Hipertiroidili kadınlarda, serum seks hormonu bağlayıcı globulin (SHBG), total testosteron ve dehidroepiandrosteron sülfat (DHEA-SO4) düzeyleri

Abstract

ÖZET Bu çalışmada hipertiroidili kadınlarda serum seks hormonu bağlayan globulin(SHBG),total testosteron ve dehidroepiandrosteron sulfat(DHEA-S04) düzeylerinin ötiroidik kadınlara göre nasıl bir değişkenlik gösterdiğini araştırdık. Çalışmaya hipertiroidili premenopozal ve postmenopozal 21 kadın hasta aldık. Hastaların 12 tanesi diffüz toksik guatr, 9 tanesi multinodular toksik guatrlı idi. Bu hastalarda serum total triodotironin (T3), Total tiroksin (T4) tirotropin (TSH), SHBG, total testosteron, DHEA-SO4 düzeylerine bakıldı.Hasta grubu ile aynı cinste ve yaş ortalamaları arasında anlamlı bir fark olmayan 26 ötiroidik sağlıklı kontrol grubu ile karşılaştırıldı. Hipertiroidili hasta grubunda, ötiroid kontrol grubuna göre ortalama serum SHBG (P<0,001) total testosteron (P<0,05) ve DHEA-SO4(P<0,05) düzeyleri anlamlı olarak yüksek bulundu. Hipertiroidizm durumlarında serum SHBG düzeylerindeki artış muhtemelen tiroid hormonlarının hepatik sentezleri üzerinde doğrudan stimule edici etkisine bağlıdır.Bu stimulasyon SHBG düzeyleri ile SHBG- bağlı testosteron düzeylerini artırmaktadır (39,40,41,42), hipotezinden yola çıkarak hipertiroidili kadınlarda SHBG ve total testosteron düzeyleri yükseldiği söylenebilir. DHEA-SO4 düzeylerinin hipertiroidide yükseldiğine dair bilinen daha önce yapılmış bir çalışma yoktur.Hipertiroidi de birçok hormonun metobolizmasının arttığı.kortizolün seviyesinin devamı için ACTH doruk değerlerinin sayısı arttığı bildirilmektedir 10). Bilinen diğer bir bilgi ACTH un adrenal bezden DHEA-SO4 sekresyonunu artırdığı şeklindedir (18) bizde bu bilgiler ışığında hipertiroidide artmış ACTH doruk değerlerine cevap olarak DHEA-SO4 salınımının ve serum düzeyinin arttığını söyleyebiliriz. Hipertiroidili grupta DHEA-SO4 seviyesinin daha yüksek olmasına katkıda bulunan diğer bir faktörde.bu grubun ortalama yaş ve BMI nin kontrol gurubuna göre daha düşük olması olabilir.. 27

SUMMARY In this study; we search for the differences in serum sex hormone binding globuline (SHBG), total testosterone and dehydroepiandrosterone sulphate (DHEA-SO4) levels between hyperthyroidsm and euthyroidism in women. A total number of 21 premenauposal and postmenauposal women with hyperthyroidism were included in the study. 12 of the patients had diffuse toxic goitre and the other 9 had multinodular toxic goitre. Serum levels of triiodothyronine (T3), thyroxine (T4), thyrotropine (TSH), SHBG, total testosterone, DHEA-SO4 were investigated. 26 persons of euthroidic population in the same age group were chosen to form the control group. Serum levels of SHBG (p<0.001), total testosterone (p<0.05) and DHEA-SO4 were found higher which is statistically significant. In hyperthyroidism, thyroid hormones directly stimulate hepatic synthesis of SHBG and SHBG-dependent testosterone levels (39,40,41,42). This stimulation causes the increase in SHBG and total testosterone levels. There is no study which investigates the effect on DHEA-SO4 levels. Metabolism of several hormones increases in hyperthyroidism (10). The number of ACTH peak for the stabilisation of cortisole levels can also increase in hyperthyroidism. It is also known that ACTH increases the secretion of DHEA-SO4 from adrenal gland (18). According to these knowledges, we can point out that ACTH peak increase the secretion and serum levels of DHEA- SC`4İn hyperthyroidism. Another factor for the increase in DHEA-S04 levels may be mean BMI and age of hyperthyroidic patients are lover in comparison with the control group. 28
