığı foramen infraorbitale ve aksesuar nervus infraorbitalis'in çıktığı aksesuar foramen infraorbitale'nin anatomik ve morfometrik analizleri planlandı.Foramen infraorbitale ve aksesuar foramen infraorbitale'nin anatomik ve morfometrik özelliklerinin belirlemek için İstanbul Üniversitesi İstanbul Tıp Fakültesi Anatomi Anabilim Dalı ve İstanbul Üniversitesi Diş Hekimliği Fakültesi Anatomi Bilim Dalı'na ait toplam 112 adet kuru kafatası kullanıldı.Foramen infraorbitale'nin inspeksiyonel görünümü ve orta hatta göre doğrultusu, aksesuar foramen infraorbitale görülme sıklığı ve aynı taraf foramen infraorbitale'ye göre doğrultusu belirlendi. Her bir foramen infraorbitale ve aksesuar foramen infraorbitale'nin belirlenen noktalara olan en kısa mesafeleri ölçüldü.Çalışmanın sonucunda foramen infraorbitale'lerin %64.7'sinin oval, %27'sinin daire, %8.3'ünün yarımdaire; %90'ının orta hatta göre oblik, %6'sının vertikal, %4'ünün ise horizontal yerleşimli olduğu belirlendi.Foramen infraorbitale'nin en uzun çapı sağda 4.10+/-0.87 mm, solda 4.33+/-0.94 mm; en kısa çapı sağda 3.03+/-0.59 mm, solda 3.19+/-0.64 mm; orta hatta uzaklığı sağda 28.27+/-2.38 mm, solda 28.67+/-2.59 mm; foramen supraorbitale/incisura supraorbitalis'e uzaklığı sağda 43.25+/-3.37 mm, solda 43.61+/-3.42 mm; sutura nasomaxillaris alt ucuna uzaklığı sağda 26.73+/-3.15 mm, solda 26.20+/-2.93 mm; kanin diş juga alveolaria'sı alt ucuna uzaklığı sağda 31.09+/-2.92 mm, solda 32.16+/-3.19 mm; margo infraorbitalis'e uzaklığı sağda 6.98+/-1.68 mm, solda 6.98+/-1.75 mm; sağ ve sol foramen infraorbitale'ler arası uzaklık 56.85+/-3.89 mm olarak ölçüldü.Aksesuar foramen infraorbitale görülme sıklığı %7; saptanan aksesuar foramen infraorbitale'lerin %66.5'inin solda, %33.5'inin sağda; %46.7'sinin oval, %53.3'ünün daire şeklinde, %93.3'ünün foramen infraorbitale'nin superomedialinde, %6.7'sinin ise foramen infraorbitale'nin inferomedialinde olduğu belirlendi.Aksesuar foramen infraorbitale'nin en uzun çapı 1.93+/-0.67 mm, en kısa çapı 1.56+/-0.45 mm, orta hatta uzaklığı 22.18+/-2.56 mm, foramen supraorbitale/incisura supraorbitalis'e uzaklığı 39.92+/-2.86 mm, sutura nasomaxillaris alt ucuna uzaklığı 21.95+/-6.59 mm, kanin diş juga alveolaria'sı alt ucuna uzaklığı 34.45+/-1.73 mm, margo infraorbitalis'e uzaklığı 5.61+/-1.82 mm, karşı taraf foramen infraorbitale'ye uzaklığı 51.18+/-2.93 mm ve aynı taraf foramen infraorbitale'ye uzaklığı 3.95+/-1.60 mm olarak ölçüldü.Kafataslarının sağ ve sol taraflarına ait ölçüm sonuçlarının ortalamaları SPSS versiyon 10.0 istatistik programı kullanılarak student-t testi ile karşılaştırıldı ve taraflar arasında istatistiksel olarak anlamlı fark olmadığı görüldü.Değerlendirilen parametrelerden klinikte en kullanışlı olanların erişim kolaylığı açısından foramen infraorbitale'nin margo infraorbitalis'e, kanin diş juga alveolaria'sı alt ucuna ve orta hatta olan uzaklıkları olabileceğini düşünmekteyiz.Anahtar Kelimeler: Anatomi, Morfometri, Maxilla, Foramen Infraorbitale

Success of local anaesthesia in maxillofacial and plastic surgery depends on the true determination of the localization of the infraorbital and accessory infraorbital nerve. To this end, the anatomical and morphometric analyses of the infraorbital foramen which gives rise to the infraorbital nerve and the accessory infraorbital foramen which gives rise to the accessory infraorbital nerve, were worked out.112 dry skull obtained from the anatomy departments of dental school and medical school of Istanbul University were used to determine the anatomic and morphometric features of the infraorbital foramen and accessory infraorbital foramen.The aspect and the direction of the infraorbital foramen, incidence of the accessory infraorbital foramen and the direction of accessory infraorbital foramen with regard to the infraorbital foramen, the shortest distances to the designated points from each of the infraorbital and accessory infraorbital foramina were measured.As a conclusion of the study, the infraorbital foramen is found to be located ovally in 64.7%, round in 27%, semicircle in 8.3%; while oblique in 90%, vertical in 6%, horizontal in 4%, in relation to midline.The longest diameter of the infraorbital foramen was 4.10+/-0.87 mm at the right side, 4.33+/-0.94 mm at the left side; the shortest diameter was 3.03+/-0.59 mm at the right side, 3.19+/-0.64 mm at the left side; the distance between the infraorbital foramen and the midline was 28.27+/-2.38 mm at the right side and 28.67+/-2.59 mm at the left side; the distance between the infraorbital foramen and the supraorbital notch/foramen was 43.25+/-3.37 mm at the right side, 43.61+/-3.42 mm at the left side; the distance between the infraorbital foramen and the inferior point of nasomaxillary suture was 26.73+/-3.15 mm at the right side and 26.20+/-2.93 mm at the left side; the distance between the infraorbital foramen and the alveolar yokes of canine tooth was 31.09+/-2.92 at the right side, 32.16+/-3.19 ? mm at the left side; the distance between right infraorbital foramen and infraorbital margin was 6.98+/-1.68 mm and 6.98+/-1.75 mm at the left side; the distance between the right and left infraorbital foramina was 56.85+/-3.89 mm.Incidence of the accessory infraorbital foramina was found to be 7%, located on the left side in 66.5% and on the right side in 33.5%, oval in 46.7%, round in 53.3 %, superomedially to the infraorbital foramen in 93.3%, inferomedially to the infraorbital foramen in 6.7%.The longest diameter of the accessory infraorbital foramen was 1.93+/-0.67 mm, the shortest diameter was 1.56+/-0.45 mm; while the distance between the accessory infraorbital foramen and the midline, supraorbital notch/foramen, inferior point of nasomaxillary suture, inferior border of alveolar yokes of canine tooth, infraorbital margin, contralateral infraorbital foramen and ipsilateral infraorbital foramen was 22.18+/-2.56 mm, 39.92+/-2.86 mm, 21.95+/-6.59 mm, 34.45+/-1.73 mm, 5.61+/-1.82 mm, 51.18+/-2.93 mm and 3.95+/-1.60 mm, respectively.The average values of left and right sides of the skulls were analyzed with the student-t test SPPS version 10.0 and no significant difference was found statistically.The most practical measurement parameters in clinics may be the infraorbital foramen, the infraorbital margin, inferior border of alveolar yokes of canine tooth, midline with respect to access facility.Keywords: Anatomy, Morphometry, Maxilla, Infraorbital Foramen
