Doğal ve cerrahi yolla menopoza girmiş kadınların depresyon düzeylerinin cinsel doyum üzerindeki etkisi

Abstract

Bu çalışma, doğal ve cerrahi yolla menopoza girmiş, doğal menopoz için en az 1 yıldır adet kanaması olmayan ve hormon tedavisi almayan; cerrahi menopoz için en az 3 ay önce bilateral ooferektomi yapılmış kadınların depresyon düzeylerinin cinsel doyum üzerindeki etkisini incelemeyi amaçlamaktadır. Araştırmada, her iki grupta da menopoz sürecinde depresif belirtiler olabileceği ve bu belirtilerin sonucunda cinsel doyumun olumsuz yönde etkileneceği düşünülmüştür. Özellikle cerrrahi menopoz olgularında depresyonun cinsel doyum üzerindeki etkisinin, doğal menopoz olgularına oranla daha şiddetli ve olumsuz olacağı beklenmektedir. Bu çalışmaya 45-65 yaş arasında 40 doğal, 40 cerrahi yolla menopoza giren toplam 80 gönüllü kadın katılımcı dahil edilmiştir. Değerlendirmede; `Sosyo-demografik Bilgi Formu`, `Menopoz Semptomlarını Değerlendirme Ölçeği`, `Belirti Tarama Ölçeği`, `Zung Depresyon Ölçeği`, `Golombock Rust Cinsel Doyum Ölçeği` kullanılmış sonrasında katılımcılara `Rorschach Testi` uygulanmıştır. Yapılan projektif testin sonuçları Fransız Okulu'na özgü içerik analizi yöntemiyle değerlendirilmiştir. Araştırmadan elde edilen veriler SPSS (Sosyal bilimler için İstatiksel Analiz) Windows 21.0 programı kullanılarak analiz edilmiştir. Elde edilen bulgular %95 güven aralığında istatistiksel olarak 0,05 anlamlılık düzeyinde yorumlanmıştır. Sonuçlar incelendiğinde, her iki grupta da cinsel problematiklerin öne çıktığı görülmekle birlikte cerrahi yolla menopoza girmiş kadınlarda kadının bedeni, cinsellik algısı dramatik bir şekilde etkilenmekte ve kadınsılık kavramına atfedilen anlam, kadınsılığın azalması hissi ve cinsel işlevin olumsuz yönde etkileneceği düşüncesi ile beraber işlev yitimi cerrahi yolla menopoza girmiş kadınları depresif açıdan daha çok etkilemiştir. Sağlıklı bir organın çıkarılması düşüncesi olumsuz bakış açısıyla beraber hem menopoz hem de kayıp sürecini daha şiddetli yaşamalarına sebep olmaktadır. Menstürasyon kanına dair söylemlerin, kadınsılık kaybının, depresif duygulanımın, pasif yönelimli cevapların cerrahi menopoz grubunda daha fazla olduğu görülmüştür. Doğal menopoz grubunda da aynı kayıp olmasına rağmen içsel yaşantılarının çok faal olduğu ve düşünce sistemine fazla yatırım yapılmasıyla depresif duygulanımın cerrahi menopoza göre daha az olduğu ve cinsel doyumun depresif durumdan etkilendiği yönünde anlamlı bulgular elde edilmiştir. The aim of this study is to review naturally or surgically menopausal women. The effect of depression over sexual satisfaction is aimed to be examined in naturally menopausal women who has not menstruated at least for 1 year and did not receive any hormone therapy and in surgically menopausal women who had bilateral ooferektomi at least 3 months ago. In this study, the existence of depressive symptoms and a negative effect of those depressive symptoms over sexual satisfaction is hypothesized. It is expected that the effect of depression over sexual satisfaction in natural menopause is negative and more intense. The sample of this study is 40 naturally and 40 surgically menopausal, in total 80 women between the ages of 45 and 65. The instruments for using data collection are `Socio-demographic data form, Golombock Rust Inventory of Sexual Satisfaction (GRISS), Menopause Rating Scale (MRS), Zung Depression Scale and Symptom scale` for quantitative and Rorschach Ink Test for qualitative evaluation. The Projective tests' results were evaluated with specific content analysis method of French School. The data were evaluated by using SPSS for Windows 21.0 package program. The results were interpreted in 95% reliability interval and 0.05 significance level. As a result, even if the sexual problems occur in both groups, it is concluded that women's physical and sexual perception who are surgically menopausal are dramatically effected and besides the sexual dysfunctions, ideas about meaning of femininity, lessening of femininity and negativity in sexual functioning are seen to be effective in a depressive way in surgically menopausal women. The idea of losing a healthy organ as a part of negative point of view is a reason for experiencing the menopause and loss period more severe. Discourses about the blood of menstruation, loss of femininity, depressive affect, passive-oriented responses were found to be higher in surgical menopause sample group. Even if naturally menopausal women experience the same loss, because of intensity in internal processes and thinking process, the results show that depressive symptoms are seen to be less in naturally menopausal women than surgically menopausal women and depressive symptoms are important determinants for sexual satisfaction.
