Total kalça protezi ameliyatı geçiren hastalara klinik vaka yönetici hemşire modeli uygulamasının hasta memnuniyeti açısından değerlendirilmesi

Abstract

Total kalça protezi ameliyatı geçirecek olan hastaların hemşirelik süreci yönetimi `Klinik Vaka Yönetici Hemşireliği Modeli` kullanılarak yapılabilir.Araştırma; klinik vaka yönetici hemşire modeli uygulaması ile hemşirelik hizmeti verilen kalça protezi ameliyatı geçiren hastaların memnuniyetlerini belirlemek amacı ile 15 Ekim 2017 - 01 Mart 2018 tarihleri arasında Adana Özel Ortopedia Hastanesi, Ortopedi ve Travmatoloji Servisi-1 ve Ortopedi ve Travmatoloji Servisi-2'de yatan 46'sı deney 46'sı kontrol grubu olmak üzere 92 hasta ile yürütüldü. Deney grubuna `Klinik Vaka Yönetici Hemşireliği Modeli` kullanılarak hemşirelik hizmeti, kontrol grubuna ise rutin klinik uygulamalar doğrultusunda hemşirelik hizmeti verildi. Verilerin toplanmasında kişisel bilgi formu, Hastanın Hemşirelik Bakımını Algılayışı Ölçeği (HHBAÖ) kullanıldı. Veriler yüz yüze görüşme tekniği ile toplandı. Veriler, SPSS 17.0 programında t-testi, ki-kare testi ve tek yönlü varyans analizi testi ile değerlendirildi. Çalışmaya katılan hastaların yaş ortalaması 47,83 ± 10,81 yıl (min.: 25-maks.: 65) olarak tespit edildi. Araştırma kapsamındaki hastaların % 65,2'sini (n=60) kadın, 34,8'ini (n=32) erkek oluşturdu. Hastaların % 31,5'i (n=29) üniversite mezunu, % 30,4'ünü de (n=28) ortaöğretim mezunları oluşturdu. Araştırmaya katılan hastaların % 78,3'ü (n=72) evliydi. Hastaların % 67,4'ü (n=62) ekonomik gelirlerinin giderlerinden az olduğunu ve % 87'si (n=80) daha önce hastanede yattığını belirtti. Araştırmamızda klinik vaka yönetici hemşire modeli uygulaması ile bakım verilen hastaların HHBAÖ'ye göre memnuniyet puanı ortalamasının 73,9 ±3,6 olduğu, rutin klinik uygulamalar doğrultusunda verilen hemşirelik bakımından memnuniyet puanı ortalamasının 64,5 ±11,8 olduğu saptandı. Sonuç olarak total kalça protezi ameliyatı geçiren, klinik vaka yönetici hemşireliği modeli ile bakım verilen hastaların memnuniyet düzeylerinin (73,9), rutin klinik uygulamalar doğrultusunda hemşirelik hizmeti verilen hastaların memnuniyet düzeyinlerinden (64,5) daha yüksek olduğu tespit edildi. Fark istatistiksel olarak anlamlıydı (p=0,001, p<0,05).Anahtar Kelimeler: Hemşirelik; hasta memnuniyeti; total kalça protezi; vaka yönetimi.

The management of nursing process of patients who will undergo total hip replacement surgery can be done by using yönetim Clinical Case Management Nursing Model. The study was carried out between October 15, 2017 and March 01, 2018 in the Adana Orthopedia Hospital for the purpose of determining the satisfaction of the patients about nursing services who underwent hip replacement surgery. The study was carried out with 92 patients, 46 of patients were experimental group who are admitted to the Orthopedics and Traumatology Service-2, 46 of patient were control group who were admitted to the Orthopedics and Traumatology Service-1. Nursing service was given to the experimental group by using the clinical case management nursing model ang the control group was given routine clinical applications nursing model. The personal information form and the Scale of Patient Perception of Hospital Experience With Nursing (PPHEN) were used to collect the data. The data were collected by face to face interview technique. The data were evaluated with t-test, chi-square test and univariate analysis test. The analyzes were performed using the SPSS 17.0 software package (IBM Corp.; Armonk, NY, USA). The mean age of the patients was 47,83 ± 10,81 years (min.:25-maks.:65). 65.2% (n= 60) of the patients were female and 34.8% (n= 32) were male. 31,5% (n = 29) of the patients were university graduates and 30,4% (n = 28) were secondary school graduates. 78,3% (n = 72) of the patients were married. 67,4% (n = 62) of the patients stated that their economic income was less than their expenses and 87% (n = 80) had previously been hospitalized. In our study, it was found that the mean satisfaction score of experimental group patients who were given care by clinical case management model were 73,9 ± 3,6, and the control group patients mean nursing satisfaction score was 64,5 ± 11,8. As a result, it was determined that the satisfaction levels (73.9) of patients who underwent total hip replacement surgery and who were given care by the clinical case management nursing model were higher than the satisfaction level (64.5) of the routine clinical applications nursing model care. The difference was statistically significant (p=0,001, p<0,05).Key Words: Nursing; patient satisfaction; total hip arthroplasty; case management
