Yağ dekompresyonunun tiroid orbitopati' de hertel değeri ve hasta yakınmalarına etkisi

Abstract

Graves hastalığı toplumda %0,5 oranında görülen ve hastaların %40'ında ortaya çıkan tiroid orbitopati bulguları ile hastaları tıbbi ve sosyal olarak oldukça etkileyen bir hastalıktır. Orbital alanda meydana gelen basınç artışı ile glob öne doğru hareket etmekte ve eksoftalmi oluşmaktadır. Artan orbita içi basınç optik sinirde sıkışma ile birlikte görmeyi tehdit eder duruma gelebilir. Duyu organlarından gözün fonksiyonunu bozacak kadar önemli olan bu hastalığın medikal ve cerrahi tedavisi mevcuttur. Cerrahi tedavide orbital dekompresyon, şaşılık cerrahisi ve gözkapağı girişimleri bulunmaktadır. Orbital dekompresyon kemik ve yağ dekompresyonu olarak ikiye ayrılabilir. Kemik dekompresyonu ile kıyaslandığında daha az agresif ve komplikasyon oranı daha az olan yağ dekompresyonu son dönemlerde uygulanan yöntem halini almıştır. Bu amaçla İ.Ü. İstanbul Tıp Fakültesi Plastik, rekonstrüktif ve Estetik Cerrahi Anabilim Dalı' nda Temmuz 2002 ile Mayıs 2004 tarihleri arasında Orbital Yağ Dekompresyonu yapılmış olan yedi Graves hastasının dosyaları retrospektif olarak incelenmiştir. Belirtilen tarihler arasında opere edilen hastaların eksize edilen yağ miktarları ile Hertel Değerleri ( propitozis derecesi ) arasında anlamlı bir iyileşme bulunmuştur.Anahtar kelimeler: Graves, Propitozis, Orbital Yağ Dekompresyonu

The incidence of Graves Diseaseis %0,5. %40 of all effected patients have Tyroid Orbithopaty Symptoms and these can effect all the patients medically and socially. The globe movesforward by increasing orbital pressure therefore exophtalmia occurs. Increased intraorbital pressure can compress optic nerve then vision can be threated by this situation. There are medical and surgical treatment of this disease that could be harmfull fort he eye which is one of the senseational organ. Surgical treatment consists orbital decompression, strabismus surgery and eyelid procedures. Orbital decompression can be divided into two, bony decompression and fat decompression. The fat decompression has been started to be the most common used technique when its compared to bony decompressin which has more complication. Fort his aim we have examined retrospectively the charts of seven patients who have been operated in Istanbul University, Istanbul Faculty of Medicine, Dept. of Plastic, Reconstructive and Aesthetic Surg. Between July 2002 and May 2004.Considerable healing has been found when excised fat tissue amount correlated with hertel mesaurement.Keywords: Graves, Prpitosis, Orbital Fat Decompression
