Toksoplazma IgG ve IgM pozitif gebelerde IgG avidite sonuçlarının değerlendirilmesi

Abstract

Bu çalışmada, Temmuz 2009-2010 döneminde Yüzüncü Yıl Üniversitesi Tıp Fakültesi Mikrobiyoloji Laboratuvarı'na Toksoplasmozis şüphesi ile gönderilen toplam 104 kan örneği alınarak makroELISA yöntemi ile Architect i2000 cihazında Toksoplazma IgG ve IgM testleri çalışılmış pozitiflik saptanan 83 hastada avidite testi yapılmış ve toksoplazmozis prevalansı ile gebelerin klinik durumu araştırılmıştır. Araştırılan 83 hastadan 76'sında IgG pozitifliği ile beraber avidite değeri negatif tespit edilmiş olup akut toksoplazmoz açısından risk taşımadıkları düşünülerek hastalara herhangi bir üst tetkik önerilmemiştir. Sonuçları değerlendirilen hastalardan 4'ünde IgM ve IgG pozitifliği ile beraber avidite değeri pozitif olarak saptanmış ve bunlardan 3'ünde yapılan tekrar sonucu avidite değerlerinin negatif olduğu görülmüş ve akut toksoplazmozdan uzaklaşılarak yine bir üst tetkik önerilmemiştir. Ancak 1 gebede seroloji ile beraber avidite pozitifliğinin sürmesi neticesinde ileri tetkikler ve muayene bulguları sonucunda gebeliği sonlandırılmıştır. Son olarak 3 hastada ise avidite sonucu şüpheli bulunarak ileri tetkikler ve muayene bulguları sonucunda bir risk saptanmamış ve yakından takip edilen gebeler sağlıklı doğumlar yapmışlardır.1 yıllık dönemde laboratuarımıza gönderilen toplam 104 gebeye ait kan örneğinde yapılan serolojik çalışmalar sonucunda toxoplasma IgG seropozitifliğinin %80 olduğu görülmüş ve hastalığın bölgemiz için yüksek endemik olduğu sonucuna varılmıştır. Bununla beraber toksoplazma infeksiyonu sonucunda IgM pozitifliğinin ve düşük avidite değerlerinin uzun süre devam edebilmesi gebelik döneminde akut toksoplazmozun ekarte edilebilmesini güçleştirdiği için serolojik olarak IgM ve IgG ile beraber aviditenin değerlendirilmesi doğru tanı açısından önemlidir.Anahtar sözcükler: Toxoplasma gondii, toksoplazmozis, avidite, seroloji.

In this study, a total of 104 blood specimens sent to Yüzüncü Yıl University, Faculty of Medicine, Microbiology laboratory between July 2009 and 2010 with suspicion of toxoplasmosis were obtained and 83 serologically positive specimens for toxoplasma IgM and IgG were tested for avidity with Architect i2000 instrument, macroELISA assay and toxoplasmosis prevalence and clinical conditions of these pregnants were investigated. From the total 83 patients, in 76, toxoplasma IgG were positive with high avidity results which indicates that these patients are not at risk for acute toxoplasmosis and no additional suggestions were made. In 4 patients both IgM and IgG were detected positive for toxoplasmosis with low avidity values indicating acute or recently passed toxoplasmosis. In 3 of these patients the tests were repeated and avidity values were detected to increase and IgM to be negative so acute toxoplasmosis was eliminated however in 1 patient among persistance of serological positivity and low avidity, advanced tests and physical examinations were performed and finally the pregnancy was terminated. In 3 patients avidity results were indecisive so advanced tests and physical examinations were done and the pregnants were closely followed up and at the end they delivered healthy babies.As a result of toxoplasma serological studies conducted on a total of 104 blood specimens from pregnants in one year period, toxoplasma IgG seropositivity was detected to be 80% which indicates that toxoplasmosis is highly endemic in our region. Additionally it was concluded that evaluation of avidity results along with serological IgM and IgG for toxoplasmosis is important to rule out acute toxoplasmosis in pregnancy because of the difficulty caused by the persistence of IgM positivity and low avidity values for many months.Keywords: Toxoplasma gondii, toxoplasmosis, avidity, serology.
